You can find the medical blogosphere’s best next week at Covert Rationing. We’ve taken a different approach this week to organizing Grand Rounds. You can find all the submissions below in this post.
By Christopher PrebleA wave of violence spread across Iraq today with 70 dead and some 300 injured. Iraqi security forces are blaming al Qaida affiliates, but no group has officially claimed responsibility. The New York Times puts the events in context:
Coming a little less than two weeks after the Iraqi government said it would negotiate with the United States about keeping some of its 48,000 troops here after the end of the year, the violence raised significant questions about the capabilities of the Iraqi security forces.
This is indeed a tragic loss of life, but this level of violence actually has become less common and usually occurs when the Iraqi government is making important decisions on the future of the country and U.S. troop presence. Each time a bomb is detonated in Iraq, comm...

Dr. Deb is a psychologist and practicing psychoanalyst who just published her book: Living with Depression: Why Biology and Biography Matter along the Path to Hope and Healing. She also did a wonderful job with the new grand round of medblogs.
Grand Rounds is a weekly round up of the best health blog posts on the Internet. Each week a different blogger takes turns hosting – me this time around – and summarizes the submissions of the week.
As a music lover, I thought I’d give Grand Rounds a vintage vinyl feel. So please make sure your phonographs are ready to go.
Enjoy the read of the summary and check out what seems interesting

By Christopher PrebleThe budget deal announced last night offers two sets of potential cuts in military spending.
The first set of potential cuts, created by the budget caps, target “security” spending. That includes the Pentagon, State, foreign aid, the Department of Homeland Security and Veterans (the discretionary portion of Veterans spending, to be precise). The deal caps “security” spending at $684 billion for this fiscal year and $686 for the next. That requires little pain; the 2012 security cap is only $5 billion below what we’ll spend on those categories in fiscal 2011. The White House claims that the caps will generate $350 billion in savings from base defense spending for ten years. They get there, dubiously, by projecting security spending at the capped le...

Similar to past grand challenges, which focused on infectious diseases and chronic, noncommunicable diseases, this initiative seeks to build a community of funders dedicated to supporting research that will significantly improve the lives of people living with MNS disorders within the next 10 years.“

I never considered myself an athlete. My twin sister grew up with the reputation of being the tomboy of the family, the sporty one who participated in soccer and other organized sports. I was the brain and artsy one, who spent more time practicing my scales and arpeggios on our baby grand piano and perfecting pirouettes in the dance studio. I was intimidated by sports. And I found that I had absolutely no coordination once you threw a ball into the competition. So out were softball, volleyball, soccer, and pretty much every other sport.
I swam during the summer and for my high school, and I started running in junior high, but just to lose enough weight to stop my period (I was a tad anorexic). I continued jogging and swimming through college into early adulthood. But just to stay in shape....

By Christopher PrebleFormer Minnesota governor Tim Pawlenty’s brief visit to Cato yesterday elicited some snide commentary in the blogosphere, especially this piece by the Huffington Post’s Jon Ward. Ward notes how the just-declared presidential candidate has been pretty adept at annoying audiences with his answers to questions. This one rankled the questioner, and a number of others in the auditorium.
I’m not one who is going to stand before you and say we should cut the defense budget.

I’m not for shrinking America’s presence in the world. I’m for making sure that America remains the world leader, not becoming second or third or fourth in the list.

By Christopher PrebleThe report that Leon Panetta will be appointed Secretary of Defense, and Gen. David Petraeus will become the new CIA director, does not come as a huge surprise. But I worry that President Obama’s decision to fill these positions from within his administration signals an unwillingness to rethink U.S. foreign policy. Such a reevaluation is desperately needed.
Leon Panetta brings some experience in national security affairs to DoD, including his stints at CIA and on Capitol Hill, and as a member of the Iraq Study Group. His more relevant experience, however, may be as Director of the Office of Management and Budget in the Clinton administration. Bob Gates effectively shielded the Pentagon from spending cuts, but that merely postponed the reckoning that Panetta ...

By Christopher PrebleFor all the boldness of Rep. Paul Ryan’s proposal to reduce projected federal expenditures by $6 trillion, an initiative that I support, the Pentagon’s budget emerges essentially unscathed in Ryan’s plan. This is a mistake on both fiscal and strategic grounds. Significant cuts in military spending must be on the table as the nation struggles to close its fiscal gap without saddling individuals and businesses with burdensome taxes and future generations with debt. Such cuts will also force a reappraisal of our military’s roles and missions that is long overdue.
The Pentagon’s base budget has nearly doubled during the past decade. Throw in the costs of the wars in Iraq and Afghanistan, plus nuclear weapons spending in the Department of Energy, and a smattering ...

A new blog has entered the medical world. She’s only a couple months old, but she has an awesome name: happy internist. happy internist shows us all how to die healthy:
my patient saw her gynecologist. he told her to eat right, get lots of exercise, and lose weight. that way, he said, you can die healthy.
What a great quote. It’s called finishing strong. Given what I know about the incredible pain and suffering I witness everyday from self induced disease, dying healthy is a goal worth living for. Death is inevitable. Dying healthy takes hard work and personal sacrifice.
She was discovered at this week’s Grand Rounds, where Dr Val has done an excellent job of organizing the best of this week’s Internet medical offerings.

This brand new Grand Round is up at Better Health

When I host Grand Rounds I like to organize the posts into emotion categories – kind of the way that movies are categorized into “drama, action, comedy, etc.” This achieves two goals: first, readers have the option of starting with their favorite emotion – maybe you need a “pick me up” or maybe you’re looking for something to get your blood pumping? Second, I get a general feeling for how my fellow bloggers are feeling these days. Judging from the volume of posts in each category, it seems that the majority of you are either surprised or outraged!

By Christopher PrebleOn Monday, Defense Secretary Robert Gates, speaking in Kabul, stated that the United States “will be well-positioned to begin drawing down some U.S. and coalition forces this July.” But as Greg Jaffe of the Washington Post reports, the planned reductions likely wouldn’t lead to a major change in the U.S. mission in Afghanistan. Indeed, even as Gates is stating that the United States will adhere to its date to begin withdrawing troops, negotiations are in the works that could establish a long-term security presence for the U.S. beyond 2014 and might include permanent military bases.
Secretary Gates and General Petraeus both claim progress in Afghanistan. But their concepts of progress are murky and exist within a strategy that has never had clearly defined obj...

By Christopher PrebleIn testimony yesterday before the House Armed Service Committee, Defense Secretary Robert Gates stated that the United States has an “interest” in keeping troops in Iraq past the agreed date of withdrawal, December 31, 2011. Rep. Duncan Hunter (R-Calif.) pressed Gates by asking:
How can we maintain all of these gains that we've made through so much effort if we only have 150 people there and we don't have any military there whatsoever," Hunter asked. "We'd have more military in Western European countries at that point than we'd have in Iraq, one of the most central states, as everybody knows, in the Middle East?
The logic of Rep. Duncan’s question provides some interesting context. His logic implies that the thousands of U.S. troops stationed in wealthy, develo...

By Christopher PreblePresident Obama might want it to appear as though he is reining in defense spending with his budget submission for FY 2012, but his approach to the Pentagon’s budget reveals the opposite.
Perhaps the president hopes that his adoption of the faux cuts that Secretary Gates put on the table last month will be seen as responsible. Perhaps he is taking a prudent first step and signaling to the military, and its suppliers and contractors, that the days of double-digit increases are over. That may be; but far deeper cuts are warranted. . If the president had truly wanted to send a signal, he would have followed the advice of his own deficit reduction commission and endorsed far deeper cuts in military spending.
The Department of Defense will spend $78 billion less over the ...

By Justin LoganVia Erik Voeten, Thanassis Cambanis has a long piece in Sunday's Boston Globe about academic critics of America's bipartisan grand strategy. Cambanis rightly points out that Republicans and Democrats basically agree about American strategy, and spend all their time haggling over price and implementation. By contrast, there is a burgeoning group of critics in the academy who disagree:
[The critics'] call for a humbler foreign policy hasn’t gained much of a hearing with the foreign policy elite, and is hardly talked about in mainstream circles. They question many of America’s basic habits and reflexes, at a time when it’s increasingly clear that the “long war” has not eliminated the threat of terrorism or neutralized rogue states and their nuclear black market.

By Christopher PrebleThe calls for cutting the federal budget continue to build in Congress as the new GOP members try to make good on their promise to rein in the deficit. And, right on time, the latest issue of the Weekly Standard features an article by Robert Kagan critiquing the chorus of calls for cuts to military spending.
I think Kagan’s critique is reasonably fair, certainly more so than others of the recent past. But his basic premise, that national security spending is unrelated to the national debt, simply is not true. At the The Skeptics, I address this:
It is of course true that entitlements and mandatory spending pose the greatest threat to the nation’s fiscal health, but $700+ billion [in defense spending] isn’t chump change. The question of what we should spe...

Like all professionals, doctors have a certain image about themselves . Similarly, patients have expectations of their doctors and expect them to conform to their mental model of how a doctor should behave.Life was much easier in the past when both patients and doctors expected doctors to behave as demi-Gods. The doctor was a shaman who was considered to be a healer who had been inspired by divine powers which he could use to help the sick to get better. In modern society, however, things have changed considerably; and few patients will treat their doctors as God-like figures ( and I feel most doctors would also be very uncomfortable in this role !)What roles do doctors adopt today ? These are primarily three, as articulated so well by the British economist, Julian Le Grand. We perceive doc...

By David RittgersNew Jersey Governor Chris Christie has commuted the seven-year sentence of Brian Aitken, the man wrongfully convicted on firearms charges under that state’s draconian gun laws. Good.
While a full pardon seems more appropriate – the judge in this case should have given the jury instructions on the “moving exception” that protected Aitken – this is at least recognition of an injustice and relief for one man and his family.
The New Jersey state judicial system’s webpage describes the grand jury’s function as “a screening mechanism to protect citizens from unfounded charges.” That didn’t happen in this case. For more on this phenomenon, read this Cato Policy Analysis, “A Grand Façade: How the Grand Jury Was Captured by the Government.”

By Christopher PreblePresident Obama’s Fiscal Commission’s report is out and they have wisely kept military spending on the table. Having not seen the accompanying list of specific cuts, it seems that rather than micromanage DoD’s decisions with respect to which weapons systems to cut or keep, the commissioners have laid down a different marker: find the cuts that make sense, but understand that the business-as-usual of the past decade is over.
The report fixes on a number of spending cuts and reforms that Benjamin Friedman and I call for in the Cato Policy Analysis “Budgetary Savings from Military Restraint” including cuts to the civilian workforce (see recommendation 1.10.4). They also hold fast to the proposition that all spending must be on the table, and reject out of ha...

The Thanksgiving 2010 edition of Grand Rounds is up at Amanda Brown’s facebook page.
The theme is gratitude. It’s probably the first time the Grand Round is on a Facebook page. So go over to Amanda Brown for an excellent read and selection of posts from Medblogs.

At this time of year, I always stop and think about what I’m thankful for, and this year I’d say the medical blogging community is definitely on my list.

In a high-profile paper in the September issue of Health Affairs, Thorson and coworkers showed that the care at St. Mary’s Hospital in Grand Junction, CO was superior to that of 20 other unnamed hospitals. Grand Junction is, of course the smal town in SW Colorado that became famous when President Obama visited there during the health care reform debates during the summer of 2009, and here’s what he said:
“Hello, Grand Junction! It’s great to be back in Southwest Colorado. Here in Grand Junction, you know that lowering costs is possible if you put in place smarter incentives; if you think about how to treat people, not just illnesses. That’s what the medical community in this city did; now you are getting better results while wasting less money.”
So, Grand Junction, a town of 58...

Welcome to the Veterans Day edition of Grand Rounds. Elvis and I are delighted that you dropped by. The King is very excited today because we are saluting celebrities that have served in the Armed Forces. Elvis said that his days in the army were memorable. I imagine having Life Magazine take your picture while you’re sitting in your underwear would be a memorable experience. My co-host and I want to thank everyone for their submissions, and we especially want to thank Dr. Nick Genes for allowing us to hold Grand Rounds at Nurse Ratched’s Place.
Elvis told me that the doctor gave him a lot of shots right after that underwear picture was taken. The King knows that vaccinations are important and he wants everyone to get their shots. Dr. Val from Better Healt...

By David RittgersBuilding on Ilya Shapiro’s post on the sealed grand jury proceedings against Siobhan Reynolds, founder of the Pain Relief Network, and the sealed Reason Foundation/Institute for Justice amicus brief, here is some more background on the Wichita witch hunt:
The U.S. Attorney’s Office in Wichita, Kansas, indicted physician Stephen Schneider and his wife, Linda, a nurse, for illegal drug trafficking in December 2007. Reynolds found an eerie parallel between Schneider’s case and the prosecution that denied her husband pain medication, so she took action. Her public relations campaign on behalf of Dr. Schneider so annoyed Assistant U.S. Attorney Tanya Treadway that Treadway sought a gag order to bar Reynolds’s advocacy. The presiding judge denied the gag order.

By Christopher PrebleYesterday, Prime Minister David Cameron announced Britain’s biggest defense cuts since World War II. The cuts affect the British military across the board.
The Army will shed 7,000 troops; the Royal Navy and Royal Air Force will each lose 5,000 personnel; the total workforce in the Ministry of Defence, including civilians, will contract by 42,000. The Navy’s destroyer fleet will shrink from 23 to 19. Two aircraft carriers — already under construction — will be completed, but one of the two will be either mothballed or sold within a few years. Whether the one remaining flattop in the British fleet will actually deploy with an operational fixed-wing aircraft is an open question. They’ve decided to jettison their Harriers; a technological marvel ...

By Christopher PrebleToday ForeignPolicy.com has a feature article examining possible “Plan B’s for Obama,” with contributions coming from numerous experts. My contribution to the feature is titled “Cut (Really Cut) Military Spending.”
It is time for President Obama and the administration to finally notice the increasing calls—from across the political spectrum—that the Pentagon’s budget should not be off limits when reducing the deficit. From the Foreign Policy article:
Despite all the hype about Defense Secretary Robert Gates and his cuts of big-ticket military projects, the Pentagon’s $680 billion budget is actually slated to increase in coming years. This is unconscionable at a time when taxpayers are under enormous stress and when the U.S. government must r...

Most people know that one of the keys to success in relationships is good listening.
Experts tell us to use “active” listening, “I messages,” and open-ended questions. Articles urge us to stop talking when someone speaks, to use our body language effectively to encourage the other guy, and to work to understand what is meant as well as what is said. We’ve been told that men are from Mars and women are from Venus and we’ve been taught how to translate the gender languages. Yet despite all that, developing good listening skills continues to be a challenge for some people.
Generally, it’s better to emphasize the positive and teach folks useful skills. But at least some people some of the time find it equally useful to have the negative pointed out and explained. They want guidel...

Space Food: The Best and Worst (Boing Boing)

Astronaut foods during the Gemini and Apollo programs were highly processed, because “low-residue” food meant fewer encounters with the dread fecal bag. To prevent crumbs, which could float into eyes and instrumentation panels, many foods – even “sandwiches” — took the form of bite-sized cubes lacquered with waxy, congealed oils. Rarely has anything so cute been so loathed. The coating stuck to the roof of the mouth and the cubes had to be rehydrated by “holding in the mouth for ten seconds.”

Sydney J. Freedberg Jr. of NationalJournal.com asks:
The U.S. military is already unaffordable — and yet it needs to be larger to sustain America’s global leadership, especially in the face of a rising China. That’s the bottom line from a congressionally chartered bipartisan panel, co-chaired by Stephen Hadley, George W.
Better Health’s Grand Rounds this week is hosted by the ever-so-crafty Life in the Fast Lane team of Australian physicians at the Utopian College of Emergency for Medicine.
These docs “take great pleasure in sharing their medical experiences, clinical knowledge and insights into waiting-room medicine with health-conscious technophiles to facilitate the learning process by providing diverse and hopefully entertaining reading material.” It’s always worth a read (and a chuckle), no doubt.
With the theme of ”Killer Posts” (just a hint — hate to blow the surprise), this edition of Grand Rounds is sure to educate in more ways than one! Experience it HERE. (Source: Better Health)

Dr. Toni Brayer has a guest post at ACPInternist about how insurance companies are killing primary care. I would like every Grand Rounds Reader to take a look at this and think very hard about what they will support in terms of insurance industry oversight. This post was first published on Dr. Brayer’s blog EverythingHealth.net.

It’s not just the soccer world cup that’s on South African shores, but the great Grand Rounds (something that some would say is far more important than the world cup — okay, only one guy would actually say that, and he’s in a psychiatric institution in Outer Mongolia) is also presently hosted in South Africa! Proudly South African!
Time to see what the bloggers have dished up for us this week. (more…)

June 27 or 28 was Paul Bunyan Day (the date varies regionally). While the fictional lumberjack isn’t usually known as the most eco-friendly hero, his myths include more than just chopping down forests. Legend has it that Bunyan created the Grand Canyon, the Great Lakes, and Mount Hood.

Dr. Elaine Schattner of Medical Lessons is the host for this week’s Grand Rounds. It’s the “customer service in healthcare” edition. You can read this week’s edition here. Learning about medicine is a lifelong endeavor whether you’re a patient, a doctor, a caregiver, a hospital administrator or, perhaps, even an insurance company executive. In today’s Grand Rounds, we’ve an array of eleven perspectives that, directly or indirectly, bear on the suggested theme of education. If there’s a motif that emerged unsolicited this week, it’s empathy, a term highlighted in the titles of two submitted posts: In Glass Hospital, Dr. John Schumann considers what motivates health care workers in a thoughtful post, Finding Empathy. The submissions covered a wide-range of topics; from surgical procedures to how the progression of 80’s music mirrors the evolution of birth control (seriously). ….. Thanks again to everyone that participated and I hope you all enjoy this week’s picks of the best medical blog reads as much as I did! ………………………………………. I've had a rather interesting time compiling this post for this edition of Grand Rounds. I must say the motive for the theme being humour and laughter was purely selfish. This is the first time I am hosting here and I knew that if I had to keep up the good work of the previous hosts I would have to be totally involved with the selection process of so many many fantastic entries that this event brings on! The only way to screen them would be to enjoy reading every bit and what better way to do that than over a laugh. So, in no particular order, here are this week's lol posts!………. ……………………………...

FIFA WORLD CUP – SOUTH AFRICA 2010

I would like to express my gratitude to Jeffrey Leow of Vagus Surgicalis (Australian medical student with lots of interest and knowledge of surgery) and the creator of this Great Carnival. Australia is in Group D.
Bongi, a lucky SouthAfrican general surgeon who will host the FIFA World Cup in 12 days, shares a story that will push you to learn Afrikaans. South Africa will be head of the Group A and will play with México, Uruguay and France.
rlbates, my favorite (female) plastic surgeon who lives to sew, wrote an excellent review of Scalp avulsion injuries. USA will play against England, Algeria and Slovenia in Group C.

Obama’s National Security Strategy: Long on Rhetoric, Short on Change

By Christopher PrebleThe key theme that the Obama administration wants us to take away from the National Security Strategy (PDF) is “burden sharing.” The United States, the document explains, can no longer afford to be the world’s sole policeman. We need capable and willing partners to preserve global peace and prosperity.
These are valid concerns. Unfortunately, the Obama administration lacks a vision for addressing them.
Real change can only come from a fundamental reorientation of our current approach. We need a new grand strategy predicated on restraint both at home and abroad. Instead, for all the talk of new directions, the Obama administration has given us more of the same.
In geopolitics, as in life, actions speak louder than words. So long as the United States sp...

It’s been a tough week for the anti-vaccine movement but an important week for pediatric health. Yesterday the UK’s General Medical Council announced that Andrew Wakefield, who’s fraudulent manipulation of data spawned the vaccine-autism cottage industry, would be ‘struck’ from the medical register. This action by the GMC is one more nail in the coffin of the man who has singlehandedly turned back the clock on two generations of pediatric public health.
Male Birth Control: Is Ultrasound The Key?

Finally men everywhere might have a birth control option that won’t rob them of the joys of living.
Scientists at the University of North Carolina at Chapel Hill may have discovered a cheap, convenient and noninvasive method of male birth control — ultrasound. The scientists believe that a single treatment can provide up to six months of infertility that is reversible.
The team has received a $100,000 Grand Challenges Explorations grant from the Bill and Melinda Gates Foundation for their work. If the project pans out, this could have an incredible impact on global health. (more…)

And, thank you for allowing me to make it a "women’s” theme as March is the month of International Women’s Day and Women’s History Month (here in the United States). The book contains 34 copper engravings. 31 of these magnificent medical illustrations were made by a mysterious man called Jan van Rymsdyk. ………………...

Pain and suffering Other Things Amanzi is profoundly affected by the death of a kidney transplant patient. It was the first time that the two of us collaborated, and I was very pleased with the end result. Most of the clever turns of phrase are Heather’s including the title, “The Wrong Manhood Test.” And I’m grateful to Harrison Moar and Charles Zakaib for helping me on Monday to sift through the gargantuan defense budget, and pull out the relevant facts.
Heather and I don’t agree on everything. We faced off at Bloggingheads.tv several months ago to discuss my book, The Power Problem, and I’m sure that we’ll continue to spar from time to time in the future. But the bottom line from the op ed is this:

By Christopher PrebleAs usual, Ben Friedman beat me to the punch regarding the Quadrennial Defense Review (QDR) (.pdf), and, as usual again, he nails it.
I do see some value in the exercise, however. So let’s not “forget it” just yet.
By constructing a rationale to justify our existing defense posture, and providing a blueprint for force planning into the future, the QDR can be particularly useful for taking on some sacred cows. For example, the proposals to cancel the CG(X) cruiser, shut down production of the C-17 and the F-22, restructuring the DDG-1000 destroyer and the Future Combat Systems program, are sure to rile up members of Congress who continue to treat the defense budget as just another vehicle for dispensing pork barrel goodies to a handful of constituent...

I’ve included the submitted posts and some posts from MedLiblogs I frequenty visit, here it goes at random:
Mesh terms as used in PubMed for medical literature searches are a kind of tags. MeSH terms will tell you how a particular issue is talked about and labeled.
My new post on Politics Daily / Woman Up:
On Tuesday afternoon, a young Toronto singer-songwriter named Taylor Mitchell was walking the beautiful Skyline Trail in Cape Breton Highlands National Park when two coyotes attacked her.
A nearby hiker heard her cries and came to her aid. Mitchell was airlifted to a hospital, but she died the next morning, on October 28.
Had she lived, in three weeks Mitchell would have found out whether or not she won a Canadian Folk Music Award. She’d been nominated in the category of Young Performer of the Year. She was 19 years old.
Cape Breton officials insisted an attack by coyotes is highly irregular, since they usually shy away from humans….

The discussion about H1N1 flu and vaccination is another topic leading to several excellent posts.

It's been very crazy clinically this week, and I'm soon to head off to Philly for the CABANA trial investigator meeting, but I wanted to share a few key links before I left:First, Change of Shift, the best posts of the week from our nurses is up over at Emergiblog.Next, Paul Levy, President and CEO of Beth Israel Deaconess hospital has a nice series of videos covering the culture of safety and how their hospital is making their safety data transparent, using patient advocates, and how patients there keep in touch with friends and family. Good stuff. Just try to look past the J&J ads. Finally, if you haven't done it yet, shoot over to Medgadget to vote for who you think will be the next nobel prize recipients.-WesMusings of a cardiologist and cardiac electrophysiologist.
