            <description>I&amp;#8217;m honored to be hosting Grand Rounds for the fourth time. This is not the GR theme today, but I did want to mention that it&amp;#8217;s Doctor&amp;#8217;s Day tomorrow in the United States. Doctor&amp;#8217;s Day was first observed on March 30, 1933. Eudora Brown Almond, wife of Dr. Charles Almond, decided to set aside a day to honor physicians.
The red carnation is commonly used as the symbolic flower for National Doctors Day. In 1990, law was passed designating March 30 as &amp;#8220;National Doctors Day.&amp;#8221; Big shout out to all my physician colleagues out there!

            <description>It’s been a while since I’ve attended a conventional medical Grand Rounds. These were events where a medical luminary would fly in to give a medical audience a state-of-the-art presentation on a medical subject. Ideally, the speaker was a thought leader and a researcher on the issue.
These presentations were usually not a demonstration of the virtue of humility. We physicians, as a class, have generous egos. Academic physicians occupy a higher rung on the ego ladder. Medical Grand Rounders (MGRs), who are on the GR speaking circuit, often must bring their own ladders to assure they will be able to reach their desired atmospheric height.

And now, (drumroll please), my second attempt at Fat Doctor&amp;#8217;s Grand Rounds. This is a Mayan calendar. The last year has been&amp;#8230;special. A year ago, I was mired in a bad mood which turned into eight bilateral frontal lobe CVAs a week or so later. Thus, the first FDGR was devoid of the witty brilliance I had planned when taking on the task. I see now that I even screwed up the date on that post. Ha!

Since then, I&amp;#8217;ve enjoyed the straightforward approach to GR, so I will stick to the basics. We had 39 submissions this time, many of them before the posted deadline! Welcome!

I have the honor of hosting Grand Rounds for the second time! I can&amp;#8217;t believe it has already been a year since the last time I put all these great medical posts together. Time does indeed fly when you are blogging away.

I thought of many different ways to &amp;#8220;spice up&amp;#8221; Grand Rounds. After much deliberation, I finally decided to&amp;#8221; stick to the basics&amp;#8221; and just present &amp;#8220;the facts&amp;#8221;, as I did many times before as a resident during our weekly nerve-wracking, anxiety-inducing Urology Grand Rounds.

            <description>There is a nice thing Nick Genes does for the Grand Round hosters. He writes a nice monograph with a Q&amp;A with that weeks&amp;#8217; Grand Rounds host. It&amp;#8217;s called &amp;#8216;Prerounds&amp;#8217;, and it used to come out prior to Grand Rounds, but now it comes out afterward. Here&amp;#8217;s mine:

I think the reason it&amp;#8217;s not &amp;#8216;pre&amp;#8217; anymore was difficulties with the Grand Rounds link. They want one prior to publishing their prerounds, and people (like me) have screwed that up in the past, so maybe that&amp;#8217;s why.

            <description>Thanks for coming!&amp;nbsp;&amp;nbsp;It's an&amp;nbsp;honor to be the first four-time host.&amp;nbsp; As for how much things change, here's the first time I hosted (November 9, 2004, #7). The biggest change is the number of blogs, hence submissions, and the quality of the submissions has increased every time.&amp;nbsp; Also, GR is no longer dominated by&amp;nbsp;a few&amp;nbsp;'bigger bloggers' but is now quite wide-open, hence the increasing quality and scope of the posts submitted.&amp;nbsp; By the way, I didn't have 200 posts when I made the announcement, I had 5 (the +/- 195 was meant to be a joke), but I did wind up with 60 submissions.
