Conclusion
Don’t accept an EHR that’s slow. Make sure that the EHR performs at a satisfactory level. I know of nothing that frustrates a clinic more than a slow EHR.

We’re back again with our ongoing series on Common EMR Implementation Issues. Seems like readers really liked my first entry in the series about Unexpected EHR Expenses. To be quite honest, I was really happy with how that post turned out myself. It’s one of the most comprehensive and useful posts I’ve written in the 5.5+ years I’ve been writing about EMR and EHR. Hopefully we can continue that trend.
Today’s Common EMR Implementation Problem: EHR Performance Issues

Followed by a *huff* and...

Well, it is safe to say that most of you think anonymous comments are a bad idea. I will leave them off for most likely permanently. If you want to comment, then sign up for a Google account. Stacey asked me to help her get out of debt last night. I was overjoyed to help – often worrying about her tendency to frivolously charge items on her many credit cards. She loves to shop for clothes and jewelry online. I told her it is going to take probably a year, but we could do it. Mom called me last night complaining about dad’s obsessive football watching habits. I smiled. I don’t much care for watching the games, but I will keep up with the scores on ESPN.com. Dad and my brother are just fanatical about it, though. I cooked mom’s spaghetti last night and it was delicious. I mad...

My father wasn’t too pleased, but what could he do? Cast his son aside despite all his faults and drunkenness? I had planned on going to Nashville to live. From reading “The Homeless Guy” I knew I could get a place to sleep, three meals a day, and social worker help. I would also have my full disability allotment to drink with. I wouldn’t have any expenses other than cigarettes and beer. It was sad, though, that my life had come t...

“Your hair looks pretty tonight,” dad said rubbing the back of my head as we sat on my couch. “You have hair just like my father’s.” “Why did you do that today about the orange drink?” I asked, diverting my father’s affections. “You carry it too far sometimes. You and mom can both go overboard. I have too few comforts left.” “I’m sorry,” my father replied kindly. “We just don’t want you to go back to the way you were. Do you realize how far you’ve come? You are like a different person – a completely different person. You are the son I lost years ago.” I sat quietly watching The Weather Channel waiting on my medications to take effect. Dad was very, very late tonight and it was almost midnight. I...

You walk into a health food store and find a flyer that warns of the hazards fluoridated water. What’s this all about? Fluoride is in half of the public water in the US. It is accepted by some, including the government, that fluoride deters tooth decay, thus it is good for public health. However, a study published in 2006 in the Journal of Evidence Based Dental Practice provided these facts:

Fluoride does not have to be delivered in the water supply
Fluoride does not have to be ingested to protect teeth
Caries has declined at the same rate in countries with/without fluoridated water
The public receives fluoride from sources other than public water supply
Studies show that fluoride can cause a list of adverse side effects

What You Should Know
Fluorine, the ionic form of fluoride, is use...

Saving lives? It’s all in a day’s work for a proactive dentist!
In this economy, dentists can increase their value to patients by offering dental-related, preventive services. Do you offer stem cell preservation? Do you look for signs of serious non-oral diseases during checkups? Do you treat obstructive sleep apnea and snoring? Do you offer early oral cancer detection? If not, maybe you should!
Stem cell preservation
In the past, stem cell preservation was isolated to extracting cells from a person’s umbilical cord at birth. Now, however, dentists can get in on saving lives! As a dentist, you can bank stem cells for your patients, which could actually save their lives down the road. Stem cells are used to heal bone, skin, cartilage, nerves, and muscles. They can treat diseas...

This may not be breaking news or even important information, but it’s definitely interesting.
An 18-year veteran dentist, Dr. Igor Tsarik of Siberia, has carved/sculpted about 300 teeth, many of which are on display. He uses a drill, heat, and lacquer to create his tooth art, which is a form of Japanese netsuke. Primary teeth are too soft for netsuke, so Dr. Tsarik uses extracted permanent teeth for his art.
How do patients react? Some think the tiny reliefs are really cool. Kids, in particular, appreciate them. However, one African woman fainted when she saw her own tooth transformed.

I am proud to present to you the openECGproject. Something I helped to start up and the thing that kept me away from my blog for almost two weeks.
The openECGproject is essentially an online community conceived around a simple, but challenging and worthy goal - to develop an open source hardware and software solution for electrocardiography. More precisely, to develop an open source, low cost, and clinically functional 12-lead PC-based ECG with interpretive software.
The idea was conceived to help those hospitals and doctors, mainly from the Third World, who cannot afford similar currently available and expensive commercial products. Development of a free alternative could, I believe, have a profound impact on health care not just in poor countries, but in others too.

Results of the 2007 British Academy of Cosmetic Dentistry audit show that our English neighbors across the pond really like their cosmetic dentistry. The BACD is a non-profit organization that promotes ethics and advancements in cosmetic dentistry. The long and short of the audit:

Female patients accounted for over half of all cosmetic dentistry cases.
Orthodontics is up 345%! This includes clear braces and traditional ortho. Removable braces make up over half of the cases.
Veneer cases held steady, but the number of male cases increased.

From most popular to least popular, cosmetic dentistry procedures fall in this order: white fillings; crowns, inlays, onlays; veneers; whitening; bridges; implants; orthodontics; gum contouring.

I have just installed a new Home-Office computer for Emmott on Technology. It is awesome…quad core dual monitors…total geek out.
However even for a big geek like me setting up a new computer is a tedious and frustrating task. However this time I used a slick product that made it much faster and easier. PC Mover from Laplink. I was able to connect the new and old machine with a USB cable and transfer all of my installed applications, all of my data and files and even all of my settings. It worked very well. Not perfect but well.
I still had several irritating issues, like Windows would not recognize my very standard HP printer and the new version of Outlook wouldn’t send out e-mail. But I have it all working now…so far :-). (Source: dental blog for dentists about dentistry)

I sheepishly walked into Goodwill filled with social anxiety. "Do y'all except volunteers?" I asked the brusque cashier, mustering up all my will. "Check with the manager. In the back. To your left," she told me as she pointed. Sadly, the manager told me they didn't have a volunteer program.
Last night I fell asleep dreaming of computers -- of RAM and Motherboards. Video cards and processors. It was a pleasant dream of one of my favorite hobbies besides model railroading. I dreamed of building computers and someday making a living putting together machines for clients. I once got hired by our local Catholic Church to update their computers. I was never a good business man and did it for cost. My then wife was exasperated with me. "We need the money," she would moan. "And you're giving out charity." It was more about my love of the hobby rather than the dollars I could earn. I will never forget the smile on the church's secretary's face when she booted up her now fast computer. She gave me a hug. It was the best payment I could ever get. While we are on the subject of my ex-wif...

I am a big proponent of multiple monitors everywhere in the dental office. See the PC Mag article linked below for instructions on how to do it. Here is my office manager Jackie with two monitors at the front desk.

I’m going to assume that you have Microsoft Windows XP or Windows Vista. Given that, you’ll need a graphics card or graphics subsystem that supports multiple displays. Any video card manufactured in the past three years—even the budget cards—can do that. But take a minute to refer to your PC’s manual if your system has only integrated graphics hardware—that is, a graphics adapter that’s built into your system’s motherboard chipset. Some integrated graphics systems can handle more than one display, but others can’t. The b...

D-Link Slashing Prices of 802.11n Routers to under $50?
According to DigiTimes, D-Link is in the process of slicing the price of its entry-level 802.11n routers to under $50. Those sort of price cuts may be hard to believe, but a quick search of Google turned up an indication that prices are indeed heading south: the DIR-615 Wireless-N router can be purchased from PC Connection for $59.95, and at TigerDirect for $59.99. (Both are fairly well-regarded shopping sites, but be sure and check ResellerRatings to help determine if a cheap price is too good to be true.) I don’t believe PC Magazine has reviewed the DIR-615, but the DIR-625 is being sold for about $75 on TigerDirect, where it has received some nice reviews. Stepping up to the DIR-655 will cost you about $130 at NewEgg, ...

A simple and inexpensive, yet very powerful component to effective use of technology in the treatment room is multiple inputs. Input refers to any device, which allows the user to access the computer. The most common input devices are a keyboard and mouse. However there are other options as well such as light pens, touch pads, track balls, pen tablets, mini keyboards, voice and even bar code scanners.
When we first moved computers to the back we used the desktop model and had a single data entry point with a keyboard and mouse on the assistant’s side. As we developed the electronic chart and started using digital radiology, computerized notes, digital imaging and patient education we soon discovered we needed more input access.

I received an e-mail from Tom Block regarding a new ultra small computer he has been putting in dental offices. You can read what he has to say below and contact him at www.blockdata.net

“These are awesome and very versatile machines, I think they will transform dental computing in the near future. We have them installed in four different offices to date and our clients love the size and mounting options. Plus they are just plain cool (literally) at 95 Watts.
We did a show special featuring the 945-D with slot loading DVD-RW, Intel 2.0GHz Core 2 Duo CPU 667MHz FSB, Intel 945 Chipset, 2GB DDR-2 SODIMM 667MHz RAM, 80GB HDD, and XP Pro for $999.00.”

Hard Drive: The hard drive is the main storage memory of the computer. Hard drive storage is measured in Gigabytes or GBs. In computer talk this is called a gig. The higher the GB of the hard drive the more total storage you have.
Random Access Memory: This is abbreviated as RAM. RAM was measured in Megabytes, or MB. New computers are now available with 1000MB which is a gigabyte. RAM is the active memory a computer has to process or manipulate data. The higher the RAM number the more memory is available. More RAM will allow you to do more things on the computer simultaneously and faster. Having too little RAM is a common cause of computer freezes or crashes. If this happens then you use the alternative RAM definition, Rarely Adequate Memory.

Bottom Line:The Netgear ReadyNAS NV+ is an excellent combination of small-business RAID muscle, gigabit throughput, and home-oriented media features. It’s expensive, but worth the price if abundant storage, fast transfers, and RAID reliability are what you need.
Pros: Fast transfers. Supports the three major desktop OSs. Granular performance-tweaking settings. Well-behaved network client. Many features.
Cons: Some parts of setup are tricky. Attached USB hard drives must be FAT or FAT32, not NTFS. Expensive.

OATS Stands for Office Automation Technology Solutions. This is the acronym for a new service from Sullivan Schein Dental. OATS will provide hardware and IT (Information Technology) support for dentists through the existing Sullivan Schein dealer network.
Dentists will have a single source for hardware purchase, set up, service and support. It will eliminate the blame game between hardware and software vendors and help dentists get the right computers and keep them running efficiently.
OATS will launch within a limited area by the end of the year. I will provide more information as the project progresses. (Source: dental blog for dentists about dentistry)

Moore’s Law states that computing power will double every eighteen months. He has been right for forty years. This means that it is foolish to shop for a computer that will last for the rest of your practice life. It is foolish to worry about filling up your server hard drive in four or five years. It is foolish because you will be able to upgrade to much better equipment at a lower expense in a relatively short time.
So the lesson is buy what you need to do a good job in the office today. Don’t try and buy a computer for what you will be doing three years from now. Because in three years there will be computers four times better than what you can get today. (Source: dental blog for dentists about dentistry)

As configured it is ideal for a typical dental office with RAID 1 dual 160GB SATA hard drives 1GB RAM and the newest dual core processors. The price also includes Microsoft small business server software.
I have seen many hardware proposals for dental offices with servers specked at $7,000 or more. You don’t need that. This $1500 server will run most dental offices very well.

Most high quality hardware is extremely reliable and will give years of service. When a computer or a computer component stops working the first things to check are low tech. Check to see that the connections are secure. Is the mouse plugged in? Is the power cord tight? Is it plugged into the wall? Is the power Strip or UPS turned on? Is the network connection tight? Is the network switch plugged in and working? And most commonly are the batteries good?
If all the basic low tech items check out next try and replace the non working component to see if that’s the problem. Try another mouse, change the keyboard or replace the monitor.
At this point it is time to call the technician. If a mission critical piece of dental equipment fails, such as your compressor or central vac most dent...

The future vision for the dental office will be computers assigned not to rooms but to people. Each team member and the dentist will be able to roam freely about the office with a portable computer that is always connected wirelessly to the office network and beyond to the Internet.
The tablet PC drives this transformation. There are two basic tablet styles, slates and convertibles. The convertible has a keyboard the slate does not. Convertibles seem to be more useful while maintaining the extreme portability of the tablet design.
A key feature to tablets is the use of touch screens and inking. That means a user can easily enter information by tapping on the screen with one hand while holding the tablet in another. Ink allows users to write or draw on the screen just as if it was a piece...

Computers in the treatment rooms are not just simple data input and retrieval devices. They are also communication, diagnosis, education and entertainment centers. They are used by the dentist, the dental assistant and the even the dental patient. Accommodating all these uses and users means that the computer components need to flexible and movable.
Multiple monitors use a function of Windows which allows two or more monitors to run off of one computer. Dual monitors aren’t really new; I have been using them and telling other dentists about the concept since1998. What is new is that program designers are building in dual monitor functionality and hardware vendors, such as Dell are offering multiple monitors as a common upgrade.

Many dentists are so afraid of a possible computer failure that they refuse to use it, just in case. That is silly. Ask yourself, what if my car breaks down on the way to the office? What if the power goes out? What if the air compressor dies? The fact is that we rely on all kinds of systems that possibly could fail. A mission critical system failure, like a power outage or a compressor breakdown could put you out of business for a short time. A computer failure is the same. However the best way to protect yourself from a computer failure is to use redundant data storage systems, make daily off site back ups and most importantly have a local computer technician who you can rely on to be there that day, to get the office back to work.
If you collect business cards, you’ll want a way to organize their information—and typing it into a database by hand doesn’t cut it anymore.
By M. David Stone No matter what you do for a living, odds are that if you meet with clients or prospective customers, you exchange business cards and slowly (or not so slowly) build a list of contacts. You may even have a stack of cards sitting on your desk right now, waiting for you to type the names, addresses, and phone numbers into your contact database. Or maybe you don’t let them accumulate—probably because you’ve already discovered the joys of having a business card scanner.Business card scanners, and the software that comes with them, aren’t perfect, but they speed up the job of converting business ...

When you’re buying a new printer, there are two things to consider: the printer’s price and the cost of operation. Will you save more in the long run with a cheap printer and expensive ink, or vice versa? It gets quite confusing. To help you out we’ve put together a handy guide that lays out everything you need to know about printer ink, from which company’s is best to how much a particular printer will cost you per page printed

One of the amazing new products I had a chance to preview at the ADA was a new digital impression device from Brontes. The system is now being developed by 3M which purchased Brontes earlier this year. The actual digital impression device is in clinical beta testing and not yet available but is expected to be on the market soon.
This will compete with the CaDent product which I recently mentioned. The Brontes has a different system to capture the digital impression and can take a full arch. This is one of the most exciting developments since digital x-ray and will in my mind have an even greater impact on the profession.
	Stay tuned.

Somewhere between a PDA and a laptop are some new mobile devices; they’re not as big as laptops but still not as small as a smartphone or PDA. They’ve been called sub-laptops or mega-PDAs; they’re the “middle things.” These devices are smaller, sleeker, and may be capable of doing some of the things you need it to do in a dental office.

The following is From Lorne Lavine. He has a nice essay on choosing computer hardware in the latest Dentists Network Newsletter. I agree with all his suggestions which just shows me he is a sharp guy

It’s important to understand that there is a reason that operatory computers cost more: it’s because they have higher requirements than the computer you use at home to surf online and check email. Be very cautious of those ‘special” deals you see offered all the time from computer vendors. In almost all cases, the computers are underpowered and have poor warranties. The only thing you really gain from a cheap computer is the fact that you’ll need to replace it sooner: hardly a “win-win” in my book.
An interesting (ugly but cool) product is the Power Squid.I am constantly frustrated by bulky plug in transformers hogging all the space on my power strip. Right now I have eight of them under my desk connected to the cable modem (2), speakers, routers, phone chargers and so on. The squid makes all of the outlets from the power strip or surge protector available. (Source: dental blog for dentists about dentistry)

High-performance LCD Monitors - Review by PC Magazine

PC Magazine has a complete rundown of LCD monitors. For diagnosis in dentistry we need good resolution. That is best achieved with a small dot pitch so that the maximum number of pixels are shown. The Samsung mentioned in this article has a 2560 x 1600 resolution. That converts to over 4GB of image.

Walk into any electronics store and you’ll see dozens of LCD monitors. With a wide range of screen sizes and feature sets, choosing the right one is no easy task. This is particularly true if you’re very demanding in terms of image quality. Fortunately, we’ve done the legwork, and have come across our share of amazing monitors. Here are a few standouts in displaying bright, sharp, and true-colored images.

Conventional mini towers can present a space problem in some areas; especially in the treatment rooms. For example we recently installed a new rear delivery cabinet like this one from DentalEz. It has a center console that looks like a perfect place for a computer. However the standard size computer tower wouldn’t fit. We solved the problem with a slim line tower. These towers are almost half the width of a conventional tower. See the link below for the Dell version of a slim tower.

In order to scan an x-ray the scanner must have a transperency adaptor. Many scanners have a small transperancy adaptor designed to scan a single 35mm slide. This is OK for a single x-ray but useless for a panoramic or a full mouth series. The Epsom 4990 listed below has an 8” x 10” adaptor and is priced at about $300. Be warned; some vendors will try and sell you a special x-ray scanner for well over $1,000.

This powerful performer scans multiple slides simultaneously, as well as negatives and photos. And, with its built-in transparency unit, it accommodates film up to 8″ x 10″.

I am instaling a dual monitor system at the front desk this week. The following is a link to PC World magazine to show you how it is done. I have used dual monitors in the treatment rooms since 1998. I currently have four monitors in my personal office! (It is waaaay cool). I will let you know in a few weeks how it is going up front with the new system.

Adding a second monitor can seriously boost your productivity, especially if you work with large images or spreadsheets. It’s also extremely simple. We’ll show you how to do it in this step-by-step guide.

You can take it with you after all. External storage is becoming sleek and svelte and insanely portable. We have three pocket hard drives to always keep your files on hand.
By Tony Hoffman Some external hard drives are bulky, hold massive amounts of data, and require their own power supply. Not so the new breed of portable hard drives. They’re pocketable—especially in winter. They draw all their power from your computer’s USB connection. And they hold far more data than a thumb drive. Pocket hard drives? If you have data to transfer, don’t leave home without one

Mirrored server hard drives are a great idea in the dental office. A mirrored hard drive also called a RAID array (Random Array of Inexpensive Disks) copies all the essential data instantly to a second hard drive. This gives you great protection against hardware failures.
RAID systems can be set up in five different arrays ranging from RAID 0 to RAID 5. In dentistry we do not need fancy (and expensive) SCSI RAID 5 systems. A simple (and inexpensive) IDE mirror works great.
Two vendors make IDE RAID controllers Arco form Florida www.arcoide.com and Accusys from Taiwan. www.accusysusa.com. An IDE system including two hard drives and installation should cost less than $500.

Buying Guide: 22-Inch LCDs Reviews by PC Magazine

Today’s LCDs are bigger and better than any we’ve seen before, and they can handle virtually any office or multimedia task you throw at them. Not long ago, desktop LCD monitors were designed almost exclusively with an eye toward business, targeting users with space constraints and those who generally worked with office applications and didn’t require a high-performance display. Not to mention that they were quite expensive. Think about it: Five years ago an 18-inch NEC MultiSync LCD monitor had a street price of $1,800. Things have changed. Widescreen models in particular are becoming the popular choice for businesses and consumers alike, enabling users to view multiple document pages at the same time or watch a DVD movie in its native aspect ratio on a big, bright panel....

It would also be a great way to transfer images or a patient record to a colleague you are meeting for lunch, or carrying around any data you want to keep safe and handy.

Don’t knock the VerticalMouse 3 because it doesn’t sport a kooky, odd façade like its ergonomic rivals. Evoluent’s sequel to the VerticalMouse 2 boasts an improved design and several new features, making it a very capable mouse in the hands of all types of users. The VerticalMouse 3 (VM3 for short) from Evoluent looks like a regular mouse turned 90 degrees clockwise. It orients your hand in a vertical, handshake position. The idea is to allow your arm to control the mouse in a more natural position, with the thumb up, as opposed to the not-as-healthy pronated position where the palms are facing down, like when you’re typing on a keyboard. Some doctors who specialize in ergonomics consider the vertical position preferable. And because the VM3 resembles a regular mou...

Most have recognized and are now familiar with the advantages of using a stereoscopic microscope for applications where loupes do not provide sufficient magnification.A stereo microscope allows you to judge depth and vary the level of magnification to the task at hand. Unfortunately, it also affects the way you work because it is required to be between you and the patient, limiting your field of view and limiting your choice of postures. To address these issues, StereoImaging has developed the DentiMag3D™.
This is just a dull Linux hardware post so I’ll cut to the chase: if you use Linux, never buy an ATI graphics card. I don’t know what I was thinking - normally I research this stuff pretty thoroughly, but it seems that 95% of Linux users have suffered the same fate. Having most stuff “just work” under Ubuntu has lulled me into a false sense of security.

Back when I was in graduate school, workstations from SGI and Sun Microsystems were the primary machines on which my colleagues and I did the majority of our computing, occasionally buying time at supercomputing facilities (SDSC for most of my work). Then the Linux cluster happened, and almost every decent computing facility I knew was doing the bulk of its work on a dedicated cluster or some sort of shared resource. Sun and SGI were going through bad times, while HP and IBM workstations were primarily being used for visualization and small local jobs. However, more recently, I’ve started seeing reports like this one about a powerful workstation from Appro. These x86 workstations contain multiple CPU’s and tons of memory. I wonder if the market for these workstations is growing...

It’s a matter of time before the drive(s) in your mission-critical desktop or workstation fail. The Velocity2 RAID Backup System is a “belt and suspenders” type of backup drive, but if you’re the kind of user who regularly burns out hard drives, it’s the drive you’re looking for.
Blazingly fast drive transfer. RAID 0, 1, JBOD, single-volume compliant. Relatively quiet. User-serviceable drives. Uses standard SATA drives as replacements. Includes comprehensive backup utility. Compact chassis. Quick and easy to configure. Tool-less drive replacement.
Quite pricey on a per-GB basis.

1. Update your system.Make sure that you are using the latest drivers for your hardware-and that you’ve installed all the Windows Updates-and check for updates to any applications you use, especially any that seem to be related to your current computer problem. Microsoft is always updating Windows with minor patches and bug fixes. Depending on whether you use Windows Automatic Updates, keeping the OS up to date may require some diligence on your part. In Windows XP, select Start | All Programs | Windows Update. (Other programs will often include update options on their Help or File menus.)
	2. Check your connections. This may sound obvious, but check all the cables to make sure they are plugged in securely. Don’t ignore this step. USB, printer, and serial cables have a way of wo...
