I recently treated two patients, one with chest pain and one without. The one without chest pain was much sicker with some dangerous arrhythmias, and while his initial cardiac enzymes in the ER were negative, the following morning his troponin was over 50. Yet he survives, so far, despite his advanced age.The one with chest pain didn't seem particularly sick at all, and his Cardiologist was unimpressed with my presentation. Stable ECG, negative cardiac enzymes, and atypical features in an anxious patient with a previously negative workup suggests maybe he could go home, said he. Fortunately the Hospitalist agreed to admit him after some negotiation. This patient expired the next day.Reading the electronic medical record of my admitted patients gives me perspective and feedback that is diff...

Administrators require emergency nurses to document a lot of crap that has nothing to do with the patient's actual emergency condition. We have domestic violence screening questions, tuberculosis screening questions, immunization status queries, and so on and so on. These add a significant amount of time and effort to the triage process. Fortunately, we now use electronic medical records which have little checkboxes for those questions in order to speed up the process.I'm not certain, but I suspect these boxes must be checked or else the triage note cannot be completed. The combination of these two faulty systems sometimes results in unintentional comedy:1) 6 month old with fever and cough. Denies suicidal ideation. (He is crying a lot though. Maybe he's depressed)2) 55 year old with CPR i...

Since I posted tips on being a student on placement, it was mentioned there should also be one on tips for university. This will largely be stolen from the comments on the placement one, editted and/or added to.

You should go to lectures/classes. Just relying on handouts and powerpoint sheets won’t do much. You’re not really likely to read it, and if you do, are you likely to follow it? The best time to get a proper understanding is in the classroom. There is bound to be at least 1 tit who consistently asks stupid questions and/or has a story to share about the current topic (often totally unrelated), but if you can get a word in, you should to clarify anything you are unsure about. You can also speak to the lecturer at the end or via email if you need to.

From the Age of Autism:

They need to start listening to parents’ concerns and take them seriously,” said Amy Carson from Moms Against Mercury. She and cofounder Angela Medlin came all the way from balmy North Carolina to freeze in front of the AAP, abetted by TACA and NAA Chicago, led by Chapter Development Director Karen McDonough. This was their fifth rally outside the AAP, timed to greet employees arriving at work and remind them that thousands of American families believe vaccines have triggered an epidemic of autism and other developmental and chronic health disorders.

And here is the proof positive of the teeming throngs of ‘thousands of American families’.

A couple of days ago, Brad Handley wrote a blog entry on Age of Autism called ‘DR. NANCY MINSHEW & ME: WHO’S CRAZY?’.

The temptation to reply with a one word answer starting in ‘y’ and ending in ‘ou’ is nearly irresistible so I’ll resist it. Let us instead examine Brad’s criteria for deciding on who is crazy and who is not between Dr Minshew and he.

I disagree with almost every single thing you have written or said about autism. Since we both can’t possibly be right, one of us has to be crazy. I’m scared to death it might be me. As a psychiatrist, I thought you could help.

Says Brad to Dr Minshew in an email. He then continues with:

It is maddening for parents like me that our “experts” can’t ag...

The Eli Stone thing is very closely following the ‘fallout’ trajectory of the MMR/Wakefield program (Hear The Silence)over here a few years ago in that the protagonists were virtually canonised and the evil medicos painted as uncaring, duplicitous swine.

If the path of trajectory continues to be followed then Eli Stone’s canonisation should enjoy a brief, bright blaze followed by a long slow loss of interest from the general public and he will be relegated to one more point of disagreement that only matters to Age of Autism readers.

However, I do wonder if the Legal Editor at Age of Autism might be coming ever so slightly detached from reality:

Thanks to Eli Stone, our new patron saint of autism, we are no longer “Lost” to the mainstream media.

	Well no...

A bad day for antivaccinationists: Yet another study fails to support an association between vaccines and neurodevelopmental disorders

The statement is plainly false. The study’s conclusions do not reflect the study’s data or the limitations of the study,

You’d think that at least A-CHAMP would correct that hanging comma at the end of the sentence there.

Sarcasm aside, the study’s conclusions do reflect the study’s data, quite well, as we will see, and A=CHAMP’s complaints boil down to the usual crank technique of cherry picking the evidence, combined perhaps with sour grapes.
And so, this was the week that the anti-vaccine/autism hypothesis got its first real airing in a public arena. Jenny McCarthy went on US TV and told her audience that her son was her science (quite possibly the silliest thing on the show since Tom Cruise’s couch/brain malfunction).

I’m going to level with you here. I don’t really care too much about Jenny McCarthy spouting on about the evils of vaccines. She’s not the first and she won’t be the last. Despite the raptures the anti-vaccination people are having over her appearance she wasn’t on Oprah because of her vaccine ideas.

This is what bothers me: she was on Oprah because she was famous. It scares the shit out of me that we can only apparently have a conversation about something after a celeb ha...

Now that the scientific evidence as presented by Stephen Bustin at the OAP has finally displayed to the world how utterly wrong you are about measles in the guts of autistic kids, when do you plan to issue an apology to all the children you and your colleagues needlessly scoped?

When do you plan to issue an apology to all the parents who believed you and who subject their children to outlandish autism ‘treatments’ that have resulted in both death and hospitalisation?

When do you plan to offer an apology to all the people who are now living in a UK where herd immunity teeters on a knife edge and the numbers of people hospitalised by measles grows every year?

	You have been proven scientifical...

Have you ever tried to talk to someone involved with autism from the “other” side about recovered children?

Typically, the conversation goes something like this:

Us: So you think autism is a genetic condition and biomedical intervention is quackery?

	Them: Yes, exactly.

Us: But what about the children who are recovered, how do you explain them?

Them: Well, they may have been misdiagnosed…

Us: Well what about the kids who have multiple diagnoses saying they were autistic and have been re-screened and no long...

Every god-damn quarter, without fail. Every single one. The CDDS data gets released – Rick Rollens releases his usual ‘full syndrome’ crapola and tells us how autism is still skyrocketing and does his best Chicken Little impression and David Kirby chews at the edges of the data to try and find something that will support the idea that thiomersal plays any kind of role in autism.

	Its getting really, really old.

Please let me say it to you one more time.

CDDS is not good source data for epidemiology. They say so themselves.

When CDDS refer to ‘full syndrome autism’ they are not, repeat, not referring to classical autism. See this form here? CDDS use it to record autism. It was designed in the 1970’s. The version in use today was last updated in 198...

Andrew Wakefield, MMR and….The Observer??

I have a category tag on this blog especially for the loons at the Daily Mail (Melanie Phillips et al) which I usually select whenever I write about Andrew Wakefield or the MMR because its invariably one of them doing the writing.

This time I was amazed to see that it was that usual bastion of intelligence and propriety, The Observer, that had decided to play the role of media dumbass. Obviously the mail and Private Eye are having an off day.

First up was Andrew Wakefield himself – comparing himself to Vaclav Havel no less he pontificates:

Wakefield told The Observer that he has no regrets for saying what he did in 1998 nor for continuing to seek to prove his view of MMR as the likeliest explanation for the rise in cases of autism in Britain. Almost every child health expert, t...

RFK Jr - Attack on sense

You can always tell when the mercury militia are hurting – they wheel out their biggest gun. The biggest gun they have in their armoury is RFK Jr, a man who seems to live on the reputation of his dead family members and not a lot else. Last time he dropped the biggest, steamingest, drippiest turd on the web in the fullsome (and entirely accurate) language of Orac.

Well, after reading RFK Jr’s latest verbal tantrum I think Orac might need to get out the Dynorod and try and flush this particular big shit in a small bowl away as quickly as possible.

	RFK Jr claims firstly that:

The poisonous public attacks on Katie Wright this week—for revealing that her autistic son Christian (grandson of NBC Chair Bob Wright), has recovered significant function after chelation treatme...

And so, as we approach the end of week one of the vaccine trial, its been truly fascinating to read (albeit a day behind my US counterparts) the ongoing proceedings.

One of the things that fascinated me was the culling of the ‘expert witness’ list. Before Monday – the start of the trial – the expert list comprised:

	Jim Adams PhD
Harland Austin D. Sc.
David S Baskin MD
Jeffrey Bradstreet M.D.
Richard Carlton Deth PhD
Mark Geier MD
M. Eric Gershwin MD
Phillippe Grandjean, Ph.D.
Sander Greenland, Dr. PH
Boyd E. Hayley, Ph D
Robert Hirsch PhD
Arthur Krigsman MD
Cathy A Lally, Master P.H.
Mary Megson, MD
Elizabeth Mumper MD
Andrew J. Wakefield, MB, BS, FRCS, FRCPath

And on Monday, the people left from this list were:

	Arthur Krigsman MD.

	Amazing. I can only surm...

I found a link to Wade’s blog that talked basically about how proud he was of the 10 or so people who attended the ‘Simpsonwood remembered’ conference this weekend (big get together for tinfoil hat fanciers).

He linked to a few images of the event and as if I needed it, I was given a pertinent reminder of the average intelligence of the rank and file Mercury Militia members.

Please, feel free to point and laugh.

This, by the way, supplants my other favourite picture, taken at a David Kirby book reading: (Source: Left Brain/Right Brain)
