Improving The Detection And Treatment Of Esophageal Cancer
discuss this article
By looking at proteins expressed in esophageal cancer cells, scientists have determined new ways to detect and follow the progression of this type of cancer. Esophageal cancer is increasing rapidly in Western countries and is currently the seventh leading cause of cancer-related death. But current techniques do not allow doctors to clearly tell patients how their disease will progress and how to best treat it. David M. [click link for full article]

Patients' knowledge and expectations regarding dental implants: assessment by questionnaire.
discuss this article
Today, modern implant dentistry appeals to a wide population, but the decision for and the success of implants depend on the knowledge and expectations of patients. The aim of this study was, with the help of a questionnaire, to evaluate the level of patient knowledge before a professional consultation was performed, and hence to be better prepared in the interests of patient awareness. Fifty-eight percent of 315 patients questioned thought that implants require the same care as natural teeth, 61% expected an additional payment of 2000 Euro or less, 80% held the function of an implant-supported overdenture as very impo...

Complications and relapse in alveolar distraction osteogenesis in partially dentulous patients.
discuss this article
In this study, complications of this procedure and relapse of the transport segment were evaluated in partially dentulous patients. Twenty patients underwent distraction by means of extraosseous distractors. The location of the defects was the anterior mandible (4), posterior mandible (4), anterior maxilla (10) and posterior maxilla (2). Bone height was measured on panoramic radiographs preoperatively, after distraction and after implant placement at the mesial and distal point of the implant(s). Mean alveolar distraction was 6.5mm at the mesial point (P<0.001) and 6.1mm at the distal point (P<0.001). The mean relaps...

Objective assessment of autophony in patients with patulous Eustachian tube
discuss this article
The objective of the present study is to evaluate the Eustachian tube (ET) acoustic patency during phonation. The sound level
in the EAC during phonation of the “A” and ”N” sounds was measured by microphones in the bilateral EACs of nine normal subjects
and 31 patients with patulous ET. The measured sound pressure differences between the right and left ears were correlated
with the differences in severity of autophony between the bilateral ears assessed by a visual analogue scale (VAS). The patulous
condition was often remarkable when the “N” sound was phonated. In some patients with patulous ET, the patulo...

Negative pressure pulmonary edema: report of three cases and review of the literature
discuss this article
Abstract Pulmonary edema following the relief of an upper airway obstruction is an uncommon and unpredictable clinical entity. This
unusual disease is actually attributed to pulmonary and hemodynamic changes engendered by high negative intrathoracic pressures
during the state of obstructed respiration, such as laryngospasm, epiglottitis, laryngotracheal neoplasm, etc. In this article,
we report three cases of negative pressure pulmonary edema (NPPE) developed after the operations of tracheotomy, adenoidectomy,
and microlaryngeal surgery. The etiology, pathophysiology, diagnosis, management, and outcome of NP...

Abstract This prospective study attempts to explore the effect of glycerol on vestibular evoked myogenic potential (VEMP) in patients
with definite unilateral Meniere’s disease (MD) and to investigate whether the test reflects different pathologic states of
endolymphatic hydrops (EH) in the inner ear by comparing the results of glycerol pure tone audiometry (PTA). Twenty-eight
patients with definite unilateral MD were studied. As a control group, 11 patients with vestibular neuritis were selected.
Sixteen ears of normal volunteers were enrolled to acquire normal range of the test. Each MD patient underwent...

Vertigo caused by a nasopharyngeal carcinoma
discuss this article
Abstract A case of a 63 year-old woman with acute vertigo, hearing loss and tinnitus caused by a nasopharyngeal carcinoma is reported.
Despite a long-standing unilateral Eustachian tube dysfunction, only the occurrence of vertigo attacks lead to the diagnosis
in this patient. Inner ear-related symptoms are rare in nasopharyngeal carcinoma and the disease is uncommon in Europe. Skull
base tumors are an important differential diagnosis of labyrinth dysfunction that can be detected by MRI. A complete diagnostic
work-up is necessary in patients with unilateral tube dysfunction, to allow early detection of t...

Unusual presentation of oropharyngeal Kaposi's sarcoma
discuss this article
The aim of this article is to focus ear, nose, and throat (ENT) practitioners on a pathology rarely seen within the head and neck mucosa.A 62-year-old black African man was referred to our ENT department for dysphagia and hoarseness. Physical ENT examination revealed a smooth normal mucosal mass on the left lateral pharyngeal wall, which masked the inlet of the larynx, and bilateral cervical lymph nodes but no other mucosal lesions. Anti-human immunodeficiency virus antibodies were found to be negative. Hemoglobin, hematocrit, white blood cell count, and platelet count were normal. Serum protein electrophoresis was also no...

Spectral-Ripple Resolution Correlates with Speech Reception in Noise in Cochlear Implant Users
discuss this article
This study examined performance on a spectral discrimination task and compared it to speech
perception in noise. An adaptive procedure was used to determine the spectral-ripple density that subjects could discriminate.
A closed-set, forced-choice adaptive procedure was used to determine speech reception thresholds for words in two-talker babble
and in speech-shaped, steady-state noise. Spectral-ripple thresholds (ripples/octave) were significantly correlated with speech
reception thresholds (dB SNR) in noise for 29 cochlear implant users (r = −0.55, p = 0.002 in two-talker babble; r = −0.62, p = 0.0...

A combined anterolateral thigh flap and vascularized iliac crest flap in the reconstruction of extended composite defects of the anterior mandible.
discuss this article
The technique of lower anterior facial reconstruction using a combination of an anterolateral thigh flap (ALTF) and a microvascular iliac crest flap is described and the results of 18 cases are reported. Eleven patients suffered recurrence after surgery and radiotherapy of oral squamous cell carcinoma of the lower face. These patients underwent a second surgery and reconstruction. In seven patients reconstruction was performed during primary surgical therapy, followed by postoperative radiotherapy. The pedicle of the ALTF was used for elongation of the iliac crest pedicle. There were no problems with the pedicle length...

Sleep nasendoscopy: a 10-year retrospective audit study
discuss this article
Abstract Sleep nasendoscopy was conceived at the Royal National Throat, Nose and Ear Hospital, UK in 1991, and has remained fully implemented
in patient selection for targeted treatment of the spectrum of sleep-disordered breathing. The senior authors (B.T.K. and
P.B.) have been performing sleep nasendoscopy together for over 10 years, and we look back at their decade’s experience. A
retrospective audit study based on case notes was performed over a 10-year period (1995–2005) in a tertiary-referral practice
setting. Case notes were retrieved on all patients who had undergone sleep nasendoscopy durin...

Metastasis is the most important predictor of survival in patients with oral squamous cell carcinoma (OSCC). We tested the hypothesis that there is a genetic expression profile associated with OSCC metastasis. We obtained samples from 6 OSCC node-positive primary tumors and their matched metastatic lymph nodes, and 5 OSCC node-negative primary tumors. Using laser capture microdissection, we isolated OSCC cells from metastatic lymph nodes and compared them with those from matched primary tumors and unmatched node-negative primary tumors using Affymetrix Human Genome Focus arrays.Comparison of tumor cells from the lymph nodes...

Advantages of not using the intensive care unit after operations for oropharyngeal cancer: An audit at Worcester Royal Hospital.
discuss this article
We reviewed 68 cases of oral and oropharyngeal cancer that were managed without the routine use of intensive care units (ICU), to establish success rates for flaps, complications including nosocomial infections, cancellations, and length of stay. More than 98% of flaps survived and over half the patients had no complications. Low rates of perioperative infection were recorded with a median length of stay of 12 days (range 2-63), and there were no cancellations. We conclude that the routine use of a specialist head and neck ward is more appropriate than ICU for selected cases; it fulfils current guidelines for cancer se...

ORIGINAL ARTICLE: Life After Total Laryngectomy: A Measure of Long-term Survival, Function, and Quality of Life
discuss this article
Conclusions Pretreatment clinical variables (including primary tumor site, tumor stage, regional metastases, and number and type of comorbidities) have an effect on long-term survival after TL. Despite common belief, many patients who have undergone TL maintain a good QOL overall. This study sheds light on which patient-related factors may affect health-related QOL outcomes after TL. These findings may be used to select patients who are good candidates for TL based on anticipated functional and QOL outcomes.

Arthrocentesis for the treatment of temporomandibular joint closed lock: a review article.
discuss this article
Although arthrocentesis is not a panacea, it has been widely used for a variety of temporomandibular joint (TMJ) disorders. Uncritical acceptance of the procedure has been cautioned against, as further analysis and clarification of published studies was required. The aim of this article was to review the efficacy of arthrocentesis in the treatment of patients with TMJ closed lock. The rationale behind this approach was based on the notion that TMJ surgical results should be reported by the specific category of TMJ disorder. After a Medline search of the international literature, 14 articles were found fulfilling the cr...

Sulindac exerts its antitumourigenic effects in oral squamous cell carcinoma (SCC) cells by modulating survivin in a Stat3-dependent manner. Immunohistochemistry was used to detect the protein levels of phosphorylated-tyrosine Stat3 (p-tyr Stat3) and survivin in SCC tissues. Western blot, reverse transcriptase polymerase chain reaction, Annexin-V and cell proliferation assays were used to determine p-tyr Stat3 and survivin protein and mRNA expression, and cell viability following treatment with cyclooxygenase (COX) inhibitors, Stat3 siRNA, or the forced expression of Stat3 or survivin. Immunohistochemical analysis reve...

Benign fibrous histiocytoma in the condylar process of the mandible: Case report.
discuss this article
A 48-year-old man was referred for investigation of an asymptomatic radiolucent lesion in the mandible. The margin was partly irregular, and there was no peripheral sclerosis. The tumour was composed of histiocytic cells, spindle cells, and fibrous tissue. Immunohistochemical analysis showed that the tumour cells stained for CD68 and vimentin, and not for cytokeratin, smooth muscle actin, S-100 protein, or CD34. The tumour was therefore diagnosed as a benign fibrous histiocytoma.

Endoscopically-guided core biopsy of the condylar head.
discuss this article
The effective management of mandibular cysts and tumours relies on obtaining an accurate tissue diagnosis with the acquisition of specimens representative of the lining and/or contents of the lesion. Access to such abnormalities is not commonly an issue since bony windows can be made in readily accessible areas. However, less accessible regions such as the ascending ramus, coronoid process and condyle can be surgically challenging without an extensive lateral approach. This paper reports a minimally-invasive technique in which an endoscope was used to facilitate the core biopsy of a condylar lesion.

Quantitative methylation analysis of resection margins and lymph nodes in oral squamous cell carcinoma.
discuss this article
Resection is the preferred treatment for oral squamous cell carcinoma, and pathological staging of the resected specimen is crucial. The role of molecular biology in the diagnosis of minimal residual disease has not been fully investigated and may improve staging. Multiple adjacent specimens were taken from the tumour, the invasive front, the surgical margin, and the lymph nodes of 20 specimens from patients with oral cancer. Bisulphite-treated DNA from these specimens was assayed quantitatively with pyrosequencing methylation assays (PMA) of CpG islands within the gene promoters of the p16 and CYGB genes. Results were...

Free flap reconstruction of head and neck cancer defects is complex with many factors that influence perioperative complications. The aim was to determine if there was an association between perioperative variables and postoperative outcome. We evaluated 185 patients undergoing free flap reconstruction following ablation of head and neck cancer between 1999 and 2001. Demographic, laboratory, surgical and anesthetic variables were analyzed using univariate and multivariable techniques.Ninety-eight patients (53%) developed complications, of which 74 were considered major, giving a major morbidity rate of 40%. Screening for glandular fever in patients with Quinsy: is it necessary?
discuss this article
Abstract Quinsy (peritonsillar abscess) is a common emergency seen in otolaryngology practice. These patients are often screened for
glandular fever in addition to routine haematological tests. In our unit, we have screened 66 patients with quinsy for glandular
fever over a period of 12 months. All these patients were screened for glandular fever by rapid immunoassay. Only one out
of 66 patients was tested positive for glandular fever. Due to the extremely low incidence of glandular fever in quinsy patients,
we do not see any relevance in screening for glandular fever in quinsy patients. Hence we recomm...

Abstract We undertook a systematic evaluation of spike rates and spike amplitudes of auditory nerve fiber (ANF) responses to trains
of electric current pulses. Measures were obtained from acutely deafened cats to examine time-related changes free from the
effects of hair-cell and synaptic adaptation. Such data relate to adaptation that likely occurs in ANFs of cochlear-implant
users. A major goal was to determine and compare rate adaptation observed at different pulse rates (primarily 250, 1000, and
5000 pulse/s) and describe them using decaying exponential models similar to those used in acoustic studies. R...

Two of these patients still complained because they were perceiving too low pitch sounds, and because of the presence of echoes
and poor discrimination after 3 years of implant use and many tuning sessions. We hypothesized that the electrode array was
too deeply inserted and could be stimulating overlapping populations of neurons in the low fre...

Voice improvement in unilateral laryngeal paralysis during loud voicing: theoretical impact
discuss this article
Abstract Voice of patient with unilateral laryngeal paralysis (ULP) shows a nonlinear behaviour with sudden octave jumps, bifurcations
and chaos. Such a behaviour may be due to an increased number of freedom degrees in the glottal system. We hypothesized that
voice intensity (with increasing sub glottal pressure) could improve vocal signal stability with less freedom degrees in vibrating
system, and then a decrease of nonlinearities. A prospective study of 32 consecutive voices of patients with ULP and severe
dysphonia was conducted. Jitter and Lyapunov exponent from vocal signals were compared at comfortabl...

Mutations in SH3BP2, the cherubism gene, were not detected in central or peripheral giant cell tumours of the jaw.
discuss this article
Giant cell granulomas of the jaw (GCGJ) are non-familial, generally unilateral osteoclast-rich lesions that are histopathologically indistinguishable from cherubism. Cherubism is an autosomal dominant disease that is characterised by bilateral radiolucencies of the jaw, and caused by mutations that occur in SH3BP2 exon 10. The aim of the study was to screen lesional GCGJ tissue for SH3BP2 mutations. Lesional mononuclear stromal or spindle cells were microdissected from paraffin-embedded tissue from GCGJ, and DNA was then extracted and sequenced for SH3BP2 mutations associated with cherubism. No mutations were detected ...

Ototoxicity of ototopical drops-an update.
discuss this article
Various topical preparations are widely used for treating external otitis, acute otitis media with perforation, post-tympanostomy tube otorrhea, and chronic suppurative otitis media. Solutions and suspensions that enter the middle ear through a perforated tympanic membrane can reach the inner ear by crossing through the round window membrane. The safety of various components of otic solutions used in this way, particularly aminoglycosides, corticosteroids, solvents, and preservatives, has been questioned in light of their ototoxic potential. The authors' objective is to review the literature discussing the safety of co...

Management of complications in neurotology.
discuss this article
Neurotologic and skull base surgery involves working around important neurovascular and neurotologic structures and can incur unwarranted complications. Knowledge of surgical anatomy, good preoperative planning, intraoperative monitoring, and excellent microsurgical technique contribute to minimizing and avoiding complications. In the event of a complication, however, the neurotologic surgeon should be prepared to manage it. In this article, the authors focus on the management of complications encountered in neurotologic skull base surgery, including hemorrhage, stroke, cerebrospinal fluid leak, extraocular motility de...

Image-guided technique in neurotology.
discuss this article
To date, clinical application of image-guided surgery (IGS) to otology/neurotology has been limited, but a large potential market and numerous applications support use. Such applications include control of surgical instruments (eg, turning off a drill when close to an anatomic boundary), robotic surgery (eg, robotic mastoidectomy), and minimally invasive surgery (eg, percutaneous cochlear implantation).

Surgical approaches and complications in the removal of vestibular schwannomas.
discuss this article
Vestibular schwannomas are benign tumors that usually originate from the balance portion of cranial nerve VIII. The treatment options currently available for vestibular schwannomas include observation with serial imaging, stereotactic radiation, and microsurgical removal. Although the ultimate goal in treatment of vestibular schwannomas is preservation of life, the best option for each patient depends on symptoms, tumor size, tumor location, and the patient's general health and age. Surgical exposure of the cerebellopontine angle for removal of vestibular schwannomas can be performed safely via a translabyrinthine, ret...

Stereotactic radiation techniques in the treatment of acoustic schwannomas.
discuss this article
Medical decision-making is based on benefit-to-cost analysis. Optimally, treatment obtains a high degree of benefit while minimizing the physical, social, and financial costs. The goals of the treatment of acoustic schwannomas are prohibiting tumor growth and alleviation of symptoms caused by damage to local structures. These symptoms-tinnitus, ataxia, and hearing loss-secondary to eighth nerve dysfunction, as well as symptoms arising from damage to adjacent structures such as the facial nerve, trigeminal nerve, or pons, can be caused by tumor growth or treatment. Determination of optimal therapy must also take into ac...

Guiding patients through the choices for treating vestibular schwannomas: balancing options and ensuring informed consent.
discuss this article
Counseling patients who are diagnosed with vestibular schwannomas, formerly known as acoustic neuromas, can be challenging. The health care provider has the responsibility to explain, in understandable language, to the patient or legal representative the proposed treatment options, risks and complications associated with each form of treatment, and alternatives to treatment, including no therapy. Patients should be encouraged to gather information before making a treatment decision. For the physicians managing these patients, information should be delivered in a balanced way to ensure patient understanding of their opt...

Lesions of the petrous apex: diagnosis and management.
discuss this article
Management of petrous apex pathology poses a unique challenge even to the most seasoned skull base surgeons. The central location in the skull base with adjacent critical neurovascular structures makes access to this region more than a trivial matter. Significant advances in diagnostic imaging have greatly facilitated the diagnosis of petrous apex lesions. The introduction of modern skull base surgery techniques also has provided skull base surgeons with numerous avenues to the petrous apex while significantly decreasing morbidity. The latest diagnostic and management strategies are discussed and an update of some of t...

Advances in skull base imaging.
discuss this article
Despite being introduced in the 1970s and 1980s, CT and MRI continue to be the primary imaging modalities for the temporal bone and skull base. Although the general concepts and physics remain the same, the images obtained currently are far superior. Augmenting these traditional modalities are nuclear medicine imaging techniques, functional imaging, and the fusion of different techniques. Advances in these areas are expanding our options and illuminating pathology in unique ways. As basic physiologic processes are better understood, new opportunities for novel imaging techniques should arise.

Anesthesia, antisepsis, microscope: the confluence of neurotology.
discuss this article
Neurotology and skull base surgery slowly emerged out of a confluence of interrelated disciplines and technologies over the past century to become the field that we know today. Its formation required the marriage of neurosurgery and otology, the introduction of the operating microscope, and advances in surgical technique, anesthesia, and radiology. Along the way, the field also began involving specialists within ophthalmology and craniofacial and plastic and reconstructive surgery. Lastly, and perhaps most importantly, the formation of neurotology and skull base surgery required pioneering surgeons who were willing to ...
