Lipid Nanodiscs Unlock the Potential of Cytokine Treatment
Researchers at the University of Texas at Austin created a nanocarrier for stem cell factor, a regenerative cytokine. The nanotechnological approach renders the treatment much safer, as previous attempts to use stem cell factor as a pro-angiogenic tr...

These wounds will never heal
May is National Stroke Awareness Month, and I want to share my story of my dad ’s journey after his strokes. Doors open and shut during the early morning hours. I hear the drip dispensing the next dose and some muffled chatter outside the room. Several phones are ringing in all directions, and I listen toRead more …These wounds will never heal originally appeared inKevinMD.com.

A man in his 40s with RUQ abdominal pain
Conclusion:In hemodynamically stable patients with chest pain, sinus tachycardia aids in the identification of patients unlikely to have type I MI, especially in those with HR greater than 120 bpm.===================================MY Comment by KEN GRAUER, MD (4/28/2020):===================================Insightful case presented by Drs. Goss and Meyers highlighting the importance of recognizing the ECG signs of acute PE. I focus my comments on further dissecting some of these " tell-tale " ECG features. The " theme " of today ' s case was "pattern recognition". Like the ECG diagnos...

March 2022: Is It Really a Seizure?
The EMS notification was that a 71-year-old man was going in and out of seizures. His vital signs were stable.I followed the stretcher into the room when they arrived five minutes later. The EMS crew reported that he had previously had a stroke, but he wasn't on any medications for seizure. The family called 9-1-1 because he had been going in and out of seizures for 20 minutes. He suddenly became unresponsive during these episodes, but came back to himself immediately. The patient had another episode while being placed on the monitor. He stopped talking and his limbs shook, and it seemed that his eyes deviated to the left. ...

What Has A.I. In Medicine Ever Done For Us? At Least 50 Things!
Remember Monty Python’s brilliant Life of Brian movie scene where the Judean Jewish insurgent commando, planning the abduction of Pilate’s wife in return for all the horrors they had to endure from the Roman Empire, asks the rhetorical question: what have the Romans ever done for us? With the hype and overmarketing, not to speak about the fears around A.I, we asked the same question. What has A.I. in medicine ever done for us? Well, we found at least 50 things. I have 50 responses to the pressing question on everyone’s mind who is interested in healthcare but tired of the hype or the doomsday scenarios around A.I....

Curve of wisdom in ACS : Open Cath lab doors may mean nothing , if the windows are closed !
“We have a 24/7 cath lab with an open door policy. Our cardiologist arrives at 15 minutes’ notice. Door to balloon time is less than 60-90 minutes”,  “Great, so, you can always offer a successful treatment for STEMI” “No, that we can never guarantee.”   “Oh, It Is not the answer, I  expected” “I agree, it sounds disappointing, but. truths are less pleasing. What I am trying to say is, there are a number of factors other than the availability of a grand cath lab and agile and effortless hands, that try to reperfuse the myocardium in distress.  I ...

Wide Complex Tachycardia with Huge ST Elevation. What is going on?
This 70-something woman with no significant past history (no previous ECGs or cardiac history) presented by EMS with fairly acute chest pressure and shortness of breath, with nausea and diaphoresis.  " Like an elephant sitting on my chest. "  She had no history of atrial fibrillation and was not on any anticoagulants. She stated that she had had a similar episode a couple weeks earlier, lasting 24 hours, with rapid heart beat but without chest pain, that spontaneously resolved.  She thought she was having a panic attack.  Since then she has had " little spurts " of the same thing lasting 1-2 hours.E...

A presentation on atrial fibrillation : Old wine in Old bottle
Caution: 2008 Presentation    This one is from the archive, a presentation on Atrial fibrillation done 14 years ago at KG.hospital, Coimbatore.   Link to PDF download  Good news: Nothing much has changed since 2008 Recognizing the clinical importance of AF and the need to rule out a systemic cause is the key, Further, a genuine bedside debate about the pros and cons of simple vs aggressive treatment discussion is welcome. The nomenclature issue of valvular vs non-valvular has finally seemed to have settled. The latter is banished for good reason. (Funny to note Aortic valve  was conside...

A man in his 50s with acute chest pain, now resolved, has 2 undetectable troponins. CT Coronary Angiogram?
Written by Pendell MeyersA man in his 50s presented to the ED with chest pain described as pressure, without radiation, acute onset about three hours prior to arrival. He had had stuttering less severe versions of this pain all week that usually went away after a few minutes. He also had diaphoresis and dyspnea. He had extensive family history of CAD with CABG ' s in the mid 50s for multiple relatives, but he had no personal known history of CAD. It is unclear whether he had pain at the time of triage, but notes describe that his pain had subsided by the time of EM physician evaluation:Triage at approximately 21...

Reflections on HLTH2021: The Lens of the Patient and Carepartner
By GRACE CORDOVANO Attending HLTH 2021 in-person in Boston solidified that there is no comparison between attending live vs. virtual conferences.  While content and presentations can be solid both virtually or in-person, it is the energy of the connections that are made between scheduled presentations and the conversations that are shared throughout that move the needle. Kudos to the organizers of HLTH 2021 for prioritizing the safety of all in-person attendees with COVID-19 vaccination requirements, proof of negative PCR testing within 3 days of arrival, and mask requirements on-site. After reflecting on all th...

Extremely high HDL – beneficial or harmful?
This study showed extremely high HDL was associated with lower risks for all-cause mortality and cardiovascular events compared to low HDL. But they had higher risks compared to very high HDL. Paradoxically high all-cause mortality in those with extremely high HDL has also been reported in another study of 52 268 men and 64 240 women [5]. Though most of these studies showed a negative role for extremely high HDL, there is recent study from Korea which showed that extremely high HDL was not associated with increased risk of mortality, myocardial infarction and stroke [6]. That study with 343,687 subjects also defined extrem...

Paradoxical Low flow-Low gradient- Aortic stenosis: What is the paradox & why does it happen?
Aortic stenosis evaluation was so simple in our early days. Gradients across the valve were the key. Now, we have more parameters to bother about. Dynamic AVOs, flow state, resting LV function, contractile reserves, GLS, dobutamine response, etc. MRI assessment will soon overtake echocardiography.  Hemodynamics of flow across LVOT. MRI 4D volumetric model of normal Aortic stenotic flow in the bicuspid valve (On the left). The more we know, the more we tend to miss! Image courtesy: Northwestern Medicine The current AS algorithms, though scientific, I am afraid, appear much complicated with some frightening terminol...

What is the Impact of Atrial fibrillation on blood pressure ? … It is also irregularly irregular !
Atrial fibrillation has a direct effect on systemic blood pressure as stroke volume swings from beat to beat because of changing  RR Interval ( preload ). The variation in systolic pressure actually reflects not only the changing stroke volume but also the enhanced contractility of the ventricle to the preload( Frank-Starling principle ). The net effect is reflected in the pulse as an irregularly irregular pulse (Both rate and volume /Amplitude).* However, In severely dysfunctional ventricles this variation in systolic  BP can be minimal. Also realize, If the preload is changing every beat, there is a considerable dynami...
