Pericardial effusion associated with Infective Endocarditis : Incidence , mechansim and clinical Implication.
Pericardial effusion is often detected in patients with Infective endocarditis. Incidence can be as high as 25% . Most often it is mild, can be moderate in few. Mechanism Sympathetic effusion in response to endocardial infection. It’s never more than minimal. (Evidence ? it’s only an assumption) IE related cardiac failure (Raised systemic venous pressure to which pericardial veins drain) Local sepsis, Abcess formation tracks to pericardial space through transmural lymphatics Fungal , granulomatous , Tuberculous IE (Rare) Here IE and PE  share the same pathology Part of systemic sepsis activated Immune mechan...

Ischemic cardiomyopathy: Why is it, still searching for a definition?
The term Ischemic cardiomyopathy(ICM)  was originally coined by Dr. Burch from Tulane University, New Orleans, USA in 1970. For many decades there was skepticism regarding the existence of such entity. WHO classification over the years never included this term. ESC working group of 2008  (Elliott P,  European Heart 29(2):270–276) decided not to include CAD as a cause for cardiomyopathy. Even the current MOGES system doesn’t invoke CAD as a cause for cardiomyopathy.  But, I am sure, most of practicing cardiologists would agree, there is a need for such an entity. Why there is much reluctance to diagnose Ische...

Was the past perfect ? I don ’ t know, but the future looks tense … welcome on-board, to automated ACS management.
Charles river esplanade ,Boston* : A healthy middle-aged man who was jogging quietly, while his heart was under intense scrutiny by the bionic eyes of Apple i-watch’s smart patch electrode. Suddenly, it detected some bizarre ST segment fragmentation (Seems it can predict in advance , Ischemic signals 10 minutes prior to onset of ACS ) The built-in cosmos direct GPS instantly alerted & summoned a titanium powered Space X drone that pulled the patient from the riverside to the nearest human wellness port . It dropped him through a remotely accessed split glass roof right inside the hybrid heart lab, to find , men ...

Coronary Stent Price Control in India: Two Years and Counting
By SOMALARAM VENKATESH MD With a stated intent of bringing social justice and financial relief to hundreds of thousands of patients undergoing coronary angioplasty in the country every year, the Government of India capped the sale price of coronary stents in Feb 2017. Stent prices fell by as much as 80% with this populist move, seen as anti-trade within the industry circles. It is tempting for a practising interventional cardiologist to look at two years of this government control on medical device prices in a market economy. Before price-capping, angioplasty patients were indeed getting a raw deal. There was no uniformity...

What we can learn from the tragic deaths of CEOs
An excerpt from Dead Execs Don’t Get Bonuse$: The Ultimate Guide to Surviving Your Career With a Healthy Heart.  The details of the careers and tragic deaths of these CEOs are a matter of public record, and the disruption that followed their deaths was enormous. Take Ranjan Das, for example. At age forty-two, he was the regional CEO of SAP for the Indian subcontinent and the youngest CEO of a multinational corporation in India. He was returning home after using the corporate gym when he suffered a fatal cardiac arrest. Das was known for his healthy diet and exercise habits. He had finished a marathon just months be...

Left main coronary artery disease on ECG
Left main coronary artery disease on ECG ST segment elevation in aVR and diffuse ST segment depression in inferior and anterolateral leads is considered a feature of significant left main coronary artery disease. The ECG shown above, there is ST segment elevation in aVR and V1 along with ST depression in inferior and lateral leads. ST elevation in aVR more than that in V1 is also suggestive of left main disease. In a study comparing acute obstruction of left main, left anterior descending (LAD) and right coronary (RCA) occlusions, aVR ST elevation of more than 0.05 mV was noted in 88% of left main (LMCA) obstruction, 43% ...

Idiopathic Ventricular Tachycardias for the EM Physician
Written by Pendell Meyers, reviewed by Steve Smith and Scott Weingart“Idiopathic ventricular tachycardias” refer to a group of tachydysrhythmias originating below the AV node and bundle of His but differing in etiology, prognosis, and treatment compared to classic ventricular tachycardia (VT). The name “idiopathic” is becoming more and more inappropriate, as various specific subgroups and specific etiologies are being discovered, including right ventricular outflow tract VT, fascicular VT, and bundle branch reentrant VT. To understand these entities one must first understand the differences between classic VT and i...
