Ventricular tachycardia – management
Ventricular tachycardia is a potentially life threatening cardiac arrhythmia. If the rate is very fast, hemodynamic deterioration can occur rapidly. On the electrocardiogram, ventricular tachycardia can be defined as three or more ventricular ectopic beats occurring in a sequence at a rate more than 100 per minute. Ventricular tachycardia which gets spontaneously terminated within 30 seconds is called non sustained ventricular tachycardia (NSVT). Prolonged (63 minutes) Ventricular Fibrillation, Followed by Unusual Cardiogenic Shock
In this study, 5% of VF arrest was due to PE: V fib is initial rhythm in PE in 3 of 60 cases.  On the other hand, if the presenting rhythm is PEA, then pulmonary embolism is likely.  When there is VF in PE, it is not the initial rhythm, but occurs after prolonged PEA renders the myocardium ischemic.--Another study by Courtney and Kline found that, of cases of arrest that had autopsy and found that a presenting rhythm of VF/VT had an odds ratio of 0.02 for massive pulmonary embolism as the etiology, vs 41.9 for PEA.

The tracing above shows a fast wide QRS tachycardia (in the initial part of the strip), which is terminated by a direct current (DC) shock, leading to restoration of sinus rhythm in the latter part of the rhythm strip. Paroxysmal SVT (PSVT) that repeatedly recurs in spite of successful conversion with adenosine
An elderly male complained of dyspnea.  His pulse was regular at just under 150.  BP was 110/70.  There was no evidence of shock or pulmonary edema.  Here is his ED ECG:There is a very rapid, wide complex tachycardia (QRS = 150 ms).  There are no P-waves before the QRS's.  There is a definite Right Bundle Branch Block and Left Anterior Fascicular Block pattern, so this is not VT.  The bifascicular block was new.  It could be a new block, or a rate-related BBB  If you look closely, you can see the inverted (retrograde) P-waves. Are doctors being duped through medical education? Could social media help?
I made a discovery this week about the novel anticoagulant medications, dabigatran (Pradaxa), rivaroxaban (Xarelto), apixaban (Eliquis) and edoxaban (Lixiana). I was looking into the often-asked question of how these new drugs compare to the old standard, warfarin. The discovery felt like a Eureka moment. I ran it by my stats guy–my son–and a couple of colleagues, and they confirmed, that my discovery was truth.
