Cuffless Blood Pressure Monitoring – Not for the Faint-of-Heart (Pun Intended)
This article will summarize these challenges and also provide optimism for the refreshingly near future. Namely, while the progress towards FDA-cleared cuffless BP monitoring may feel protracted, significant technological advances, regulatory modernization, and clinical use case validation are rapidly advancing and aligning.  Key Challenges Confronting OTC Cuffless Blood Pressure Monitoring Scientific Risks At the most fundamental level, the jury’s still out on whether sufficiently accurate noninvasive, cuffless, calibration-free blood pressure monitoring is physically realizable at all. Namely, “Do the laws of nature...

A 50-something with chest pain. what to do? And get the PM Cardio app for your own use here!
This was sent to me by a friend. It is from a 50-something with chest pain:What do you think? This was marked as " Not a STEMI " by the physicians.  It is not a STEMI, but it isdiagnostic of an LAD OMI (Occlusion MI).  There is subtle ST Elevation in V1-V4 and hyperacute T-waves in V2-V6.There is also subtle but diagnostic INFERIOR ST Elevation, with reciprocal ST Depression in aVL.I uploaded this to our new PM Cardio AI Bot app.  We call the app the " Queen of Hearts "First, it transforms it to a digital file and standardizes the image.  Here is the result:Next, it interprets the digital data:A...

ERS pattern in ECG : New-onset “ Iatrogenic panic ” is unwarranted !
This is an ECG of a 25-year-old, recorded in master health check-up.  It was reported as ERS pattern of concern. When he went for an expert opinion, he was suggested to understand, there is a small risk of SCD. That’s it, the panic has set in already, which got amplified in the following conversation.. How small is the risk,?  he wanted to know. Yours’ is possibly an intermediate risk. Intermediate means what?  Do I have any genetic or EP studies to predict the risk accurately? Sorry, we have some gene mapping but all primitive. We aren’t sure. But keep calm, nothing will happen. Do I nee...

ERS pattern in ECG : “ Iatrogenic panic ” is unwarranted !
This is an ECG of a 25-year-old, recorded in master health check-up.   It would be mind-boggling to know the prevalence of such ERS patterns in the general population. One estimate suggests it could be anywhere between 3 to 13  % depending upon the criteria used. Let us assume the mean as 5 %. Then, it would be 30 crores of human beings in our habitat show this ECG pattern. If applied, in my city Chennai alone 5 lakh people could carry this tag. While it is true, some forms of ERS and J wave syndrome can be markers of serious ventricular arrhythmias, either spontaneous or at times of Ischemia. Curr...

A big data COVID train wreck
BY ANISH KOKA If there was any doubt the academic research enterprise is completely broken, we have an absolute train wreck of a study in one of the many specialty journals of the Journal of the American Medical Association — JAMA Health. I had no idea the journal even existed until today, but I now know to approach the words printed in this journal to the words printed in supermarket tabloids. You should too! The paper that was brought to my attention is one that purports to examine the deleterious health effects of Long COVID. A sizable group of intellectuals who are still socially distancing and wearing n95s ...

Myocarditis update from Sweden
BY ANISH KOKA The COVID19/vaccine myocarditis debate continues in large part because our public health institutions are grossly mischaracterizing the risks and benefits of vaccines to young people. A snapshot of what the establishment says as it relates to the particular area of concern: college vaccine mandates: Dr. Arthur Reingold, an epidemiology professor at UC-Berkeley, notes that UC also requires immunizations for measles and chickenpox, and people still are dying from COVID at rates that exceed those for influenza. As of Feb. 1, there were more than 400 COVID deaths a day across the U.S. “The arg...

COVID-19 myocarditis illusions: A new cardiac MRI study raises questions about the diagnosis
BY ANISH KOKA One of the hallmarks of the last two years has been the distance that frequently exists between published research and reality. I’m a cardiologist, and the first disconnect that became glaringly obvious very quickly was the impact COVID was having on the heart. As I walked through COVID rooms in the Spring of 2020 trying to hold my breath, I waited for a COVID cardiac tsunami. After all social media had been full of videos from Wuhan and Iran of people suddenly dropping in the streets. My hyperventilating colleagues made me hyperventilate. Could it be that Sars-COV2 had some predilection for heart...

Primary PCI and its silent encounter with “ myocardial neutrophilia ”
Myocardial infarction,  a gross pathological entity renamed now as STEMI for clinical purposes, is the most famous medical emergency that triggers a series of calls. Right from 911/First medical contact, the ER, that ends up in CCU or a 24/7 cath lab. The heart, can’t wait for all these external responses when it is challenged with a vascular accident. The moment ATO occurs, two things happen. The endogenous fibrinolytic led by native tissue PA (Tpa) tries to get rid of the thrombotic plug by all its means. It succeeds in 15%. We call it spontaneous lysis or aborted MI. Many lives are lost in the remaining before th...

The “open data” movement runs aground on FOURIER
BY ANISH KOKA Reanalysis of a trial used to approve a commonly used injectable cholesterol-lowering drug confirms the original analysis by accident. The open-data movement seeks to liberate the massive amount of data generated in running clinical trials from the grasp of the academic medical-pharmaceutical industrial complex that mostly runs the most important trials responsible for bringing novel therapeutics to market. There are only a few elite academic trialist groups capable of running large trials and there’s ample reason to be suspicious about the nexus that has developed between academia and the pharmace...

A woman in her 50s with acute chest pain
Submitted and written by Anonymous, edits by Meyers and SmithA 50s-year-old patient with no known cardiac history presented at 0045 with three hours of unrelenting central chest pain. The pain was heavy, radiated to her jaw with an associated headache.Triage VS: 135/65 mmHg, 95 bpm, 94% on room air, 16/min, 98.6 FTriage ECG:ECG Interpretation:Sinus rhythm with normal QRS. There is slight STE in V1, V2, and aVR, with STD in V3-V6, I, aVL, and II. There are T waves in lead III which are suspicious for hyperacute T waves, with reciprocal negative large T wave inversions in aVL. I do not think this ECG is by itself diagnostico...

Missing links in IHD : What is the relationship between Ischemia & cardiac arrhythmias ?
The term Ischemic heart disease (IHD) was once very popular, but many abandoned it as it became an academic cliche.  CAD & CAHD are the other terms that are equally popular and prevalent. Stable IHD was in vogue till recently, which was again replaced by “chronic coronary syndrome’ now. Honestly, I feel the original term IHD to be restored however outdated it may look. it encompasses the entire spectrum of clinical cardiac disorders. Manifestation of Ischemia heart disease  Angina Infarction Cardiac failure Arrhythmias  Silent ischemia Sudden cardiac death  The ...

Who can answer this unusual query in mitral stenosis?
I will go with the last response. As for as I understand, we have never quantified Atrial muscle mass properly even in normality. One may be tempted to think there is no purpose to measure it, other than academic reasons. The fact that the incidence of atrial fibrillation in mitral stenosis is not linearly correlating with LA size makes us think, LA mass (Virtual LAH) may have a say in triggering AF. This post is meant for cardiology fellows. Maybe someone can do a study on this by measuring LA mass pre and post-PTMC, we might get an idea about regression as well. Meanwhile, we are well versed with infiltrative disease...

An elderly woman with acute vomiting, presyncope, and hypotension, and a wide QRS complex
Written by Pendell MeyersFamily of an elderly woman with many comorbidities called EMS when she suddenly experienced shortness of breath, nausea, vomiting, and near syncope. She was alert and oriented and hypotensive with initial BP 70/50. A 12 lead ECG was obtained by EMS and is shown below:What do you think? There is sinus tachycardia (do not be fooled into thinking this is VT or another wide complex tachycardia!) The ECG is diagnostic of LAD occlusion (or even left main occlusion possibly), with the classic pattern of RBBB and LAFB with huge concordant STE in V1-V2, I, and aVL, with reciprocal depression in mo...
