April fooled with PTMC balloon !
PTMC is a revolutionary    interventional cardiology procedure .A fibrosed obstructed  mitral valve is  opened up just like that ,  with few wires and a balloon  .The procedure is performed within 30 minutes .No  anaesthesia . No surgery .No scar. Thousands of procedures are done world-wide (Rich countries excluded    they do not have RHD) .We in our institute , have gained considerable experience in PTMC ,  and  have  completed  nearly  200 procedures  in the last  few years . As we gain experience surprises also galore ! .Suddenly I realised this  funny (At least for me !) phenomenon  from the unique P...

Changing the culture of American Medicine — Start by removing hubris
This may be the most important post I have ever published. I’m going to tell you about a study that should change the entire way doctors approach patients, and how patients should think of prescribed treatments. These findings should begin a culture change in American medicine. Background: I used to think Medicine would get easier over time. It makes sense, right? You see patterns, you learn how treatments work, and you just get to know stuff. Experience should make it easier to diagnose and treat. That’s not been the case for me. In fact, it’s closer to the opposite. In the exam room, as I look up to the patient fro...

The state of US health: The truth stings
It’s going to be hard to anesthetize this one with compassion. I’m sorry, but the report card on US health is not good. We aren’t heading in the right direction, and everyone involved knows that it’s because of a failure to accept the truth. You have probably already heard the news concerning the state of US health. This week, a mega-group of health researchers (who call themselves the US Burden of Disease Collaborators) published an exhaustive report detailing the health of this nation. (The actual study is available (free) at JAMA. Ron Winslow from the WSJ has this excellent summary.) On the upside, and I...

HHS OIG Reports Small Percentage of Prescribers May be Overutilizing Part D Drugs
This report is a view into things to come under the Sunshine Act, as OIG relied on several of the same factors CMS will rely on including: (1) NPI #; (2) name as reported in the NPPES database; (3) National Drug Code (NDC); and (4) specialty. For example, OIG explained that it identified each prescriber's specialty based on the primary taxonomy code that he or she reported in the NPPES. The taxonomy code indicates a provider's specialty and subspecialty, if any. For example, it may indicate that a prescriber is a family-medicine physician specializing in geriatric medicine. OIG then grouped the taxonomy codes for simila...

Think twice before getting an injection for low back pain or sciatica
It’s been a while since I did a Cycling Wednesday topic. As I was skimming thorough the Journal of the American Medical Association last night, I came across this review article on spinal injection therapy for low back pain. It was a shocker. Two factors brought my attention to the article: First, almost all the cyclists or runners I know have been beset with either back pain or sciatica (referred pain or weakness down the leg). In fact, in some cases, back issues have ended their competitive careers. I too have been flattened with low back pain. It was awful. The other reason to pay attention to the review is the huge n...
