Why don’t we have pre-formed RCA catheter ?
Interventional   cardiology has  grown leaps and bound in the last few decades  .We are able to clip the wings of mitral valve  without surgery when it prolapses We  can deliver a huge aortic valve and fix it with wires . But  . . . we have no proper  preformed   guiding catheter  that can  sit into RCA ostium directly   and snugly  for a long time to enable complex RCA angioplasties ! Why we often follow “A reckless time window” for rescue Angioplasty ?
Myocardial salvage is like  coronary fire fighting.When fire is fought  very early after the accident  , benefits are accrued  more . Text book primary angioplasty is . . .  fire engine arriving at the scene when the house is on fire . Rescue angioplasty is asking for more force ,  when the initial fire fighting  was inefficient to control the fire. ESC Gives A Shot In the Arm To Radial Access For PCI Procedures: The New Default?
Radial access is now the preferred approach for percutaneous coronary interventions, according to a consensus document from the European Society of Cardiology and other European organizations and published online in EuroIntervention. However, at least one prominent US interventional cardiologist thinks the “hard benefits” of radial access “are more controversial,” though he supports increased use of the newer approach. European Society of Cardiology: "Transradial Access Should Be 1st Choice for Angioplasty"
This is very big news. Today the European Association of Percutaneous Cardiovascular Interventions (EAPCI), the Acute Cardiovascular Care Association (ACCA) and the Working Group (WG) on Thrombosis of the European Society of Cardiology (ESC) published their joint consensus document on the radial approach to PCI, online ahead of print in EuroIntervention.

It is still a mystery . . . why we ignored this simple innovation in PTCA
In this complex world , simple innovations fail  . . . just because  they are simple ! Here is a new* PTCA  catheter which has a  two balloons  ,  the distal one dilates the lesion and the proximal one has a stent over it  . The stent is just deployed after the dilatation by the proximal balloon . The proximal balloon  not only help us prepare the lesion before stenting it also helps  in crossing difficult lesions  .
