Impact of sex and age on carotid plaque instability in asymptomatic patients-results from the Munich Vascular Biobank.
CONCLUSIONS: Asymptomatic men had in total more frequently unstable plaques in contrast to women. Thus, male sex seems to be an additional risk factor for ischemic stroke. PMID: 27351411 [PubMed - as supplied by publisher]

EphrinB2 Activation Enhances Vascular Repair Mechanisms and Reduces Brain Swelling After Mild Cerebral Ischemia.
CONCLUSIONS: We show that reducing swelling could result in improved outcome because of reduction in damaged brain tissue. We also identify a novel role for ephrinB2/EphB4 signaling in the maintenance of the neurovascular homeostasis and provide a novel therapeutic approach in reducing brain swelling after stroke. Achieving better detection of vascular risk factors for stroke prevention (P4.062)

The European Society for Vascular Surgery (ESVS) is to be commended for updating1 the “ESVS Guidelines. Invasive Treatment for Carotid Stenosis: Indications, Techniques.”2 Managing patients with cerebral ischaemia is of paramount importance, taking into consideration that despite advances in medical and surgical/interventional therapies, the number of strokes remains high in most parts of the world,3 and the prevalence of vascular risk factors in patients with stroke is increasing.4

SS24. Transcarotid Artery Revascularization versus Transfemoral Carotid Artery Stenting in the Society for Vascular Surgery Vascular Quality Initiative
The Safety and Efficacy Study for Reverse Flow Used during Carotid Artery Stenting Procedure (ROADSTER) multicenter trial reported the lowest stroke rate in high-risk patients compared with any prospective trial of transfemoral carotid artery stenting (TFCAS). However, clinical trials have selection criteria that exclude many patients, and are highly selective of operators performing the procedures, which limit generalizability. The aim of this study was to compare in-hospital outcomes after transcarotid artery revascularization (TCAR) and TFCAS as reported in the Vascular Quality Initiative.

VESS19. Addition of Proximal Intervention to Carotid Endarterectomy Increases Risk of Stroke and Death in the Vascular Quality Initiative
Adding ipsilateral, retrograde endovascular intervention (IPE) to carotid endarterectomy (CEA) for treatment of tandem bifurcation and supra-aortic trunk disease is controversial. Some suggest this combined strategy (CEA + IPE) confers no risk over isolated CEA. Others disagree, reserving CEA + IPE for symptomatic patients. Using the Vascular Quality Initiative (VQI), this study assessed the effect of adding IPE to CEA on stroke and death risk. We further weighed CEA + IPE outcomes in the context of symptomatic status and Society for Vascular Surgery guidelines.

Contemporary trends and outcomes of thrombolytic therapy for acute lower extremity ischemia.
Conclusion Thrombolysis remains an effective treatment for acute limb ischemia with increased utilization over time. There was a significant increase in thrombolysis and endovascular procedure leading to improved outcomes. Thrombolysis alone carried the highest mortality and stroke rate, with T+OPEN associated with the highest amputation and complications. Although thrombolysis is effective, 25% of patients required an open procedure suggesting that patient selection for thrombolysis first instead of open surgery continues to be a clinical challenge. The different scenarios of urgent carotid revascularization for crescendo and single transient ischemic attack.
Conclusions cTIA subjected to CEA have a higher stroke and stroke/death risk compared with patients with sTIA. The urgent setting seems to reduce the stroke/death rate cTIA; for sTIA with a stable neurological condition, the timing of CEA did not influence the outcome. Association between serum uric acid and carotid disease in patients with atherosclerotic acute ischemic stroke.

The endovascular repair of blunt traumatic thoracic aortic injury in Asia: A systematic review and meta-analysis.
CONCLUSION: Short- to mid-term results for thoracic endovascular aortic repair for blunt traumatic thoracic aortic injury in Asia are encouraging. However, there is a distinct disparity in reporting across Asia. We propose a prospective database for outcome reporting post thoracic endovascular aortic repair in this patient population, and ongoing follow up to assess long-term efficacy of this treatment strategy.
