Seropositive Neuromyelitis Optica in a Case of Undiagnosed Ankylosing Spondylitis: A Neuro-Rheumatological Conundrum
In this study, a 35-year-old Indian man with an undiagnosed progressive axial spondyloarthropathy (i.e., AS) is reported presenting with acute-onset longitudinally extensive transverse myelitis, a clinical subset of NMOSD. Teaching NeuroImages: Diffuse cerebral neurosarcoidosis mimicking gliomatosis cerebri
A 30-year-old man with a history of bipolar disease presented with 6 months of cognitive decline, unsteady gait, urinary retention, and bilateral upper extremity tremors. Multiple punctate enhancing lesions were seen on brain MRI (figure). Differential diagnosis included infection (viral, tuberculosis, or cryptococcus), toxic leukoencephalopathy, gliomatosis cerebri, angiocentric lymphoma, and neurosarcoidosis. Neurosarcoidosis Presenting with Prominent Periventricular White-Matter Lesions during Steroid Treatment for Autoimmune Hepatitis
A 63-year-old woman under treatment of autoimmune hepatitis presented with headache, memory loss, and somnolence. Three months before admission, the patient experienced liver inflammation relapse after prednisolone (PSL) cessation. Consequently, PSL was resumed and then tapered. Conclusions:Neurological complications reported with immune checkpoint inhibitors are non-specific and pleomorphic, the most common being meningitis, radiculoneuropathy and myasthenic syndromes. In almost all cases, the outcome was favorable after drug interruption and steroid treatment. As suggested for other irAEs, nAEs may be associated with a clinical benefit and could be indicative for tumor-specific immune activation.

Elevation of the ACTH/cortisol ratio in female opioid dependent patients: A biomarker of aging and correlate of metabolic and immune activation.
CONCLUSIONS: These data establish the ACTH/cortisol ratio as a negative biomarker of aging in females, and show that this decline is more pronounced in ODP an effect which is partly related to HCV seropositivity, immune and metabolic factors. Dementias are one of the most serious health and socioeconomic issues. Targeting cytokines in reduction of depressive symptoms: A comprehensive review
The objectives of the current narrative review are to critically evaluate the literature about the effects of cytokine blockers on clinical outcomes in MDD and in the reduction of depressive symptom severity in individuals using these medications primarily to treat inflammatory conditions.
