Pharmacological management of fibromyalgia: a Bayesian network meta-analysis
Expert Rev Clin Pharmacol. 2022 Feb 23:1-10. doi: 10.1080/17512433.2022.2044792. Online ahead of print.ABSTRACTINTRODUCTION: The identification of the most effective therapy for patients with fibromyalgia (FM) remains controversial. Thus, we conducted a Bayesian network meta-analysis to compare several drugs employed as pharmacological management for FM.AREAS COVERED: The following databases were accessed in October 2021: PubMed, Google Scholar, Embase, and Scopus. All the randomized clinical trials (RCTs) that compare two or more pharmacological management for fibromyalgia were accessed. Only studies involving a minimum o...

P126  The effectiveness of pharmacological interventions and cognitive behavioural therapy in improving sleep in fibromyalgia: a systematic review
Conclusion The optimal management of sleep disorders in FM is complex and needs to be tailored to the individual patient. We have shown that there are varying degrees of efficacy throughout a range of commonly used drugs in FM. Unfortunately, lack of consistency in outcome measures between studies makes it difficult to draw definitive conclusions pertaining to superiority between these agents. Conversely, CBT interventions appear to show consistent sleep improvement in FM. It is not clear whether this translates to online variations of CBT. We believe that a longitudinal appraisal of sleep as a primar y variable, with ro...

Endometriosis and chronic overlapping pain conditions
DISCUSSION: Pain physicians can play a role in the management of patients with endometriosis and COPCs. A multimodal therapy should include physiotherapy and pain-related psychological treatment and possibly centrally acting pain modulation medication.PMID:33576864 | DOI:10.1007/s00482-021-00535-8

Analgesic Effects of Topical Amitriptyline in Patients with Chemotherapy-Induced Peripheral Neuropathy: Mechanistic Insights from Studies in Mice
Antidepressants are an essential component of the therapeutic strategy for treatment of many causes of persistent pain 15, 49. Tricyclic antidepressants (TCAs) are the recommended first-line treatment for many forms of persistent neuropathic pain 11, 15, 22. Amitriptyline hydrochloride (amitriptyline), a TCA approved for the treatment of major depression, has been shown to be effective for the treatment of pain associated with a range of neuropathic pain conditions, including diabetic neuropathy, fibromyalgia and postherpetic neuralgia 19, 25, 27.

Evaluation of the testicular protection conferred by damiana (Turnera diffusa Willd.) against amitriptyline-induced testicular toxicity, DNA damage and apoptosis in rats.
Abstract Psychiatric drugs, such as antidepressants, are used to treat depression based on their ability to modify chemical imbalances of the key neurotransmitters in the brain, including dopamine, serotonin, and norepinephrine. Amitriptyline, a first-reference tricyclic antidepressant derived from dibenzocycloheptadine, is frequently used, especially in neuropsychiatry, to address general depression, major depressive disorders, and fibromyalgia. Therefore, this study attempted to examine the sexual dysfunction attendant on the use of Amitriptyline by investigating the protective role that can be played by damiana...

Patterns of pharmacologic and non-pharmacologic treatment, treatment satisfaction and perceived tolerability in patients with fibromyalgia: a patients' survey.
CONCLUSIONS: Patients with FM in Spain are overtreated with a combination of non-pharmacologic and pharmacologic therapies. Several of these therapies lack adequate support from randomised clinical trials and/or clinical practice guidelines. This overtreatment is not associated with relevant clinical benefits or patient satisfaction and, in the case of pharmacologic treatments, poses tolerability and safety issues. Vitamin D deficiency and treatment in Iraqi patients with primary fibromyalgia syndrome
ConclusionVitamin D deficiency is common in FMS patients and it is associated with worsening of symptoms. Vitamin D supplementation in deficient FMS patients is associated with significant improvement. Screening of FMS patients for hypovitaminosis D is recommended.

Combination pharmacotherapy for the treatment of fibromyalgia in adults.
CONCLUSIONS: There are few, large, high-quality trials comparing combination pharmacotherapy with monotherapy for fibromyalgia, consequently limiting evidence to support or refute the use of combination pharmacotherapy for fibromyalgia. PMID: 29457627 [PubMed - as supplied by publisher]

Antiepileptic drugs for chronic non-cancer pain in children and adolescents.
CONCLUSIONS: This review identified only two small studies, with insufficient data for analysis. As we could undertake no meta-analysis, we were unable to comment about efficacy or harm from the use of antiepileptic drugs to treat chronic non-cancer pain in children and adolescents. Similarly, we could not comment on our remaining secondary outcomes: Carer Global Impression of Change; requirement for rescue analgesia; sleep duration and quality; acceptability of treatment; physical functioning; and quality of life.We know from adult randomised controlled trials that some antiepileptics, such as gabapentin and pregabalin, ca...
