Comparative efficacy of amitriptyline, duloxetine and pregabalin for treating fibromyalgia in adults: an overview with network meta-analysis
This study included systematic reviews (SR) of randomized clinical trials (RCTs) targeting adult patients over 18  years of age diagnosed with fibromyalgia according to the criteria of scientific societies, which include the basic clinical diagnosis characterized by the presence of pressure sensitivity in at least 11 of the 18 tender points, in addition to the presence of widespread musculoskeletal pain for a period longer than 3 months and a general assessment of the patient’s health status. Pregnant women and children or adolescents were excluded. The Rob 2.0 tool from the Cochrane Collaboration was used to assess th...

The Influence of Opioids on Transcutaneous Electrical Nerve Stimulation Effects in Women with Fibromyalgia
Fibromyalgia (FM) is characterized by chronic widespread pain, fatigue, sleep disturbance, and reduced function67. Recommended treatment for individuals with FM includes both non-pharmacological and pharmacological strategies. Non-pharmacological strategies with good efficacy for pain management in FM include exercise, cognitive behavior therapy, education, and relaxation techniques1, 36. Pharmacological strategies for fibromyalgia may include three medications that have US Food and Drug Administration approval for management of fibromyalgia: duloxetine (Cymbalta)2, 50 and milnacipran (Savella)25, 37 and pregabalin (Lyrica)3.

Depression and Long-Term Prescription Opioid Use and Opioid Use Disorder: Implications for Pain Management in Cancer
Opinion statementPreventing depression in cancer patients on long-term opioid therapy should begin with depression screening before opioid initiation and repeated screening during treatment. In weighing the high morbidity of depression and opioid use disorder in patients with chronic cancer pain against a dearth of evidence-based therapies studied in this population, patients and clinicians are left to choose among imperfect but necessary treatment options. When possible, we advise engaging psychiatric and pain/palliative specialists through collaborative care models and recommending mindfulness and psychotherapy to all pa...

Pharmacological management of fibromyalgia: a Bayesian network meta-analysis
Expert Rev Clin Pharmacol. 2022 Feb 23:1-10. doi: 10.1080/17512433.2022.2044792. Online ahead of print.ABSTRACTINTRODUCTION: The identification of the most effective therapy for patients with fibromyalgia (FM) remains controversial. Thus, we conducted a Bayesian network meta-analysis to compare several drugs employed as pharmacological management for FM.AREAS COVERED: The following databases were accessed in October 2021: PubMed, Google Scholar, Embase, and Scopus. All the randomized clinical trials (RCTs) that compare two or more pharmacological management for fibromyalgia were accessed. Only studies involving a minimum o...

Evidence Needed for Efficacy of Antidepressant Medications Among Patients With Rheumatoid Arthritis
Ann Pharmacother. 2022 Jan 7:10600280211062271. doi: 10.1177/10600280211062271. Online ahead of print.ABSTRACTPatients with rheumatoid arthritis (RA) experience pain from inflammation, joint destruction, and neuropathy. Antidepressants may play a role among patients with RA and depression, fibromyalgia, or neuropathy to achieve desired outcomes. This commentary evaluated evidence for medications individually and identified important variables for future research. While we await the results of well-designed studies, a trial of duloxetine or milnacipran may be considered for patients with remnant pain and RA remission. Resea...

The Effect of Cognitive-Behavioral Therapy Versus Duloxetine on the Laboratory Indices of Inflammation in Fibromyalgia: A Randomized Controlled Trial
In this study, we aimed to evaluate the effect of cognitive-behavioral therapy (CBT) versus duloxetine on the serum levels of candidate cytokines, besides their impact on the clinical outcome measures. A total of 128 patients were randomized to either CBT or duloxetine group, 64 patients each. For the CBT group, traditional face-to-face CBT was offered in twice-weekly sessions over ten weeks. For the duloxetine group, the treatment was started with 30  mg/day duloxetine and continued with 60 mg/day duloxetine after a week, up to 10 weeks. The primary outcome measures were the serum levels of IL-6, IL-8, and TNFα. Second...

Suicidality and other severe psychiatric events with duloxetine: Re-analysis of safety data from a placebo-controlled trial for juvenile fibromyalgia
CONCLUSIONS: Antidepressants may put adolescents at risk of suicidality and other severe psychiatric disorders even when the treatment indication is not depression or anxiety.PMID:34397423 | DOI:10.3233/JRS-200033
