The overlap between vascular disease and Alzheimer¿s disease ¿ lessons from pathology
Kurt  Jellinger and Johannes Attems review the pathological overlap between Alzheimer's disease (AD)  and vascular disease, but although common factors exist, several questions remain unresolved, in particular regarding how these lead to onset and progression of the disease.

Pathogenesis of pulmonary arterial hypertension: lessons from cancer
Although the causal pathomechanisms contributing to remodelling of the pulmonary vascular bed in pulmonary arterial hypertension (PAH) are still unclear, several analogous features with carcinogenesis have led to the emergence of the cancer-like concept. The major similarities concern the altered crosstalk between cells from different tissue types, unexplained proliferation and survival of pulmonary smooth muscle and endothelial cells, the metabolic (glycolytic) shifts, and the association with the immune system. However, major differences between PAH and cancer exist, including the absence of invasion and metastasis, as w...

Caveat Emptor: Lessons Learned from the Endovascular Treatment of Complex Aortic Pathologies
The management of aortic aneurysm has evolved considerably over the last 20 years and few vascular surgeons in 1990 would have believed that within 25 years, aneurysm treatment would be close to becoming a day case intervention. Endovascular aortic aneurysm repair (EVAR) has largely replaced open repair of infrarenal abdominal aortic aneurysm (AAA) in anatomically suitable patients. This is despite the fact that while multicentre randomized trials revealed a clear survival advantage in the first 30 days (and again at 3 years), no late survival advantage favouring EVAR has been demonstrated.

Modulating the vascular response to limb ischemia: angiogenic and cell therapies.
Abstract The age-adjusted prevalence of peripheral arterial disease in the US population has been estimated to approach 12%. The clinical consequences of occlusive peripheral arterial disease include pain on walking (claudication), pain at rest, and loss of tissue integrity in the distal limbs; the latter may ultimately lead to amputation of a portion of the lower extremity. Surgical bypass techniques and percutaneous catheter-based interventions may successfully reperfuse the limbs of certain patients with peripheral arterial disease. In many patients, however, the anatomic extent and distribution of arterial occ...

Lessons Learned in the Surgical Treatment of Neurogenic Thoracic Outlet Syndrome Over 10 Years
Conclusion: Excellent results were seen in this surgical series reported for NTOS. Younger patients with shorter duration of symptoms with less narcotic use led to even better FRRS results in the second 5 years of surgical intervention. An established vascular practice for referrals for NTOS resulted in an increased number of appropriate patients for surgical intervention, requiring fewer lidocaine and/or Botox injections preoperatively.

Systemic effects of periodontitis: lessons learned from research on atherosclerotic vascular disease and adverse pregnancy outcomes
Studies conducted over the past 25 years have focussed on the role of periodontitis, an inflammatory condition of microbial aetiology that destroys the tooth‐supporting tissues, as a systemic inflammatory stressor that can act as an independent risk factor of atherosclerotic vascular disease (AVSD) and adverse pregnancy outcomes (APOs). It has been suggested that periodontitis‐associated bacteraemias and systemic dissemination of inflammatory mediators produced in the periodontal tissues may result in systemic inflammation and endothelial dysfunction, and that bacteria of oral origin may translocate into the feto‐pl...

Creating and Maintaining a Successful Service Line in an Academic Medical Center at the Dawn of Value-Based Care: Lessons Learned From the Heart and Vascular Service Line at UMass Memorial Health Care
The service line (SL) model has been proven to help shift health care toward value-based services, which is characterized by coordinated, multidisciplinary, high-quality, and cost-effective care. However, academic medical centers struggle with how to effectively set up SL structures that overcome the organizational and cultural challenges associated with simultaneously delivering the highest-value care for the patient and advancing the academic mission. In this article, the authors examine the evolution of UMass Memorial Health Care’s heart and vascular service line (HVSL) from 2006 to 2011 and describe the impact on its...

Bifurcation lesions: Bioresorbable vascular scaffold and drug coated balloon, an efficacy association. Lessons from optimal coherence tomography
The treatment of a coronary bifurcation lesion (CBL) is still a technical challenge for interventional cardiologist and although the use of a bioresorbable vascular scaffold (BVS) is an interesting and tempting treatment option, the risk of side branch (SB) complications and/or occlusion (SBO) after BVS implantation is considerable and it should be taken into account in the decision of revascularization modality.

Rebranding the Perioperative Surgical Home: Lessons From the Duke Experience
In this issue of the Journal of Cardiothoracic and Vascular Anesthesia, we introduce a new series that will focus on economics, regulation, and outcomes. While virtually all research involves outcomes in a broad sense, the goal of this section is to illuminate the complexity of the interplay among economics, regulation, and outcomes as they affect the field of anesthesiology. Complexity emerges when important priorities conflict and must be prioritized or creatively restructured based on our values, available evidence, resource limitations, and biases.

The search for an alternative to piped water and sewer systems in the Alaskan Arctic.
Abstract Forty-two communities in rural Alaska are considered unserved or underserved with water and sewer infrastructure. Many challenges exist to provide centralized piped water and sewer infrastructure to the homes, and they are exacerbated by decreasing capital funding. Unserved communities in rural Alaska experience higher rates of disease, supporting the recommendation that sanitation infrastructure should be provided. Organizations are pursuing alternative solutions to conventional piped water and sewer in order to maximize water use and reuse for public health. This paper reviews initiatives led by the Sta...

Vascular inflammation and low density lipoproteins: is cholesterol the link? A lesson from the clinical trials
Abstract For long time the role of LDL and inflammation in the pathogenesis of atherosclerosis have been studied independently from each other and only more recently a common platform has been suggested. Accumulation of excess cholesterol due to the presence of increased circulating LDL promotes endothelium dysfunction and activation, which is associated with increased production of pro‐inflammatory cytokines, overexpression of adhesion molecules, chemokines and C‐reactive protein (CRP), increased generation of reactive oxygen species and reduction of nitric oxide levels and bioavailability. All these processes favor t...

Lesson Learned with the Use of Iliac Branch Devices: Single Centre 10 Year Experience in 157 Consecutive Procedures
Absence of an adequate iliac seal rarely represents an absolute contraindication to endovascular abdominal aortic aneurysm repair. Iliac branch devices (IBD) are increasingly used in patients with extensive aorto-iliac aneurysmal disease, but few data are available on the long-term results of these procedures.

Small bowel capsule endoscopy revealing neuromuscular and vascular hamartoma of the jejunum: A case report
We report the case of a 58 -year-old man with extremely rare hamartomatous neurovascular lesion. Following recurrent episode of intestinal hemorrhage the patient underwent small bowel capsule endoscopy. Diagnoses: Diagnosed with small intestine neoplasia. Interventions: The patient underwent curative small bowel resection. Histologic diagnosis was neuromuscular and vascular hamartoma (NMVH). In the small intestine, neoplastic lesions are very rare (2%) and mostly malformative while the more frequent cause of cryptic digestive hemorrhage remains angiodysplasia (50%) . The preexisting NMVH was exacerbated by the use of...
