Alzheimer's disease and vascular deficiency: lessons from imaging studies and down syndrome.
Authors: Reed-Cossairt A, Zhu X, Lee HG, Reed C, Perry G, Petersen RB Abstract Down syndrome (DS) individuals are at high risk for developing Alzheimer's disease (AD) and consequently provide a unique opportunity to examine the factors leading to the onset of AD. This paper focuses on the neglected vascular parallels between AD and DS that can readily be examined in DS. Several recent AD studies provide evidence that internal jugular vein (IJV) reflux may result in white matter lesions and a 30% decrease in cerebrospinal fluid (CSF) clearance of amyloid-β. At the same time, studies analyzing the synthesis of amylo...

Trial Has Lessons for Search for 'the Perfect Anticoagulant'Trial Has Lessons for Search for 'the Perfect Anticoagulant'
Allergic reactions cut short a major trial of the REG1 anticoagulant-antidote drug pairing, but researchers seem undeterred in their search for a safer, controllable anticoagulant for use in PCI. Heartwire from Medscape

Lessons Learned
The Patient-Centered Outcome Research Institute (PCORI), an independent, nonprofit, nongovernmental organization that was authorized by Congress in 2010, is dedicated to making health research patient-centered and stakeholder-driven. PCORI does this through a focus on comparative clinical effectiveness research (CER). I had the opportunity to attend the PCORI Annual Meeting November 17-19, 2016, at the Gaylord Convention Center in National Harbor, Maryland, and was struck by what we can learn from organizations like PCORI as we create our research agenda, advocate for vascular access professionals, disseminate vascular-acc...

Vascular Ehlers–Danlos syndrome with cryptorchidism, recurrent pneumothorax, and pulmonary capillary hemangiomatosis-like foci: A case report
We report a case of an 18-year-old high school student, with a past history of cryptorchidism, presenting with recurrent pneumothorax. Diagnoses: Routine laboratory findings were unremarkable. Chest high resolution computed tomographic scan showed age-unmatched hyperinflation of both lungs, atypical cystic changes and multifocal ground glass opacities scattered in both lower lobes. His slender body shape, hyperflexible joints, and hyperextensible skin provided clue to suspicion of a possible connective tissue disorder. Interventions: The histological examination of the lung lesions showed excessive capillary prolifer...

Embolic retinal and choroidal vascular occlusion after peribulbar triamcinolone injection: A case report
We report an extremely rare case of embolic retinal and choroidal vascular occlusion after peribulbar triamcinolone injection in a patient with thyroid-associated ophthalmopathy. Diagnoses: Central retinal artery occlusion. Interventions: First, we performed a fundus examination in the patient. Triamcinolone embolus was observed in both retinal and choroidal vessels. Anterior chamber paracentesis and ocular massage combined with venous injections of alprostadil and Ginaton as well as an acupoint injection of compound anisodine were performed immediately. Sublingual glyceryl trinitrate and intraocular pressure-lowerin...

Type 2 Diabetes in Youth: New Lessons from the SEARCH Study
AbstractPurpose of ReviewThe purpose of this review is to provide an update on the recent body of evidence emerging for type 2 diabetes as identified through the SEARCH for Diabetes in Youth study.Recent FindingsThis body of evidence illustrates that type 2 diabetes continues to increase in incidence, although this increase may be partially attributable to increased surveillance. Disease management is influenced by the transition from adolescent to adult care and psychosocial factors may also contribute. This evidence also describes a high prevalence of disease-associated complications and comorbidities. Risk factors for c...

Klippel-Tr énaunay syndrome: a case report of a rare vascular disorder identified in a rural Canadian hospital.
Klippel-Trénaunay syndrome: a case report of a rare vascular disorder identified in a rural Canadian hospital. Rural Remote Health. 2019 Nov;19(4):5348 Authors: Yilmaz OK, Smithson LE Abstract CONTEXT: Klippel-Trénaunay syndrome (KTS) is a very rare congenital vascular anomaly. It is characterized by the presence of capillary malformation, venous malformation as well as limb overgrowth, generally affecting one extremity. Although clinical characteristics of KTS are well known, the epidemiology and pathophysiology still remain to be defined. Awareness of these disorders is important for rura...

Adipose tissue derived stromal vascular fraction as an adjuvant therapy in stroke rehabilitation: Case reports
Introduction: Stroke often causes residual hemiparesis, and upper extremity motor impairment is usually more disabling than lower extremity in those who are suffering from post-stroke hemiparesis. Cell therapy is one of the promising therapies to reduce post-stroke disability. Patient concerns: Three male participants were included in the study to investigate the feasibility and tolerability of autologous adipose tissue derived stromal vascular fraction. Diagnosis: All participants had hemiparesis after 1st-ever stroke longer than 6 months previously. Interventions: Under general anesthesia, liposuction of abdom...

Pathological changes in neurovascular units: Lessons from cases of vascular dementia.
Abstract Vascular dementia (VD) is the second leading cause of dementia after Alzheimer's disease (AD). The decrease of cerebral blood flow (CBF) to different degrees is one of the main causes of VD. Neurovascular unit (NVU) is a vessel-centered concept, emphasizing all the cellular components play an integrated role in maintaining the normal physiological functions of the brain. More and more evidence shows that reduced CBF causes a series of changes in NVU, such as impaired neuronal function, abnormal activation of glial cells, and changes in vascular permeability, all of which collectively play a role in the pa...

Sensors, Vol. 21, Pages 7843: The Internet of Cooperative Agents Architecture (X-IoCA) for Robots, Hybrid Sensor Networks, and MEC Centers in Complex Environments: A Search and Rescue Case Study
This article proposes X-IoCA, an Internet of robotic things (IoRT) and communication architecture consisting of a hybrid and heterogeneous network of wireless transceivers (H2WTN), based on LoRa and BLE technologies, and a robot operating system (ROS) network. The IoRT is connected to a feedback information system (FIS) distributed among multi-access edge computing (MEC) centers. Furthermore, we present SAR-IoCA, an implementation of the architecture for search and rescue (SAR) integrated into a 5G network. The FIS for this application consists of an SAR-FIS (including a path planner for UGVs considering risks detected by ...

Lessons learned from study of depression in cardiovascular patients in an acute-care heart and vascular hospital.
Abstract Depression is highly prevalent in patients with cardiovascular disease, but questions about the effectiveness of screening and intervention remain unanswered. To evaluate the effects of proactive intervention at an acute-care heart and vascular hospital, patients who reported depressive symptoms on admission were randomized to an active, counseling-based depression intervention plus standard care (referral to a primary or psychiatric care physician) or to standard care alone. Despite early termination of patient enrollment because of lower-than-expected recruitment rates, the project had a positive impact...

Reoperation after Vascular Ring Repair
The majority of patients having surgical intervention for a vascular ring have resolution of their symptoms. However, 5% to 10% of these patients develop recurrent symptoms related either to airway or esophageal compression and may require reoperation. In our series of 300 patients with vascular rings, we performed a reoperation on 26 patients, not all of whom were originally operated on at our institution. The four primary indications for reoperation were Kommerell diverticulum (n = 18), circumflex aorta (n = 2), residual scarring (n = 2), and tracheobronchomalacia requiring aortopexy (n = 4). All patients undergoing reop...
