Disseminated Streptococcus gallinaceus infection. A new breed of zoonotic Streptococcus
We report a case of Streptococcus gallinaceus bacteremia complicated by aortic valve endocarditis and lumbar osteomyelitis and paraspinal abscess in a patient with chicken exposure. The patient presented with progressive lower back pain and malaise. Blood culture was positive for Streptococcus gallinaceus. Magnetic resonance imaging (MRI) of the spine showed L2-L3 osteomyelitis with a compression fracture and paraspinal abscess. Transthoracic echocardiography revealed severe aortic insufficiency, 1-cm aortic valve echo density suspected to be a vegetation, and perforation of the right coronary cusp. He subsequently underwe...

Scurvy, abnormal MRI, and gelatinous bone marrow in an adolescent with avoidant restrictive food intake disorder
ConclusionsAlthough this patient had common manifestations of scurvy, including perifollicular petechial hemorrhagic rash, joint effusions, anemia, and recurrent fevers, she still underwent an extensive workup. Clinicians should be aware that scurvy can present with multiple symptoms that mimic infectious, rheumatic, oncologic and hematological disease. Clinicians should have a high index of suspicion for scurvy in patients with malnutrition and eating disorders.

Corpus spongiosum flap: a unique technique in the management of urosymphyseal fistula
Am J Clin Exp Urol. 2023 Feb 25;11(1):69-74. eCollection 2023.ABSTRACTUrosymphseal fistulas are a debilitating but treatable consequence of the combination of radiotherapy and endoscopic interventions in prostate cancer patients. Treatment includes fistula excision with a tissue flap to prevent communication of involved structures and provide structural and vascular support. We introduce a unique surgical approach of mobilizing and utilizing a corpus spongiosum flap in urosymphyseal fistula repair. A retrospective study identified two patients who underwent repair with a pedicled corpus spongiosum flap at our institution. ...

< em > Aspergillus fumigatus < /em > causing vertebral osteomyelitis in an immunocompetent patient: a case report and literature review
We describe a previously healthy 68-year-old male who was referred after 2 years of lower thoracic back pain and gibbus. His inflammatory markers and HIV test were normal. Imaging demonstrated bony destruction of T12/L1 and L2 with vertebral collapse. Following inconclusive CT-guided biopsy, he underwent reconstructive spinal surgery. Histopathology showed fungi and Aspergillus fumigatus was cultured. He was treated with isavuconazole 200 mg once daily for 12 months with a satisfactory clinical outcome. We present a summary of recently published cases of atraumatic Aspergillus vertebral osteomyelitis in immunocompetent pat...
