Emergency department experiences of people who use drugs who left or were discharged from hospital against medical advice
ConclusionsStructural vulnerabilities in ED can negatively impact the care received among PWUD. Findings demonstrate the need to consider how structural factors impact care for PWUD and to leverage existing infrastructure to incorporate harm reduction and a structural competency focused care. Findings also point to the need to consider how withdrawal and pain are managed in emergency department settings.

Unusual Case Presentation of Genital Herpes
CONCLUSION The process of removing nonviable tissue is called debridement. Debridement is only necessary when a herpetic ulceration is not healing on its own, which is when necrotic tissue that can harbor bacteria that may cause more extensive infections is formed. Removing the necrotic tissue speeds up healing and reduces the risk of further complications.PMID:36812284

New Treatment Paradigms in Primary Biliary Cholangitis
Primary biliary cholangitis (PBC) is an archetypal autoimmune disease. Chronic lymphocytic cholangitis is associated with interface hepatitis, ductopenia, cholestasis and a progressive biliary fibrosis. People living with PBC are frequently symptomatic, experiencing a quality-of-life burden dominated by fatigue, itch, abdominal pain, and sicca complex. Whilst the female predominance, specific serum autoantibodies, immune mediated cellular injury, as well as genetic (HLA and non-HLA) risk factors, identify PBC as autoimmune, to date treatment has focused on cholestatic consequences.

Multiple Duodenal Polyps in a Young Patient With Liver Cirrhosis
A woman in her mid-20s with recently decompensated liver cirrhosis secondary to autoimmune hepatitis was evaluated for liver transplantation. Esophagogastroduodenoscopy was performed to screen for esophageal/gastric varices. The patient had nonspecific mild abdominal pain and dyspepsia without diarrhea or constipation. Laboratory findings were remarkable for iron deficiency anemia with a hemoglobin level of 9.5 g/dL and a ferritin level of 10 μg/L. Esophagogastroduodenoscopy revealed multiple pedunculated polyps with superficial ulcerations ranging in size from 8 to 20 mm throughout the second part of the duodenum (Figure 1).

Hepatocellular carcinoma presenting as right shoulder pain
We report an uncommon presentation of liver cancer in a 33-year-old woman who presented with persistent right shoulder pain with a normal physical examination of her shoulder and normal X-rays. An abdominal ultrasound scan and a computed tomography scan subsequently revealed a large liver cancer in this patient who was subsequently found to be hepatitis B positive.
