A new perspective on perioperative coagulation management in a patient with congenital factor VII deficiency: A case report
Rationale: Congenital factor VII (FVII) deficiency is a rare coagulopathy. There are little clinical data for congenital FVII deficiency and no evidence-based medicine guidelines for treatment. Patient concerns: A 48-year-old woman with gallbladder stones suffered from intermittent abdominal pain for 2 months that was accompanied by an abnormally prolonged prothrombin time. Diagnoses: The woman was diagnosed as having cholecystolithiasis with cholecystitis and congenital FVII deficiency. Intervention: Preoperative evaluation confirmed the necessity of recombinant activated factor VII (rFVIIa) replacement therapy...

Success of intraoperative imaging and management of suspected choledocholithiasis without pre-operative bile duct imaging – a case series
ConclusionTraditionally patients presenting with suspicion of choledocholithiasis undergo preoperative MRCP/EUS and/or ERCP prior to eventual LC. We propose an alternative, more streamlined, pathway of treatment without requiring preoperative cholangiography, applicable to both elective and emergency patients.

A challenging case of laparoscopic synchronous bilateral adrenalectomy for Cushing's disease. Case report
ConclusionLaparoscopic synchronous bilateral adrenalectomy is feasible and effective and should be considered also in patients with wide preoperative comorbidities and challenging intraoperative management, as long as the patient is meticulously studied preoperatively. An approach including a multidisciplinary team discussion is recommended.

Diagnostic laparoscopy to investigate unexplained lactic acidosis in critically ill patients - a descriptive single centre cohort study
ConclusionUDL is useful and feasible investigation for unexplained LA in the critically ill patients and it can avoid unnessesory EL in many patients. We would recommend the use of UDL as a safe and feasible investigation in such patients.

Surgical management of laparoscopic cholecystectomy (LC) related major bile duct injuries; predictors of short-and long-term outcomes in a tertiary Egyptian center- A retrospective cohort study
ConclusionSepsis at referral, liver cirrhosis, and operative bleeding were significantly associated with both early and late morbidities after definitive management of laparoscopic cholecystectomy related major bile duct injuries, so it is crucial to avoid these catastrophes when doing those major procedures.

Rates of colorectal cancer detection in screening colonoscopy post appendicectomy in patients 50 years and over
ConclusionDespite the advancement in visualization of anatomy with CT scan and laparoscopic appendicectomy there is still a role for screening colonoscopy in patients greater than 50 years of age with appendicitis particularly if they have associated bowel symptoms or risk factors for CRC.

Endo-laparoscopic reconstruction of the abdominal wall midline with linear stapler, the THT technique. Early results of the first case series
ConclusionOur THT is a feasible technique, easily reproducible, and effective in the repair of primary defects of the midline and RAD, which greatly reduces the operating times and hospitalization allowing a quick return to active life.

Healthcare Workers’ Attitudes Toward Laparoscopic Surgery for Gallbladder Disease in the Caribbean
ConclusionA change in HCW attitudes is necessary to overcome the stagnation in laparoscopic practice. We propose short-term and long-term strategies that may potentially change HCWs’ attitudes. This study identified the HCW population to be targeted.

Synchronous renal cell carcinoma and pheochromocytoma presenting as acute decompensated heart failure
We report a 49-year-old woman who presented with a hypertensive crisis and acute heart failure and reduced left ventricular systolic function. An abdominal ultrasonography revealed a huge lobulated heterogeneous mass at the lower pole of the right kidney and a mass over the left suprarenal area, which were further delineated by magnetic resonance imaging. The patient underwent laparoscopic right radical nephrectomy and left adrenalectomy. Histopathological analysis confirmed the diagnoses of clear cell renal cell carcinoma of the right kidney with metastasis to the lung; and atypical pheochromocytoma of the left adrenal gl...
