Indications and outcomes for planned Cesarean hysterectomy in non-placenta accreta spectrum disorder patients: a systematic review
CONCLUSION: The most common indication for planned Cesarean hysterectomy was malignancy, with cervical cancer being the most frequent. Other indications included permanent contraception, uterine fibroids, menstrual disorders, and chronic pelvic pain. Common complications included bleeding, infection, and ileus. The surgical skill for Cesarean hysterectomy continues to be relevant in contemporary obstetrical practice for reproductive malignancy and several benign indications. Although the data indicate relatively safe outcomes, these studies show significant publication bias and therefore further systematic study of this pr...

Anesthesia Considerations for Placenta Accreta Spectrum
Am J Perinatol 2023; 40: 980-987 DOI: 10.1055/s-0043-1761637Anesthesiologists are critical members of the multidisciplinary team managing patients with suspected placenta accreta spectrum (PAS). Preoperatively, anesthesiologists provide predelivery consultation for patients with suspected PAS where anesthetic modality and invasive monitor placement is discussed. Additionally, anesthesiologists carefully assess patient and surgical risk factors to choose an anesthetic plan and to prepare for massive intraoperative hemorrhage. Postoperatively, the obstetric anesthesiologist hold unique skills to assist with postoperative pai...

Psychosocial predictors of change in sexual activity and sexual function after hysterectomy in women with pelvic pain
Potential impact on sexual function is an often-cited concern for many patients considering hysterectomy. Existing literature indicates that sexual function remains stable to slightly improved for most patients who undergo hysterectomy, but most studies demonstrate a small subset of patients in whom sexual function declines postoperatively. Unfortunately, there is a lack of clarity as to surgical, clinical, and psychosocial factors that may influence the likelihood of sexual activity postoperatively or magnitude and direction of change in sexual function.

Placenta Accreta Spectrum
Obstet Gynecol. 2023 Jun 8. doi: 10.1097/AOG.0000000000005229. Online ahead of print.ABSTRACTPlacenta accreta spectrum (PAS) is one of the most dangerous conditions in pregnancy and is increasing in frequency. The risk of life-threatening bleeding is present throughout pregnancy but is particularly high at the time of delivery. Although the exact cause is unknown, the result is clear: Severe PAS distorts the uterus and surrounding anatomy and transforms the pelvis into an extremely high-flow vascular state. Screening for risk factors and assessing placental location by antenatal ultrasonography are essential for timely dia...

Trends in surgical approach to hysterectomy and perioperative outcomes in Michigan hospitals from 2010 through 2020
The objective of this study was to determine the trends in surgical approach to hysterectomy over the last decade and compare perioperative outcomes and complications. This retrospective cohort study used clinical registry data from the Michigan Hospitals that participated in Michigan Surgical Quality Collaborative (MSQC) from January 1st, 2010 through December 30th, 2020. A multigroup time series analysis was performed to determine how surgical approach  to hysterectomy [open/TAH, laparoscopic (TLH/LAVH), and robotic-assisted (RA)] has changed over the last decade. Abnormal uterine bleeding, uterine fibroids, chronic pel...

Transcervical radiofrequency ablation of focal adenomyosis: pilot results
Conclusion: Transcervical RF ablation with the Sonata System may be a promising therapeutic alternative method to conventional procedures such as hysterectomy for the treatment of focal adenomyosis, disease which has limited therapeutic approach, and may enable a minimally invasive, uterine preserving option.PMID:37277101 | DOI:10.1080/02656736.2023.2217366

Why So Many Black Women Die in Pregnancy
BIRMINGHAM — Angelica Lyons knew it was dangerous for Black women to give birth in America. As a public health instructor, she taught college students about racial health disparities, including the fact that Black women in the U.S. are nearly three times more likely to die during pregnancy or delivery than any other race. Her home state of Alabama has the third-highest maternal mortality rate in the nation. Then, in 2019, it nearly happened to her. What should have been a joyous first pregnancy quickly turned into a nightmare when she began to suffer debilitating stomach pain. [time-brightcove not-tgx=”trueR...
