Current Evidence-based Approaches to Multimodal Pain Control and Opioid Minimization After Arthroscopic and Knee Preservation Surgery
J Am Acad Orthop Surg. 2023 Aug 21. doi: 10.5435/JAAOS-D-23-00342. Online ahead of print.ABSTRACTNonarthroplasty knee procedures are common and may cause a wide spectrum of postoperative pain, ranging from minimal to severe, depending on the patient, pathology, and procedure. Procedures include ligament repair and reconstruction, especially anterior cruciate ligament reconstruction, meniscal débridement, repair and transplant, periarticular osteotomy, and cartilage restoration. Multimodal analgesia regimens have been implemented successfully, but notable variation characterizes current protocols. Increased public and phys...

Effects of photobiomodulation by low-power lasers and LEDs on the viability, migration, and invasion of breast cancer cells
AbstractAmong the malignant tumors, breast cancer is the most commonly diagnosed worldwide, being the most prevalent in women. Photobiomodulation has been used for wound healing, swelling and pain reduction, and muscle repair. The application of photobiomodulation in cancer patients has been controversial. Therefore, a better understanding of radiation-induced effects involved in photobiomodulation on cancer cells is needed. Thus, this study aimed to investigate the effects of exposure to low-power lasers and LEDs on cell viability, migration, and invasion in human breast cancer cells. MCF-7 and MDA-MB-231 cells were irrad...

180. Can preoperative narcotic education reduce opioid consumption in patients undergoing spine surgery?
Treatment modalities for back pain include NSAIDs, PT, steroid injections, and surgery. Unfortunately, the first 3 days after surgery are often accompanied with severe back pain, prompting surgeons to prescribe opioids. After lumbar spine surgery, it has been found that two-thirds of patients become long term opioid users (defined as>90 days).  The 2018 Neers Award study demonstrated a significant reduction in postoperative narcotic consumption through preoperative narcotic education in patients after rotator cuff repair.

97. Gene expression changes with short- and long-term cannabinoid treatments in a spinal fusion model
The rising opioid epidemic is a public health crisis at the crossroads of orthopedic care and pain management. Medical marijuana is a potential non-opioid analgesic yet to be studied in the surgical setting and its effects on fracture repair are not fully understood. In addition to the analgesic effects of cannabinoids, studies have demonstrated potentially osteo-inductive properties, albeit controversial at the molecular level. Osteoblasts, osteoclasts and bone peripheral nerve terminals have been shown to have endocannabinoid receptors (CB1, CB2), comparably expressed to that of brain tissue.

Mini- or less-open sublay (E/MILOS) operation vs open sublay and laparoscopic IPOM repair for the treatment of incisional hernias: a registry-based propensity score matched analysis of the 5-year results
ConclusionsThe E/MILOS technique allows minimally invasive trans-hernial repair of incisional hernias using large standard meshes with low morbidity and good long-term results. The technique combines the advantages of sub-lay repair and a mini- or less-invasive approach.Trial registrationClinicalTrials.gov Identifier NCT03133000.

A case report of membrane induction combined with RIA technique for the repair of distal humerus segmentary bone defect
We report a 48-year-old female patient with a comminuted supracondylar intercondylar fracture of the left humerus due to a car accident. After 8 months of emergency debridement and suture with Kirschner wire internal fixation, the fracture was found to be unhealed with extensive bone defects. We used membrane induction combined with RIA technology to repair and reconstruct the patients, and found good osteogenesis through late follow-up. In theory, membrane induction technique can realize the reconstruction of large segmental bone defects, but the scope of repair is often limited by the lack of autologous bone source. The ...

Maintaining close ties with a former partner ’s family is a worthy choice, but you don’t have to ‘fix’ everything | Gaynor Parkin and Erika Clarry
It ’s often healthier to embrace the possibility of a new family dynamic and let go of punishing old patternsAfter several years of trying to salvage her long relationship with Rosa through couple ’s counselling and other efforts, Kara eventually made the difficult decision to end it. After the separation, Kara found herself estranged and isolated from Rosa’s extended family, whom she had shared close ties with for more than 30 years. The enduring pain of being excluded from family gatherings and communications, made her wonder if she was being punished for her choice to leave. Caught in a constant struggle, Kara osci...

Do we need mesh fixation at the laparoscopic hernia repair?
CONCLUSIONS: In the light of these findings, it is seen that TEP hernia repair can be performed safely without mesh fixation, although prospective randomized studies are needed.PMID:37597983 | DOI:10.1016/j.asjsur.2023.08.061
