Development of a guide to multidimensional needs assessment in the palliative care initial encounter (MAP)
Ensuring patient-centered palliative care requires a comprehensive assessment of needs beginning in the initial encounter. However, there is no generally accepted guide for carrying out this multidimensional needs assessment as a first step in a palliative intervention.

Online ahead of print.ABSTRACTOBJECTIVES: The Cut down, Annoyed, Guilty, and Eye opener- Adapted to Include Drugs (CAGE-AID) questionnaire (CA) is a validated screening tool used to assess risk for nonmedical opioid use (NMOU) in patients receiving opioids for cancer pain. Data on consistencies and variations in responses to the CA between different clinical settings are lacking. We evaluated the frequency and consistency in scoring of the CA among patients seen between the first inpatient consult (T1) and the first outpatient follow-up (T2) visits.METH...

Hiccups at the end of life in Parkinson's disease: a case report
Hiccups can be a debilitating symptom at the end of life. Treatment of hiccups can be particularly challenging in people with Parkinson's disease (PwP) as medications used to treat hiccups, such as dopamine antagonists, are often contraindicated. Hiccups can also be an adverse effect of some dopaminergic treatments used in Parkinson's disease (PD), common with rotigotine which is often used at the end of life (1,2). Hiccup prevalence at end of life in the general population is not known, but is estimated between 1-9% in those with advanced cancer (3).

Hospice and Palliative Medicine Fellowship Training in Mental Health: A Survey of Program Directors
Psychological and psychiatric care is a core domain of palliative care. Despite a high burden of mental health comorbidity among individuals with serious illness, the Accreditation Council of Graduate Medical Education gives little guidance about training hospice and palliative medicine (HPM) fellows in this domain of care. Currently, there is a lack of empiric data on HPM physician fellowship training in mental health topics.

Should urine drug screen be done universally or selectively in palliative care patients on opioids?
Urine drug screen (UDS) is a useful test conducted in patients receiving opioids for chronic pain to aid in validating patient adherence to opioid treatment and to detect any nonmedical opioid use (NMOU). One controversial topic regarding its use in palliative care is whether to conduct the test universally and randomly in all patients who are receiving opioids for chronic pain irrespective of their level of risk for NMOU, or to conduct the test selectively in only those with a high risk for engaging in NMOU behaviors.

A meeting with the world's suffering
It is a hot Tuesday afternoon, in the middle of the summer in a tropical country. I arrived at the oncology sector of the hospital to supervise an intern student, called Lourdes, who is in the last year of her nursing undergraduate course. Shortly afterwards I met her in the nursing room. We went to a quieter place in the sector and had a seat to talk about what her experience had been like in that first week of hospital internship in oncology. After a few words, Lourdes's big green eyes are filled with tears - the reason being: her contact with human suffering.
