Outcomes of familial abdominal aortic aneurysm repair in the Vascular Quality Initiative
Recent investigations have reported increased rates of aneurysm-related complications after endovascular aneurysm repair (EVAR) in familial abdominal aortic aneurysm (fAAA) patients. The purpose of this study was to evaluate the outcomes of open aortic repair (OAR) and EVAR in sporadic AAA (spAAA) and fAAA patients in the Society for Vascular Surgery Vascular Quality Initiative.

Video Presentation: Open Paravisceral Aneurysm
Aortic disease is on the top 20 leading causes of death in the United States. The open repair of abdominal aorta aneurism was first described in 1952 and since then has been the gold standard, but with the newer endovascular procedures (EVAR) being less invasive, the use has decreased. In 2011 Desai et al raised concerns that the volume of open aneurysm repair procedures now is not a conducive of an adequate vascular trainee education and that by 2020 vascular trainees are expected to do half of the OAR cases that were done in 2010.

Assessing the institutional learning curve for pararenal aortic repair using the fenestrated Anaconda endograft.
Conclusion Albeit the reduction in lethal complications and primary technical success rates were not statistically significant, a lower percentage of unconnected fenestrations and 30-day mortality per calendar year were observed over time. At the same time, an increasing complexity of performed cases, as reflected by an increasing number of fenestrations per case, was observed. Impact of aortic stent-graft oversizing on outcomes of the chimney endovascular technique based on a new analysis of the PERICLES Registry.

Humerus fracture and combined venous injury increases limb loss in axillary or subclavian artery injury.
CONCLUSIONS: The national rate of limb loss in trauma patients with arterial axillosubclavian vessel injury has not changed in the past decade. A combined artery/vein axillosubclavian vessel injury is an independent risk factor for limb loss, as well as open repair. However, the strongest risk factor is an open proximal humerus fracture. An associated brachial plexus injury is not associated with increased risk of limb loss. Readmissions after Firearm Injury Requiring Vascular Repair
Firearm injuries can be morbid and potentially have high resource utilization. Historically, trauma and vascular surgery patients are at higher risk for readmissions. Our goal was to assess the risk for readmission among patients undergoing vascular repair after a firearm injury.

The endovascular repair of blunt traumatic thoracic aortic injury in Asia: A systematic review and meta-analysis.
CONCLUSION: Short- to mid-term results for thoracic endovascular aortic repair for blunt traumatic thoracic aortic injury in Asia are encouraging. However, there is a distinct disparity in reporting across Asia. We propose a prospective database for outcome reporting post thoracic endovascular aortic repair in this patient population, and ongoing follow up to assess long-term efficacy of this treatment strategy. Descending thoracic endovascular aortic repair does not require cardiothoracic surgery support.
CONCLUSION: These data suggest that cardiothoracic surgery support is not required for descending thoracic endovascular aneurysm repair (D-TEVAR). Outcomes after endovascular repair of abdominal aortic aneurysm involving the renovisceral arteries: A multi-center follow-up study.

Vascular Smooth Muscle Cell Plasticity and Autophagy in Dissecting Aortic Aneurysms.

Safety of EndoAnchors in real-world use: A report from the Manufacturer and User Facility Device Experience database.
CONCLUSION: In early post-FDA approval use in a real-world setting, the EndoAnchor system is associated with a low rate of adverse events. Device dislodgement and embolization remain the most common adverse events. With increasing use of these devices in more difficult anatomy, careful patient selection and careful attention to technique may help to reduce these events even further. Trends in management and outcomes of vascular emergencies in the nationwide inpatient sample.
Conclusions: VE frequency increased with a decrease in overall mortality over time. Overall hospital stay has decreased but with an increase in the cost of care. Open surgical mortality for VE has also decreased overall, suggesting perioperative care improvements, with the exception of ruptured thoracoabdominal aortic aneurysm. Endovascular utilization for VE has significantly increased; associated with lower mortality for most VE, although an increase in hospital mortality after endovascular repair of rTAA was seen. Identification of Parameters Influencing the Vascular Structure Displacement in Fusion Imaging during Endovascular Aneurysm Repair
ConclusionsThis study identified the sources of fusion error after insertion of rigid material during EVAR. As the sharpest angulation between aneurysm neck and sac increases, the overall accuracy of the fusion might be affected.
