Pain-motor integration in chronic pain: A neurophysiological study
CONCLUSIONS: M1 plasticity, as tested by Laser-PAS, is altered in patients with chronic pain, possibly reflecting abnormal pain-motor integration processes.SIGNIFICANCE: Chronic pain is associated with a disorder of M1 plasticity raising from abnormal pain-motor integration.PMID:37595480 | DOI:10.1016/j.clinph.2023.07.010

Infraclavicular Catheter in MELAS Syndrome for Analgesic Purposes
M Onay, T Tanyel Kiremitçi, G Erdoğan Kayhan, D İlhan Algın, MS GüleçNeurology India 2023 71(4):764-766 MELAS syndrome is defined as mitochondrial myopathy accompanied by encephalopathy, lactic acidosis, myoclonus, stroke-like episodes. It has a progressive course, multi-systemic effects and severe complications. Myoclonic contractions are unresponsive to many anti-epileptic drugs; these contractions and spasms may lead to severe pain. Systemic analgesic drugs are not sufficient to control pain. Therefore, continuous brachial plexus blockage may be pr...

Headache disorders and relevant sex and socioeconomic patterns in adolescents and young adults across 204 countries and territories: an updated global analysis
ConclusionsOverall, the global burden of headaches in adolescents and young adults largely increased from 1990 to 2019. Although slight declines were observed in sex differences, they remained significant and challenging. The positive correlations between headache and socioeconomic background among young people were relatively inconsistent with previous investigations, and several related hypotheses were proposed for explanation. Interdisciplinary actions involving education, policy- and law-making, and basic medical practice are desperately needed to further fight against the headache burden, promote gender equality in he...

Lipids, lipid-modifying drug target genes and migraine: a Mendelian randomization study
ConclusionIn the study, it was observed that among the 10 lipid-lowering drug targets investigated, LPL and HMGCR showed significant associations with migraine risk. These findings indicate that LPL and HMGCR have the potential to serve as candidate drug targets for the treatment or prevention of migraines.

Advanced brain MRI may help understand the link between migraine and multiple sclerosis
ConclusionStudies suggest  a migraine history predisposes patients to develop MS. Advanced brain MR imaging may shed light on shared and distinct features, while helping us better understand mechanisms underlying both disease entities.

Patients with cluster headache show signs of insomnia and sleep related stress: results from an actigraphy and self-assessed sleep study
ConclusionOur analysis suggests that sleep is negatively affected in CH both in cluster bout and in remission, manifesting in symptoms consistent with insomnia such as prolonged sleep latency and increased time in bed.

Multidisciplinary interventions increase weekly working hours and quality of life in persons with post-concussion syndrome
CONCLUSION: After the intervention, participants increased weekly workhours and improved in many aspects of life - including quality of life, performance in everyday activities, fatigue and depression. Perception of pain intensity was not improved.PMID:37590325 | DOI:10.1080/01616412.2023.2247302

Non-CGRP Antagonist/Non-Triptan Options for Migraine Disease Treatment: Clinical Considerations
AbstractPurpose of ReviewAlthough the association between CGRP and migraine disease is well-known and studied, therapies can target other pathways to minimize migraine symptoms. It is important to understand the role of these medications as options for migraine treatment and the varied mechanisms by which symptoms can be addressed. In the present investigation, the role of non-CGRP antagonist/non-triptan options for migraine disease therapy is reviewed, including NSAIDs, ß-blockers, calcium channel blockers, antidepressants, and antiepileptics. Pharmacologic therapies for both acute symptoms and prophylaxis are evaluated,...

Phenotype of new daily persistent headache: subtypes and comparison to transformed chronic daily headache
ConclusionsWhilst there is overlap in the phenotype of NDPH and T-CDH, the differences in migrainous, cranial autonomic symptoms, and vulnerability to medication overuse suggest that they are not the same disorder. NDPH may be fractionated into three sub-phenotypes, which require further investigation.
