The potential harm of not following clinical practice guideline recommendations
J Am Dent Assoc. 2023 Jun 27:S0002-8177(23)00276-3. doi: 10.1016/j.adaj.2023.05.002. Online ahead of print.ABSTRACTBACKGROUND: Clinical practice guidelines (CPGs) provide recommendations for clinicians on the basis of best evidence. CPGs are often not followed because of numerous barriers, including lack of awareness, inability to understand recommendations, and problems with implementation.CASE DESCRIPTION: A case report is presented in which treatment of a patient's incipient caries lesions may not have followed a CPG available to practitioners, recommending conservative nonrestorative medical interventions. The resultin...

Effect of premedication on the success of inferior alveolar nerve block in patients diagnosed with irreversible pulpitis: An umbrella review
Successful management of pain during endodontic treatment is essential for both patients anddentists. Achieving adequate pulp anesthesia in mandibular molars is a significant concern for patientswith irreversible pulpitis during endodontic treatment. The increased sensitization of nociceptorsdue to inflammation decreases the success of inferior alveolar nerve block (IANB). The main focusis on reducing inflammation before delivery of local anesthesia to increase the success of anestheticdrugs. This umbrella review aimed to revise, qualify and summarize the existing body of evidenceon the effect of premedication on IANB in p...

A Patient with Moderate-to-Severe Temporomandibular Joint Degenerative Joint Disease and Unilateral Joint Pain Presents for Oral Medicine Consult
A patient presents with severe one-sided temporomandibular joint (TMJ) pain. Workup reveals a diagnosis of TMJ degenerative joint disease, articular disc disorder with reduction, and myofascial pain of the muscles of mastication. A Pregnant Patient (First Trimester) Reporting for Pain in Relation to the Maxillary Left First Molar Was Prescribed a Full Mouth Series Radiographs in the Dental Office
This case scenario shows the value of conducting a thorough clinical examination that will direct appropriate radiographic selection and prescription criteria to be able to arrive at a diagnosis. Proper management of a patient ’s chief complaint and imaging needs during pregnancy is of utmost importance. It is prudent to limit ionizing radiation during the first trimester to what is minimally needed and defer elective imaging until after the birth of the baby. It is important for dental health care providers to do what is necessary for the patient for the emergent situation and postpone all elective imaging and follow th...

A Patient with Cerebral Palsy Presents for Evaluation of Third Molar Pain
Cerebral palsy (CP) is a developmental disorder caused by brain trauma in utero or within the first few days of life, although symptoms may not develop until early infancy. Each of the 4 types of CP has its own signs and symptoms and can present unique challenges to accessing and providing dental care. Providers may be reluctant to treat these individuals due to uncontrolled body movements, primitive reflexes, varying mental capabilities, seizures, visual and hearing impairments, dysphagia, and dysarthria.

Patient with Crohn's Disease Presents for Pain in Relation to Maxillary Teeth
Crohn's  disease has been associated with poor oral health and oral health–related quality of life. Myriad-specific and nonspecific oral lesions have been associated with Crohn's disease. Oral lesions in patients with Crohn's disease may be a source of referred pain, especially if mucosal ulcerations o r orofacial granulomatosis are involved. The dental provider can play an important role in evaluating for and/or managing oral lesions in patients with Crohn's disease and thereby improving patients’ oral health and quality of life.

A Diabetic Patient with Acute Osteomyelitis Presenting with Jaw Pain and Submandibular Swelling
This case scenario shows the value of applying relevant imaging and the selection of appropriate antibiotic via culture and sensitivity before prescribing especially when dealing with a patient with type 1 Diabetes Mellitus. When confronted with a diagnosis of acute osteomyelitis, it is always better to refer the patient to a hospital for admission where the management and any other intervention is easily accomplished. Admission to the hospital, culture and sensitivity, and appropriate intravenous antibiotics may have limited the progress of the infection and ultimately may have prevented the sequestration in this patient,...

A Patient Presents for Dental Extraction and Goes into Sickle Cell Crisis in the Dental Chair
Sickle Cell Disease is an inherited autosomal recessive hemoglobinopathy associated with multiorgan damage. This single gene disorder involves one DNA base pair alteration, producing HbS. The sickle-shaped cells form when deoxygenated in the capillaries. The resulting RBC stasis leads to ischemia and pain, and acute and chronic organ damage. Patients with SCD presenting to a dental office need careful examination to rule out any current infections, neurologic deficits, or other organ involvement before formulating a dental treatment plan to avoid prolonged and complicated procedures. Early intervention and dental anxiety m...

A Patient with a History of Right-Sided Stroke and Hemiplegia, in a Wheelchair, Presents with a Complaint of Upper Left Tooth Pain
Patients with a history of stroke often present with numerous neurologic deficits and varying degrees of disability. Ambulation problems requiring the use of a wheelchair can make accessing and receiving dental care difficult for these patients. Side effects from medications can compromise their oral health and complicate care. Possible dexterity limitations decrease their ability to maintain their oral health. Innovative care plans and adaptations may be needed to accommodate the needs of these patients but care generally can be provided safely and effectively in the outpatient dental setting.

Prevalence of signs and symptoms related to temporomandibular disorders and orofacial pain in patients indicated for orthognathic surgery: a meta-analysis
ConclusionApproximately 1 in 2 patients with dentofacial deformity presents some sign and symptom related to TMD. Myofascial pain and headache may be present in approximately a quarter of patients with dentofacial deformity.Clinical relevanceA multidisciplinary treatment is necessary for these patients, involving a professional with expertise in the management of TMD.

Cryotherapy and pain intensity during endodontic treatment of mandibular first permanent molars with symptomatic irreversible pulpitis: A randomized controlled trial
ConclusionsThe cryotherapy application boosted the efficacy of pulpal anesthesia of the mandibular first permanent molars with SIP in patients below the age of 18 years. Additional anesthesia was still necessary for optimal control over pain.Clinical relevancePain control during endodontic treatment of primary molars with irreversible pulpitis (IP) is a significant factor in a child ’s behavior in the dental office. Although the inferior alveolar nerve block (IANB) is the most commonly used technique to anaesthetize mandibular dentition, we found its success rate to be relatively low during endodontic treat...
