More time in a community setting: A service evaluation of the impact of intrathecal drug delivery systems on place of care of patients with cancer pain
CONCLUSION: In patients with advanced cancer, the successful establishment of intrathecal analgesia is associated with more time in the community and a lower morphine equivalent daily dose at end of life. The study has low numbers, and the sample was retrospectively selected. Nevertheless, these findings suggest the initial investment of time in an inpatient setting may be beneficial. Common conditions include xerosis, pressure ulcers, intertrigo, superficial fungal infections, telogen effluvium, pruritus, herpes zoster, eczematous disorders and edema. On an early fall morning, I set out for a coastal town in Kerala, India, with Pallium India's dedicated home care team. The road was narrow and tortuous as it contoured the coast. I joined them as part of my research exploring how community-based organizations like Pallium mobilized to meet the enormous primary care and palliative care needs of populations at the margins of health care access. Our next patient, Mathew, was due for a urinary catheter change.
