Conclusions:
There is a strong association between the presence of a haplotype in genes mapping between HLA-E and HLA-C and AS due to an increase of homozygous markers in patients. The strongest association however, is with the HLA-E functional polymorphism rs1264457. Since HLA-E is the ligand for the NKG2A receptor, these data point to the natural killer (NK) activity as possible player in the pathogenesis of AS. (Source: Arthritis Research and Therapy)

Abstract Etanercept is singular among anti-tumor necrosis factor alpha (TNF-α) agents, for being a soluble antibody to both TNF and
lymphtoxin-alpha. The long-term neutralization of two cachexins by etanercept would theoretically compromise early detection
of malignancy. This case reports a patient who was treated by etanercept for 21 months due to ankylosing spondylitis. Metastatic
malignancy of unknown origin developed, and silently led the patient to lethal hepatic rupture. With an example of a malignancy
masking effect of soluble TNF receptor, this article questions a need for vigilant attention to...

Cardiovascular involvement is one of the extra-articular manifestations of ankylosing spondylitis. Of these, aortic disease and aortic regurgitation are the most commonly associated cardiovascular disorders. Since 2005, three male patients with ankylosing spondylitis were referred to The Chaim Sheba Medical Center for surgical treatment of cardiovascular associations. The diagnosis of ankylosing spondylitis was evidenced by radiography and/or laboratory tests of C-reactive protein or HLA-B27. Apart from the usual cardiac manifestations including heart block and left ventricular hypertrophy, usual associated disorders s...

The advent of high throughput SNP genotyping methods has advanced research into the genetics of common complex genetic diseases such as ankylosing spondylitis rapidly in recent times. The identification of associations with the genes IL23R and ERAP1 have been robustly replicated, and advances have been made in studies of the MHC genetics of AS, and of KIR gene variants and the disease. The findings are already being translated into increased understanding of the immunological pathways involved in AS, and raising novel potential therapies. The IL23R findings and subsequent immunological investigations suggest that targeted ...

Osteocytes are considered to be sensors of bone damage and regulators of bone mass by specifically expressing sclerostin, an inhibitor of bone formation. The contribution of osteocytes in regulating local bone remodeling in arthritis is unknown. The aim of this study was to investigate the role of osteocytes as contributors to bone remodeling in ankylosing spondylitis (AS).Sclerostin expression and osteocyte death were assessed by immunohistochemistry in joints derived from patients with AS, patients with rheumatoid arthritis (RA), and patients with osteoarthritis (OA), as well as from control subjects. In addition, the se...

The results of a recent genome-wide association study have shown that ERAP1 and IL23R are associated with ankylosing spondylitis (AS) in Caucasian populations from North America and the UK. Based on these findings, we undertook the current study to investigate whether single-nucleotide polymorphisms (SNPs) covering the genes ERAP1 and IL23R are associated with AS in a Han Chinese population.A case-control study was performed in Han Chinese patients with AS (n = 527) and controls (n = 945) from Shanghai and Nanjing. All patients met the modified New York criteria for AS. The Sequenom iPlex platform was used to genotype case...

Conclusions: 1. Etidronate has anti-resorptive and anti-inflammatory properties in men with AS. 2. There was no correlation between CRP and bone markers, sRANKL, OPG and sRANKL/OPG ratio which may suggest a lack of relationship between inflammation and bone remodeling in AS. 3. Etidronate treatment does not influence the number of proinflammatory monocytes CD14+CD16+ in peripheral blood and increases the PHA-stimulated cytokine production by peripheral blood mononuclear cells, with prevalent immunosuppressive IL-10 synthesis. We report here a case of IgA nephropathy associated with AS that became symptomatic, whereas infliximab therapy efficiently controlled the rheumatological disease. This suggests that even though infliximab therapy effectively controls rheumatological manifestations, it may not be able to prevent IgA nephropathy associated with AS. Thus, this case report illustrates the complexity of the physiopathology of both diseases. (Source: Nephrology Dialysis Transplantation)

CONCLUSION: Relevant aspects of the influence of AS are not covered by the classic disease-specific instruments. In particular, the influence of AS on socializing and leisure and the relevance of environmental and personal factors are not adequately assessed by available instruments.

CONCLUSION: The prevalence of permanent work disability in Spanish patients with AS is significant, and the main factors related to it are age, disease duration, structural damage, and physical functioning. Longitudinal studies are needed to confirm these results.

The past 10 years has seen the description of families of receptors which drive pro-inflammatory cytokine production in infection and tissue injury. Two major classes have been examined in the context of inflammatory joint disease - the Toll-like receptors (TLRs) and NOD-like receptors (NLRs). TLRs such as TLR2 and TLR4 are being implicated in the pathology of rheumatoid arthritis, ankylosing spondylitis, lyme arthritis and osteoarthritis. Nalp3 has been identified as a key NLR for IL-1beta production and has been shown to have a particular role in gout. These findings present new therapeutic opportunities, possibly allowi...

Many of the chronic inflammatory and degenerative disorders that present to the clinical rheumatologists have a complex genetic aetiology. Over the last decade there has been a dramatic improvement in technology and methodology that has accelerated the pace of gene discovery in complex disorders in an exponential fashion. In this review, we focus on rheumatoid arthritis, SLE and ankylosing spondylitis and describe some of the recently described genes that underlie these conditions and the extent to which they overlap. The next decade will witness a full account of the main disease susceptibility genes in these diseases and...

Abstract Both in ankylosing spondylitis (ASP) and psoriatic arthritis (PsA), osteopenia is present in one-third of women and men, whereas
osteoporosis mainly affects men, even in their 30 s. Subclinical gut inflammation has been described in patients with AS or
PsA. Joint involvement also occurs with other gastrointestinal diseases such as celiac disease. We tested the hypothesis,
whether elevated serum levels of human anti-tissue-transglutaminase-IgA (htTG) are associated with changes in disease activity,
vitamin D metabolism and bone mineral density (BMD) in patients with ASP and PsA. In a cross-secti...

Conclusions:
Synovial TREM-1 expression is increased in septic arthritis and RA. In patients with acute inflammatory arthritis, elevated SF sTREM-1 levels may point the clinician to a diagnosis of septic arthritis or RA. In RA patients, targeting TREM-1 may have therapeutic benefits by reducing local proinflammatory cytokine and chemokine release. (Source: Annals of the Rheumatic Diseases)

Conclusion:
On a group level using the mini-BASDAI did not result in an advantage to assess disease activity or in the subgroup without peripheral involvement. In only approximately 5% of AS patients was the mini-BASDAI superior to the original BASDAI. (Source: Annals of the Rheumatic Diseases)
