Pediatric cardiac surgery: Effect of a miniaturized bypass circuit in reducing homologous blood transfusion

To the Editor: I read with great interest the excellent article by Koster and colleagues. We have recently published our experience in treating 150 pediatric patients with a 120-mL prime volume miniaturized bypass circuit, vacuum-assisted venous drainage, and intermittent warm blood microplegia. Our results, although less impressive than those of Koster and colleagues, confirmed the efficiency of a small prime volume for reduction of allogeneic blood transfusion. With a nadir hemoglobin value of 8 g/dL during cardiopulmonary bypass, we demonstrated the good tolerance of hemodilution using 2 markers: preoperative and posto...

Serum ferritin level changes in children with sickle cell disease on chronic blood transfusion are nonlinear and are associated with iron load and liver injury

Chronic blood transfusion is increasingly indicated in patients with sickle cell disease. Measuring resulting iron overload remains a challenge. Children without viral hepatitis enrolled in 2 trials for stroke prevention were examined for iron overload (STOP and STOP2; n = 271). Most received desferrioxamine chelation. Serum ferritin (SF) changes appeared nonlinear compared with prechelation estimated transfusion iron load (TIL) or with liver iron concentrations (LICs). Averaged correlation coefficient between SF and TIL (patients/observations, 26 of 164) was r = 0.70; between SF and LIC (patients/observations, 33 of 47) w...

Estimates for dengue transmission during blood donations have been upsetting in diverse geographical locations. During the 2004 dengue virus (DENV) outbreak in Cairns, Australia, the risk of collecting a viremic donation could have been as high as 1 in 1028 during the peak of the outbreak . Using a mathematical model and assuming a ratio of asymptomatic to symptomatic infections of 2:1–10:1, the risk for dengue-infected blood transfusions in Singapore, a city-state in Asia, during 2005 was estimated to be 1625–6/10,000 blood transfusions . Transfusion practice is reported to differ considerably between countries. Comparisons often rely on transfusion rates, incidence - or prevalence rates. Transfusion data, demographic data and patient data were retrospectively combined to calculate sex and diagnosis specific and age standardized 1-YPPR s of RBC transfusion for the complete population in a Danish county. During the calendar year of 2006, 4427 patients received RBC transfusion in Funen County. The crude 1-YPPR of RBC transfusion was 9.2/1000 citizens. M...

Blood passport has been suggested as an indirect tool to detect various kinds of blood manipulations. Autologous blood transfusions are currently undetectable, and the objective of this study was to examine the sensitivities of different blood markers and blood passport approaches in order to determine the best approach to detect autologous blood transfusions. Twenty-nine subjects were transfused with either one (n=8) or three (n=21) bags of autologous blood. Hemoglobin concentration ([Hb]), percentage of reticulocytes (%ret) and hemoglobin mass (Hbmass) were measured 1 day before reinfusion and six times after reinfusion....

Anaemia in chronic kidney disease (CKD) is a complex disease that requires an integrated approach to incorporate both diagnostic and therapeutic interventions and to address the different facets of its aetiology and pathophysiology. The advent of erythropoiesis stimulating agents (ESA) has revolutionised the therapy of anaemia of CKD, and has resulted in a significant decline in the need for blood transfusions in CKD patients. The routine application of ESA has also led to the need for concomitant iron supplementation. ESA and iron therapy now form the cornerstone of anaemia management in CKD. Intravenous iron administrati...

From 362 thalassemia cases referred to adult thalassemia clinic of the Iranian blood transfusion organization (IBTO) for genotyping, 103 cases (28.5%) had a common primary disease factor, IVSII-1 mutation in homozygous state. 61 (59.2%) of these individuals represented thalassemia major and 42 (40.8%) thalassemia intermedia clinical phenotype. To re-evaluate our current diagnostic criteria, XmnI(G)gamma polymorphism and coexistence of alpha thalassemia mutations, frequently recalled as important factors modifying the clinical phenotype of homozygous beta zero thalassemia cases in our country, were examined in both grou...

In conclusion, our results suggest that activation of cAMP-signaling system may repress p53-dependent
apoptosis in malignant cells exposed to doxorubicin.

Conclusion: In-hospital mortality of HS is low, and blood transfusion was performed in over one third of patients. There appear to be several risk factors that can predict the likelihood of transfusion and may warrant close observation before hemispherectomy. (Source: Epilepsy Research)

Cellular prion protein in blood platelets associates with both lipid rafts and the cytoskeleton.

The recently shown transmissibility of variant Creutzfeldt-Jakob disease (vCJD) by blood transfusion emphasises the need for better understanding of the cellular prion protein (PrPc) in blood. A substantial amount of cell-associated PrPc in blood resides in platelets. Platelet activation leads to up-regulation of PrPc on the platelet surface and its release on exosomes and microparticles. The sub-cellular localisation and function of platelet PrPc, however, is poorly understood. In the present study, we investigated the association of PrPc with platelet lipid rafts and the platelet cytoskeleton. Immuno-fluorescence mic...

Hepatitis C virus screening was conducted among 8,226 residents 25-65 years of age in 4 counties of China; virus prevalence was 0.9%. A subsequent case-control study indicated blood transfusion (odds ratio [OR] 4.55), esophageal balloon examination (OR 3.78), and intravenous injection (OR 5.83) were associated with infection.

Abstract: Exposure to human leukocyte antigens (HLA) via blood transfusions, pregnancies, and previous transplantations can result in anti-HLA antibody production. The presence of anti-HLA antibodies in recipient sera before transplantation is an important risk factor. To demonstrate the anti-HLA antibody status of Turkish end-stage renal disease (ESRD) patients, 674 patients (mean age, 40.35 ± 13.15 years; female/male, 328/346) were enrolled into the study. Anti-HLA antibody screening and identification tests were performed using an enzyme-linked immunosorbent assay (ELISA) method. The panel-reactive antibody (PRA)-negat...
