In this issue of The Journal, Railey et al report a quarter-century experience at Duke University Medical Center and the long-term outcome of non-ablative bone marrow transplantation for severe combined immunodeficiency (SCID) in a cohort of 161 infants who had related, predominantly non-identical, donors. The donor bone marrow gives rise to immune cells that attack the leukemia (a response known as a graft-versus leukemia [GVL] response). In some cases, however, the immune cells generated by the donor bone marrow attack other cells in the patient's body and this highly toxic response is known as graft-versus-host disease (GVHD). Conclusion
This research indicates the potential of gene therapy in ALD. The study is important as, although the disease is rare, its effects are serious and fatal if not treated. The boys have been followed for about 30 months and the longer-term effects of the treatment are not yet clear. As the authors note, larger studies with longer follow-up are needed to determine the longer-term effects of this treatment and any risks associated with it. Further studies are also needed to determine how well the therapy compares to stem-cell transplant in boys who have a matched donor.

Recent reports describe hematopoietic abnormalities in mice with targeted instability of the mitochondrial genome. However, these abnormalities have not been fully described. We demonstrate that mutant animals develop an age-dependent, macrocytic anemia with abnormal erythroid maturation and megaloblastic changes, as well as profound defects in lymphopoiesis. CONCLUSIONS: Although published standards are under consideration, practice variation exists across transplantation centers. Whether the variation is caused by a lack of compliance with published guidelines or by the poor delineation of details for providers to translate the guidelines into practice is not known. INTERPRETATION: Identifying gaps in the literature and inconsistencies in HSCT practices is an important first step in designing evidence-based projects that can be used to standardize practice and link best practices to improved patient outcomes.

Plasmacytoid dendritic cells (PDCs) have perplexed pathologists for decades, undergoing multiple adjustments in nomenclature as their lineage and functions have been characterized. Although PDCs account for less than 0.1% of peripheral blood mononuclear cells, they serve as a principal source of interferon-[alpha] and are also known as interferon-I producing cells (IPCs). We describe the case of a 23-month-old girl with combined immunodeficiency syndrome and a left ventricular aneurysm. Due to the size of the aneurysm and development of an intramural thrombus, repair was performed after confirmation of bone marrow transplant engraftment. Endoventricular circular patch plasty (Dor procedure) was performed, with excellent outcome and normalization of ventricular geometry and function.

CD81 is a component of the CD19/CD21 co-receptor complex in B cells. However, the role of CD81 in B cell activation has not been clearly elucidated. Here, we demonstrate that Cd81–/– B cells stimulated via their B cell receptor fluxed higher intracellular-free calcium ion along with increased phosphorylation of spleen tyrosine kinase and phospholipase gamma 2. CONCLUSION: HPA-1a antibodies were associated with protracted PLT-transfusion dependence and significant hemorrhagic complications.
