CONCLUSION: BNP and echocardiography may be useful determinants of the short-term outcome for patients with PE, together with clinical findings. We describe a case of cardiogenic shock caused by a very late drug-eluting stent (DES) thrombosis. The patient underwent emergent
coronary artery bypass grafting (CABG) and was discharged home 15 days after the operation. The incidence of stent restenosis
had been reduced by the use of DES, but the Achilles’ heel of DES is represented by a higher rate of stent thrombosis. In
our case, the DES thrombosis occurred 5 years after its implantation, underlining the importance of prolonged dual antiplatelet

Severe cardiomyopathy following treatment with the tumour necrosis factor-{alpha} inhibitor adalimumab for Crohn's disease

Adalimumab belongs to the group of tumour necrosis factor- inhibitors and has been approved for the treatment Crohn's Disease since 2007. Herein we report a severe adverse reaction to adalimumab in a 25-year-old female patient. One week after the initial-dose of adalimumab (160 mg), which was initiated due to an acute exacerbation of Crohn's disease, the patient developed a fulminant cardiomyopathy. In severe cardiogenic shock, the patient required an extracorporeal membrane-oxygenation system for 8 days until cardiac recovery. (Source: European Journal of Heart Failure)

CONCLUSIONS: Patients with an acute myocardial infarction and emergency aortocoronary CABG have an elevated operative risk. Logistic EuroSCORE overestimates the mortality rate. Three different risk groups can be defined, in which creatine kinase-MB/h-ratio and ST-segment elevation can more accurately predict operative risk.

CONCLUSIONS: The percTH was reliable, with no mechanical device failures and minimal associated adverse events. We support the use of the percTH in the BTD mode, allowing time for a more complete evaluation of neurologic and end-organ status without the added expense and morbidity of a long-term VAD.

We describe a patient who was a heart transplant candidate with biventricular failure who underwent CardioWest total artificial heart-temporary (SynCardia Inc, Tucson, AZ) implantation with extracorporeal membrane oxygenation to manage the patient's subsequent respiratory failure. After respiratory and hemodynamic stabilization, the CardioWest total artificial heart-temporary served as a successful 62-day bridge-to-heart transplantation.

Conclusion In women, STEMI was more often complicated by cardiogenic shock when compared to men. However, the use of early reperfusion
therapy did not differ between the sexes. Primary PCI was associated with the best outcome in female patients with STEMI complicated
by cardiogenic shock and is therefore the therapy of choice.

Conclusion This prospective study showed no differences in the incidence of developing CIN in patients undergoing PCI for STEMI or NSTEMI,
but the predisposing factors, however, differed significantly. Although STEMI patients needed significantly more contrast
medium for revascularisation, they did not develop CIN more often. CIN was associated with higher in-hospital complication
rate and mortality. Thus, better preventive strategies according to the different predisposing factors leading to CIN are
needed to reduce morbidity and mortality, especially in high risk patients.

Unprotected left main coronary disease in ACS is associated with high mortality, especially in patients with STEMI and/or haemodynamic or arrhythmic instability. Percutaneous coronary intervention is now the most common revascularization strategy and preferred in higher risk patients. Coronary artery bypass graft is often delayed and performed in lower risk patients, leading to good 6-month survival. The two approaches therefore appear complementary. We report a case of diffuse arterial calcified elastopathy discovered in a neonatal intensive care unit, during management of a cardiogenic shock in a 3-months old infant. This observation demonstrates the importance of systematic measurement of the arterial tension, family screening and the impact of the ultrasound in the detection of vascular calcifications. Treatment remains essentially symptomatic.

Conclusions: Among NSTE ACS patients undergoing coronary angiography, absence of obstructive CAD is more common in women than men. Although nonobstructive CAD was twice as common among women with NSTEMI, sex differences in characteristics and outcomes were similar to those found with obstructive CAD. Unadjusted in-hospital outcomes of NSTEMI patients with nonobstructive CAD are favorable in both sexes. We treated a 10 year 11 month old girl with severe mitral valve regurgitation, stenosis and dilated cardiomyopathy, presented with New York Heart Association (NYHA) functional classification IV. She acutely developed cardiogenic shock with a dyskinetic anterior-septal left ventricle and entered a shock state during our consultation about heart transplantation. We present our experience using extracorporeal membrane oxygenation (ECMO) in patients with cardiogenic
shock to stabilized organ function prior to LVAD implantation. Between September 2006 and March 2008, five patients in cardiogenic

This article discusses the SHOCK trial and potential mechanisms of benefit from revascularization as well as more recent information. Adjunctive therapies have been disappointing. CONCLUSIONS: A low-volume coronary interventional program can achieve angiographic percutaneous coronary interventional success rates and major adverse cardiac events and the need for target lesion revascularization comparable to those in high-volume major cardiac referral centers. These results were obtained even when high-risk patients with ST-elevation myocardial infarction were not excluded from the analysis.
