During the next decade, a rapid increase in the number of new cancer diagnoses in the population as well as a growing number of cancer survivors can be expected. Cancer is anticipated to exceed cardiovascular disease as the primary cause of mortality in the United States population. Despite efforts in tobacco control, the aging of the population and obesity epidemic will contribute toward the increasing incidence of cancer. Although oncology specialists will continue to play a critical role in the diagnosis and initial treatment of patients with cancer, primary care providers will need to play an expanding role in the ...

Breast cancer remains the most common cancer diagnosed in women and the second leading cause of cancer-related deaths in this group. Significant advances in the treatment of breast cancer and in the ability to screen for the disease mean that it is also one of the most curable forms of cancer. Long-term updates of the trials reviewed in the previous edition of this article have demonstrated that breast-conserving therapy remains a viable option for most patients, and that local control is related to overall survival. New chemotherapeutic options and endocrine therapies are available to select subsets of patients, and the u...

In all, 164 patients who were assessed as clinical T3 prostate cancer by digital rectal examination (DRE), underwent RP and bilateral pelvic lymphadenectomy at Erasmus MC between 1977 and 2004 without neoadjuvant treatment. Preoperative staging computed tomography showed no signs of metastasis. Kaplan[ndash]Meier curves...

Abstract P63 is a member of the p53 family. This protein is crucial for the maintenance of a stem cell population in the human epithelium
and necessary for the normal development of all epithelial tissues including mammary glands. In normal breast tissue, the
p63 seems to be a specific myoepithelial cell marker. P63 expression has been described in highly aggressive ER negative basal-like
breast tumors. The value of p63 expression in ER positive disease is less clear. The expression levels of p63 mRNA by Affymetrix
microarray analysis in a combined cohort of 2,158 ER positive breast cancers and its prognosti...

While novel endocrine treatment options have been implemented in the advanced - as well as adjuvant setting, recent results suggest a place for "old-fashioned" additive treatment with estrogens in advanced breast cancer. This paper reviews the biological rationale for endocrine therapy in general and additive treatment with estrogens in particular. The finding that patients becoming resistant to treatment with aromatase inhibitors may subsequently respond to estrogen therapy adds important information to our understanding of therapy resistance in general. Moreover, the return of a therapeutic option abandoned more than...

Breast cancer is a highly heterogeneous disease with distinct histologic subtypes. Targeted therapies such as endocrine therapy and growth factor receptor inhibitors have had a significant impact on the treatment of metastatic breast cancer patients. Unfortunately, resistance to these agents eventually occurs, and currently represents a significant clinical problem in the management of breast cancers. Inhibitors of histone deacetylases (HDACi) exhibit anticancer activity in a variety of tumor cell models and have been shown to target mechanisms of resistance to a number of targeted agents. It is unclear, however, if there ...

Abstract Mutations in the alpha catalytic subunit of phosphoinositol-3-kinase (PIK3CA) occur in ~30% of ER positive breast cancers.
We therefore sought to determine the impact of PIK3CA mutation on response to neoadjuvant endocrine therapy. Exons 9 (helical

Interactions with clinical, pathological, and biomarker response parameters were examined. A weak negative interaction between
PIK3CA mutation status and clinical response to neoadjuvant endocrine treatment was detected (N =...

Aims: The management of many diseases, including breast cancer, is often influenced by the pre-operative stage or dimension of the tumour. Breast cancer patients treated with primary endocrine therapy are reviewed regularly to assess tumour response, but often by different clinicians. Accurate assessment of lesion size is therefore essential. The aim of this study was to assess the accuracy of clinical sizing of lesions and investigate the impact of two measuring adjuncts on size estimation. (Source: European Journal of Surgical Oncology)

QT interval represents the period between the initiation of depolarization and the end of repolarization of the ventricular myocardium. Excessive prolongation of this interval may drive to a potentially fatal ventricular tachyarrhythmia known as "torsades de pointes". Agents used to manage many endocrine disorders have been linked with QTc alterations. Among them, oral antidiabetic agents, lipid lowering and anti-obesity drugs, somatostatin analogues, thyroid and anti-thyroid agents, and adrenal steroids should be considered. Nevertheless, it is very well known that some endocrine diseases are associated with constrain...

New research has found that hormone therapy used to treat men with advanced prostate cancer is associated with an increased chance of developing various heart problems. This study on mostly previously frozen tissue implied that 1D5 fails to identify some women who may benefit from endocrine therapy. The current prospective study compared SP1 and 1D5 immunostaining on routinely processed consecutive cases of breast carcinoma. ER was classified using the same positive threshold used in the prior study (<1% negative; > or = 1% positive). Of 508 carcinomas, 2 were SP1+/1D5-, and none were SP1-/1D5+. Although SP1 is our preferred antibody, with more intense nuclear staining, both MAbs give similar results in tissue from routine clinical samples with discrepant results in fewer than 0.5% ...

Abstract The majority of breast cancers are estrogen responsive, but upon progression of disease other growth promoting pathways are
activated, e.g., the ErbB receptor system. The present study focuses on resistance to the pure estrogen antagonist fulvestrant
and strategies to treat resistant cells or even circumvent development of resistance. Limited effects were observed when targeting
EGFR and ErbB2 with the monoclonal antibodies cetuximab, trastuzumab, and pertuzumab, whereas the pan-ErbB inhibitor CI-1033
selectively inhibited growth of fulvestrant resistant cell lines. CI-1033 inhibited Erk but not Akt...

Conclusion:
Preoperative therapy for DCIS is associated with significant pathologic alterations. These changes may be clinically significant. Further work is needed to identify which women may be the best candidates for such treatment for DCIS, and whether best responders may safely avoid surgical intervention.Trial Registration: ClinicalTrials.gov NCT00290745 (Source: BMC Cancer)

CONCLUSIONS: TOR120 was rated excellent in acceptability, and high efficacy was observed when it was used up to third-line treatment for AI-failure cases, although this study may show some selection bias because of the retrospective study. In addition, it was also considered effective for TAM-failure cases.
