Review of an article: One-year effect of a supervised exercise programme on functional capacity and quality of life in peripheral arterial disease by Marie Guidon, MSc, PhD, Hannah McGee, PhD (Disabil Rehabil 2013;35:397–404)
Peripheral artery disease (PAD) is a prevalent, progressive, atherosclerotic disorder characterized by pain with physical activity, potential limb loss, and a high rate of morbidity and mortality. Claudication is the most common symptom of PAD and is defined as the reproducible limb fatigue, discomfort, or pain that occurs during physical activity, most often walking, and subsides with rest. Individuals with PAD and claudication experience significant limitations in their physical functioning and decrements in quality of life. (Source: Journal of Vascular Nursing)

Measurement validity and statistical significance for nutritional factors in peripheral artery disease
Oka and Alley published a study recently in the Journal of Vascular Nursing that concluded, “Findings from our study suggest that the nutritional intake of PAD patients differs based on gender and BMI,” and “Additionally, EAR was lower for specific nutrients than recommended.” However, those conclusions were not warranted by the data in the study because the study had (1) low measurement validity, (2) no statistically significant findings because of Type II error, and (3) no data or statistical findings related to nutritional intake of the different genders. Control of Angiogenesis Dictated by Picomolar Superoxide Levels.
Abstract Control of vascular insufficiencies due to various cardiovascular pathologies is important for developing specific and effective treatments. Fluctuations in oxidative stress significantly alter the progression of angiogenesis under physiological and pathological conditions. However, the precise amount of reactive oxygen species (ROS) required to influence subsequent signaling pathways for ischemic angiogenesis remain undefined. Here, we have determined the effect of ROS mediated molecular mechanisms on angiogenesis in a murine model of peripheral artery disease using Gclm mutant mice (a model of compromise...

Cilostazol prevents foot ulcers in diabetic patients with peripheral vascular disease
Abstract Diabetic patients are at the high risk of foot ulcerations that may lead to limb amputations with important socio economic impact. Peripheral vascular disease may be frequently associated in diabetes mellitus type II with its main symptom, intermittent claudication. Many studies reported the known efficacy of cilostazol in treating vascular claudication. Metalloproteinase‐9 (MMP‐9) seems to be a biochemical marker implicated in chronic wounds and in particular in diabetic foot ulcers. Cilostazol seems to have also a lowering effect on levels of MMP‐9 and this may suggest a beneficial effect in order to preve...

Abstract 47: Presentation and Outcomes of Women Veterans with Suspected Ischemia and Nonobstructive Coronary Artery Disease: Insights from the VA Clinical Assessment Reporting and Tracking Program. [Poster Session I]
Conclusion: Between October 2007 and September 2011, 23% of women veterans undergoing catheterization for suspected ischemia had non-obstructive CAD. In contrast to non-VA studies, adverse clinical outcomes and re-hospitalization rates for chest pain were less frequent. These findings may represent systematic differences in treatment and follow-up care of women veterans between VA and non-VA healthcare systems. Abstract 63: The Smoking Paradox in Patients Hospitalized with Coronary Artery Disease: Findings from Get With The Guidelines - CAD [Poster Session I]
Conclusion: Smoking continues to be a major risk factor for presenting with CAD at a much younger age and with fewer risk factors. It is likely that the continued modest association with lower in-hospital mortality in smokers in this analysis after adjustment reflects residual or unmeasured confounding. This apparent smoker’s paradox in CAD should not be interpreted as a benefit of cigarette smoking. (Source: Circulation: Cardiovascular Quality and Outcomes)

Abstract 290: Comparison of Lipid Control in Patients with Coronary versus Peripheral Arterial Disease [Poster Session III]
Conclusion: Even though both PAD and CAD groups had mean LDL < 100mg/dL, we found that lipid control assessed by absolute mean lipid levels were significantly better in patients with CAD and combined PAD & CAD group. Thus, our study indicates that physicians tend to be more aggressive with lipid control in patients with a diagnosis of CAD when compared to patients with PAD alone. Onychomycosis in close quarter living review of the literature
Summary Onychomycosis is defined as a fungal infection of the nail bed and/or nail plate. The prevalence of onychomycosis has increased dramatically as a worldwide condition in the twentieth century due to occlusive footwear, global wars and natural migration. Risk factors generally leading to onychomycosis development include bodily spread of dermatophyte and non‐dermatophyte tinea pedis, peripheral vascular disease, damaged nails via sports and trauma, older age, genetics, immunodeficiency and diabetes. Many publications discuss prevalence, symptoms and treatment of the disease in individual cases, hospitals or specifi...

Salvage of a free radial forearm flap by creation of an arteriovenous fistula at the distal arterial pedicle
Abstract Interdisciplinary approaches with infrainguinal bypass grafts and free flaps between the plastic and vascular surgery may well offer an opportunity for diabetic patients with peripheral vascular disease to salvage their critically affected limbs. A free flap transfer combined with an autologous vein graft can cover large tissue defects and simultaneously improve distal perfusion even in patients with arterial occlusive disease. We are presenting a case of bypass‐free radial forearm flap used to cover a foot defect in an old diabetic patient with peripheral arterial disease. The flap perfusion deteriorated signif...

Peripheral artery disease is associated with severe impairment of vascular function
Patients with peripheral artery disease (PAD) have higher cardiovascular event rates than patients with established coronary artery disease (CAD) and abnormal endothelial function predicts cardiovascular risk in PAD and CAD. We investigated the hypothesis that PAD is associated with a greater degree of impairment in vascular function than CAD. We used several non-invasive tests to evaluate endothelial function in 1320 men and women with combined PAD and CAD (n = 198), PAD alone (n = 179), CAD alone (n = 466), or controls aged > 45 years without CAD or PAD (n = 477). Methods of symptom evaluation and their impact on peripheral artery disease (PAD) symptom prevalence: A review
Peripheral artery disease (PAD) is a common progressive atherosclerotic occlusive disease that causes insufficient blood flow to the lower extremities. The symptom that health care professionals most often associate with PAD is claudication. However, patient reporting of claudication is highly variable. A structured literature review was conducted to evaluate how PAD symptoms are identified, defined, and categorized. This review focuses on the development and performance characteristics of PAD symptom questionnaires and the identification of a spectrum of leg symptoms beyond classic claudication. Additionally, potential co...

Endothelial cell-by-cell profiling reveals the temporal dynamics of VEGFR1 and VEGFR2 membrane localization after murine hindlimb ischemia
VEGF receptor (VEGFR) cell surface localization plays a critical role in transducing VEGF signaling toward angiogenic outcomes, and quantitative characterization of these parameters is critical to advancing computational models for predictive medicine. However, studies to this point have largely examined intact muscle; thus, essential data on the cellular localization of the receptors within the tissue are currently unknown. Therefore, our aims were to quantitatively analyze VEGFR localization on endothelial cells (ECs) from mouse hindlimb skeletal muscles after the induction of hindlimb ischemia, an established model for ...
