The purpose of the study was to investigate the potential of direct machine parameter optimization (DMPO) to achieve parotid sparing without compromising target coverage in IMRT of oropharyngeal cancer as compared to fluence modulation with subsequent leaf sequencing (IM) and forward planned 2-step arc therapy (IMAT). IMRT plans were generated for 10 oropharyngeal cancer patients using DMPO and IM. The resulting dose volume histograms (DVH) were evaluated with regard to compliance with the dose volume objectives (DVO) and plan quality. Conclusion: This tool makes it possible to perform an easy-to-use and fast basic daily quality assurance check featuring an output as well as an energy evaluation. Ideally this tool should offer also the combined dosimetry check of jaw width, couch speed, leaf latency, output, leaf/gantry synchrony and lasers. This will be investigated in the future.

However, neck levels without evidence of disease on post-CRT CT imaging are unlikely to harbor cancer, which lends further support to the concept of basing neck dissection on post-CRT staging and performance of limited neck dissections for patients with limited residual disease. Conclusions The HPV-positive tumors are not more curable based on increased epithelial sensitivity to cisplatin or radiation therapy. Instead, radiation and cisplatin induce an immune response to this antigenic cancer. Conclusions Reirradiation represents the only chance for cure in patients with unresectable disease. After surgery, reirradiation is performed in patients at high risk of locoregional recurrence and may increase acute toxic effects. However, free-flap reconstruction may reduce toxic effects. Functional outcomes have declined since 2000 likely because of the addition of concurrent platinum-based chemotherapy. (Helmholtz Association of German Research Centres) Cancer researchers from the Max Delbrueck Center, Berlin, Germany, have gained new insights into how cells react to DNA damage. They have shown that the protein PARP-1, which detects DNA damage within seconds, activates the transcription factor NF-kappaB. NF-kappaB triggers a survival program, which blocks programmed cell death. Conclusion: Although infratentorial oligodendrogliomas did not show characteristic imaging findings, there was a tendency toward multifocal heterogeneous enhancement and absent or mild mass effect of infiltrative lesions. Infratentorial oligodendrogliomas may be more malignant than supratentorial oligodendrogliomas.
