Current Management Strategies for Verrucous Hyperkeratosis and Verrucous Carcinoma
Proliferative verrucous leukoplakia, a lesion of unknown origin with no strictly defined diagnostic criteria, is worthy of great clinical concern and scrutiny. Treatment options vary, with no consensus regarding the most efficient and effective strategy. Further complicating the unusual biology and behavior of this lesion is its usual multifocal presentation and progression, with lifelong vigilance required after treatment. Verrucous carcinoma is a progressive lesion with high recurrence and 5-year survival rates. Surgery remains the preferred treatment. Further investigation into the combination of surgery and antiviral a...

Immunohistochemical study of p53, Ki-67, MMP-2 and MMP-9 expression at invasive front of squamous cell and verrucous carcinoma in oral cavity.
Abstract
Squamous cell carcinoma (SCC) is the most common malignancy in the oral cavity. Verrucous carcinoma (VC) is a lower grade subtype of SCC with pathological similarities but different management. Tumor invasive front (TIF) is a dynamic location in the differentiation of malignancies. p53 is a tumor suppressor and Ki-67 a proliferation marker. Matrix metalloproteinases 2 and 9 are involved in the tumor invasiveness. We assessed these markers in TIF of oral SCC and VC to find more precise grading scales. We studied the expression of markers in the invasive front of 42 cases of SCC (21 high and 21 low grade), 2...

Carcinoma Cuniculatum: A Potentially Underdiagnosed Entity in the Absence of Clinical Correlation
We present 6 additional cases of CC from the attached gingiva and vestibular/buccal mucosa. The average age at presentation was 66.5 yrs. All of our cases except 1 were treated conservatively: 1 required an en-bloc resection to achieve clear margins. In multiple cases of this study, treatment delays resulted from under-interpretation of small or superficial biopsies. Patients subsequently went on to obtain definitive diagnoses only after clinical correlation revealed an appearance inconsistent with a benign process. Therefore, since a pathognomonic microscopic appearance is often not seen in these cases, the caveat is give...

A distinct entity: The lesion looked like a wart but was, in fact, something far more unusual
A 74-year-old woman presented with a 9-month history of a white slow-growing warty lesion on her vulva (). She reported mild pruritus, but she had not experienced any significant pain or lower urinary tract symptoms. Several antiwart creams were prescribed but none produced a response. A deep surgical biopsy led to a final diagnosis of carcinoma cuniculatum (CC) (). (Source: American Journal of Obstetrics and Gynecology)

Oral verruciform xanthoma: a case report and literature review
Oral verruciform xanthoma represents a rare entity, which affects mainly oral mucosa. A 73-years-old woman presented an asymptomatic soft tissue mass on the tongue. Physical examination of oral mucosa revealed a well-circumscribed, sessile nodule with a fibrous consistency and whitish verrucous surface fixed to the lateral edge of the tongue. These findings were suggestive of verruca vulgaris. Histopathological examination revealed a partial parakeratosis with uniform elongated epithelial ridges, parakeratin plugs and exocytosis. The connective tissue was composed by uniform papillae filled by large vacuolated foam. The Pe...

Verrucous carcinoma in epidermolysis bullosa simplex is possibly associated with a novel mutation in the keratin 5 gene
We describe the molecular background and the unusual course of the disease in a patient with EBS Dowling–Meara, severe keratoderma and a massive verrucous carcinoma. Skin and tumour samples from the patient were analysed using light microscopy, immunohistochemistry and immunofluorescence mapping. Mutation analysis of the KRT5 and KRT14 genes identified the novel KRT5 mutation p.E477D. Invasive tumour areas were characterized by downregulation of keratins 5 and 14, reduced and irregular desmocollin‐2 expression and increased expression of keratins 6, 16 and 17. Levels of Ki‐67 were increased and levels of E‐cadher...

Carcinoma cuniculatum: Not a Verrucous Carcinoma
Carcinoma cuniculatum and verrucous carcinoma are both very well differentiated forms of squamous carcinoma and thus difficult to separate histopathologically from pseudocarcinomatous hyperplasia. The diagnosis of each often requires clinical‐pathologic correlation. In reviewing the literature it is evident that most authors consider carcinoma cuniculatum and verrucous carcinoma to be synonymous even though Aird's original two cases had no verrucous features. The purpose of this report is to record a case of carcinoma cuniculatum which the authors believe to be similar to those reported by Aird et al., to emphasize the d...

Verrucous Cell Carcinoma Arising From an Underlying Giant Keratoacanthoma
Keratoacanthoma is a well-known, fast-growing, benign squamous epithelial neoplasm that usually presents as a solitary, dome-shaped, pink or flesh-colored nodule developing a central keratin-filled crater. Few authors consider it to be a benign cutaneous tumor whereas others suggest that it should be regarded as a low-grade variant of cutaneous squamous cell carcinoma (SCC). A 21-year-old female presented with a firm, nontender, well-defined plaque with irregular margins measuring about 8 x 4 cm over the dorsal aspect of the right foot along with an exuberant growth from the plaque measuring 4 x 5cm. There was no lymphaden...

A case of verrucous carcinoma of the esophagus associated with difficult histopathological diagnosis of endoscopic biopsy specimens
Abstract Verrucous carcinoma of the esophagus (VCE) is a rare, slow-growing, well-differentiated squamous cell carcinoma. Histologically,
superficial layers of this cancer tend to have less atypical epithelium with non-specific inflammatory change, which therefore
makes diagnosis of malignancy extremely difficult unless a surgical specimen is obtained. We recently encountered a case in
which a malignant tumor was suspected on initial endoscopic evaluation, but two biopsies revealed no carcinoma cells. Analysis
of a third biopsy finally suggested the possibility of squamous cell carcinoma. A subsequent surgic...

Diagnostic problems in precancerous lesions and invasive carcinomas of the penis
Penile precancerous and invasive lesions exhibit a variegated morphology. Although the diagnosis and classification of penile tumors is straightforward in most cases, a few entities are problematic, especially to pathologists from countries in which penile cancer is rarely encountered. The differential diagnosis of squamous hyperplasias from differentiated penile intraepithelial neoplasia or from extremely low-grade invasive neoplasms (eg, pseudohyperplastic and verrucous carcinomas) may be particularly difficult. Similarly, given the morphologic features shared by all verruciform tumors (ie, verrucous, warty, papillary, a...

New pathologic entities in penile carcinomas: an update of the 2004 World Health Organization Classification
Most primary malignant tumors of the penis are squamous cell carcinomas (SCC) of the usual type. In recent years several variants, each with distinctive clinicopathologic features, have been described. Pseudohyperplastic carcinoma and carcinoma cuniculatum are both low-grade, extremely well-differentiated SCC variants characterized by an indolent clinical course and good prognosis. The former, which may be confused with pseudoepitheliomatous hyperplasia, preferentially affects the inner foreskin mucosa of elderly men and the latter is a verruciform tumor with an endophytic, burrow-like pattern of growth. Pseudoglandular ca...

Carcinoma cuniculatum mimicking leukoplakia of the mandibular gingiva
Abstract: Carcinoma cuniculatum (CC) is a rare variant of low-grade squamous cell carcinoma (SCC), and occurrence in the oral cavity is quite unusual. CC has a papillomatous keratinized surface like verrucous carcinoma, but CC has a propensity for aggressive local invasion, especially into the bone. Therefore, CC can be quite difficult to remove surgically. A 68-year-old man presented with a case of CC mimicking verrucous leukoplakia in the mandibular gingiva. Repeated biopsies showed no malignancy. Local resection was performed, and histological examination revealed the presence of well-differentiated SCC. Additional hemi...

Therefore, complete excision with adequate margin is warranted for leukoplakia of the lateral tongue, preferably under general anesthesia, which could be performed with minimal morbidity and function preservation. Lichen Sclerosus and Squamous Cell Carcinoma.
Abstract
Lichen sclerosus is a chronic inflammatory disease that can progress to malignancy. The literature indicates an association with anogenital squamous cell carcinoma and verrucous carcinoma. Two pathogenic pathways, differentiated vulvar and penile intraepithelial neoplasias, which have recently been described in relation to squamous cell carcinoma, are both highly associated with genital lichen sclerosus independently of human papilloma virus (HPV) infection. Furthermore, tumor-promoting molecular changes unrelated to HPV infection have been demonstrated and may explain the malignant potential of lichen scl...

Subset of patients with verrucous carcinoma of the oral cavity who benefit from treatment with methotrexate
Abstract: Oral verrucous carcinoma (OVC) is a low-grade variant of squamous cell carcinoma (SCC) with a distinctive morphology and specific pattern of behaviour that is often diagnosed in elderly patients. Resection is the treatment of choice, with radiotherapy as a reasonable alternative. In this retrospective case review we present our experience and outcomes in a group of 12 patients with various stages of OVC who had not been treated conventionally because of the extent of their lesions and their poor general condition. All patients were given chemotherapy with methotrexate, which was given by various routes and in dif...

[Neurotrophic ulceration following thermocoagulation of the Gasserian ganglion.]
We report a severe case of NTU and describe the main features of this poorly documented disease. OBSERVATION: A 67-year-old female patient consulted in the dermatology department for a progressive ulceration of the nose ala and the right upper lip, having developed over the two previous years. She had undergone two thermocoagulations of the right Gasserian ganglion for facial neuralgia 3 years before. The diagnosis of NTU, initially ruled out because of biopsies suggesting verrucous carcinoma, was finally retained because of the clinical presentation and anamnesis. Treatment consisted in surgical excision and primary recon...
