The Road Ahead: What if Gastroenterologists Were Accountable for Preventing Colorectal Cancer?
Gastroenterologists traditionally have focused on providing high-quality colonoscopy in response to primary care physicians' referral of patients needing colorectal cancer (CRC) screening. As we travel down the Road towards “Accountable” Care and clinical integration, we (gastroenterologists) are increasingly becoming members of a team that is responsible for reducing CRC incidence and mortality for a population of patients. As Lee and his colleagues have forcefully demonstrated in this month's article, an organized approach to CRC screening using both colonoscopy and stool-based testing can result in extraordinarily h...

Teleneurology applications: Report of the Telemedicine Work Group of the American Academy of Neurology
Conclusions: Teleneurology is an effective tool for the rapid evaluation of patients in remote locations requiring neurologic care. These underserved locations include geographically isolated rural areas as well as urban cores with insufficient available neurology specialists. With this technology, neurologists will be better able to meet the burgeoning demand for access to neurologic care in an era of declining availability. An increase in physician awareness and support at the federal and state level is necessary to facilitate expansion of telemedicine into further areas of neurology. (Source: Neurology)

The Business of Health Care Reform
Since the passage of the Patient Protection and Affordable Care Act of 2010, it is unusual to go a day without hearing or seeing information on health care reform. Although there is increasing discussion and legislation on health care delivery, the focus for providers appears to be on quality and cost of care. Regardless of any future legislative changes or legal challenges, it is safe to say that “the cat is out of the bag,” that there will be a greater emphasis on defining and measuring the value of health care. As a result of this, there is an ever increasing momentum to change from a pay for procedure model of care...

Use, misuse, and underuse of work relative value units in a vascular surgery practice
Health care reform is forcing “alignment” between hospitals and physicians. The acceleration of employment of physicians by hospitals is bringing into focus contractual terms where compensation is tied to clinical productivity. Physician productivity is being almost entirely defined by work relative value units (WRVUs). However, vascular surgeons may bring value to a health system in ways that are unique and separate from clinical revenue as measured by WRVUs. Incentives for physicians should also be tied to behaviors that are desired, such as quality of care, efficiency, patient outcomes, patient satisfaction scores, ...

Physician-hospital relationships: from historical failures to successful "new kids on the block".
Abstract Trends in healthcare reveal that increasing numbers of physicians prefer to work directly with hospitals-whether through employment models, new or revived partnership structures, or other such "deals". Meanwhile, hospital executives are vigorously seeking ways to create win-win arrangements that satisfy both parties-models that will ensure medical coverage for the hospital, along with revenue gains and cost savings when possible. Add to this a competitive environment, physician shortages, and high regulatory activity and healthcare reform, and the path to clinical, operational, and financial viability in t...

The impact of scheduling policies on surgical clinical access.
We present a simulation model that features a single surgeon who splits his available time between a clinical office and surgery. Using a first-come, first-served queuing discipline, the model allows us to investigate how clinical access responds to surgeons' time allocation policies, patients' office visit frequency, access priority rules, frequency of emergencies, surgery length, and other factors. We demonstrate through simulations that linear functions oversimplify how patients' wait times increase as demand approaches capacity. There is a critical access point at which loss of new patients occurs. Around the critical ...
