Discontinuation of Radiation Treatment among Medicaid-Enrolled Women with Local and Regional Stage Breast Canceremail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Abstract: For women with nonmetastatic breast cancer, radiation therapy is recommended as a necessary component of the breast conserving surgery (BCS) treatment option. The degree to which Medicaid-enrolled women complete recommended radiation therapy protocols is not known. We evaluate radiation treatment completion rates for Medicaid enrollees aged 18[ndash]64 diagnosed with breast cancer. We determine clinical and socio-demographic factors associated with not starting treatment, and with interruptions or not completing radiation treatment. Using data from the Washington State Cancer Registry linked to Medicaid enrollmen...

Accurate assessment of primary tumor size is an essential part of management in breast cancer patients. Tumor size is part of the TNM system used to stage breast cancer and provide prognostic information. Adaptation of the principles of breast conservation surgery requires evaluation of tumor size to judge appropriateness and extent of surgery. Precise measurement of tumor size is also important to monitor response to neoadjuvant chemotherapy for locally advanced tumors. (Source: Cancercompass News: Breast Cancer)

Breast cancer remains the most common cancer diagnosed in women and the second leading cause of cancer-related deaths in this group. Significant advances in the treatment of breast cancer and in the ability to screen for the disease mean that it is also one of the most curable forms of cancer. Long-term updates of the trials reviewed in the previous edition of this article have demonstrated that breast-conserving therapy remains a viable option for most patients, and that local control is related to overall survival. New chemotherapeutic options and endocrine therapies are available to select subsets of patients, and the u...

As breast cancer screening rates increase, smaller and more numerous lesions are being identified earlier, leading to more breast-conserving surgical procedures. Achieving a clean surgical margin represents a technical challenge with important clinical implications. Optical coherence tomography (OCT) is introduced as an intraoperative high-resolution imaging technique that assesses surgical breast tumor margins by providing real-time microscopic images up to 2 mm beneath the tissue surface. In a study of 37 patients split between training and study groups, OCT images covering 1 cm2 regions were acquired from surgical margi...

Breast conserving therapy (BCT) including postoperative irradiation of the remaining breast tissue is generally accepted as the best treatment for the majority of patients with early-stage breast cancer. The question is whether there is a necessity for irradiating all patients. Between 2001 and 2005, 749 women aged 55-75 years with infiltrating breast carcinoma were randomly assigned to breast conservative surgery, with or without radiotherapy (RT), to evaluate the incidence of in-breast recurrence (IBR). After 5 years of median follow-up, the cumulative incidence of IBR was 2.5% in the surgery-only arm and 0.7% in the...

Conclusions: The number of days spent in hospital after breast cancer surgery has continued to decline for several decades. The change from mastectomy to BCS accounts for only 9% of the overall decrease in LOS. Other explanations include the adoption of new techniques and practices, such as sentinel lymph node biopsy and early discharge. This study has identified wide variation in practice with substantial cost implications for the NHS. Further work is required to explain this variation. (Source: BMC Health Services Research)

This article describes four approach routes for scarless surgery (periareolar, axillary, submammary, lateral chest) in different contexts of the oncological breast disease. The application of these incisions in the conservative treatment of breast cancer requires making a wide subcutaneous dissection over the tumour location, and a local reconstruction with adjacent breast tissue, so as not to deform the breast contour, as well as moving the nipple with its areola, thus avoiding the surgical modification of the other breast to maintain symmetry between both. We describe the use of hidden incisions to perform mastectomies t...

CONCLUSION: Pregnancy after breast cancer does not confer a poor prognosis. A higher rate of pregnancy than expected was found after treatment, possibly due to newer treatments including fertility preservation and also possibly due to the active counselling programme in this unit. Patients should have active counselling about fertility when planning treatment and fertility conservation can then be incorporated into a treatment plan. The tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) is a TNF superfamily member that is being considered as a new strategy in anticancer therapy because of its ability to induce apoptosis, alone or in combination with other stimuli, in many cancer cells. AMP-activated protein kinase (AMPK) is an evolutionarily conserved key regulator of cellular energy homeostasis that protects the cell from energy depletion and stress by activating several biochemical pathways that lead to the conservation, as well as generation, of ATP. Here we report that a number of AMPK activators, including the small molecule acti...

Ipsilateral breast tumour recurrence - is mastectomy the only option?email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Background: Patients who develop ipsilateral breast tumour recurrence (IBTR) after breast conserving surgery have traditionally been offered salvage mastectomy. In Guildford breast conserving surgery is offered as an option for these patients if feasible. There is little evidence in the literature on the outcome of these patients. Oncoplastic breast surgery for cancer: the first 20 yearsemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Introduction: For large tumours and those in less favourable locations, standard breast conserving therapy (BCT) is being replaced by more imaginative oncoplastic breast surgery (OBS) techniques that draw on plastic surgery principles to both anticipate and avoid poor aesthetic outcomes and further reduce the rate of mastectomy. Does ‘intra-operative assessment’ of sentinel lymph node biopsy increase patient's anxiety?email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Introduction: Intra-operative assessment (IOA) of sentinel lymph node biopsy (SLNB) with touch imprint cytology (TIC) is a recognized technique to stage the axilla. Axillary clearance is performed during the primary procedure (breast conservation surgery/mastectomy) if IOA is positive. This means that there is some uncertainty pre-operatively in the patient's mind regarding the extent of surgery that will be undertaken. The aim of this study was to see if IOA lead to increase in patient's anxiety about the operation. (Source: European Journal of Surgical Oncology)

What are the predictive factors of positive resection margin in infiltrating lobular carcinoma treated by breast conserving surgery?email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Aims: To determine whether infiltrating lobular carcinoma (ILC) is associated with high positive margin rates for single stage lumpectomy procedures. To define clinical or histological characteristics that might influence the positive margin rate, thereby affecting treatment decisions. To identify factors predictive of margin involvement and the need for re-operation. (Source: European Journal of Surgical Oncology)

Ex vivo terahertz pulsed imaging to differentiate malignant from normal breast tissueemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Introduction: About 60-70% of patients with early breast cancer undergo breast-conserving surgery. Of these, approximately 25-30% are found to have close or involved margins at final histology and require further surgery to ensure clear margins. Current methods of intra-operative margin assessment are unreliable. (Source: European Journal of Surgical Oncology)

Choosing mastectomy to avoid adjuvant radiotherapy. Is this accurate from previous practice or misguided patient perception?email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Introduction: Breast cancer surgery continues to evolve, aiming to conserve breast tissue and minimise axillary dissection. There is a general impression however that mastectomy avoids the need for subsequent radiotherapy particularly in elderly patients. Adjuvant radiotherapy following mastectomy (ARFM) may still be required for close resection margins or nodal disease. We reviewed ARFM for patients in our symptomatic breast unit. Can clinical and radiological features predict the need for Sentinel Lymph Node Biopsy in patients with a pre-operative diagnosis of Ductal Carcinoma in Situ, who are eligible for breast conservation surgery?email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Background: The role for Sentinel Lymph Node Biopsy (SLNB) in patients eligible for breast conservation surgery (BCS) remains controversial. 10-30% of patients with pre-operative diagnosis of DCIS will have an invasive component subsequently detected in the resected specimen. These patients will require a second operation for axillary staging. We sought to identify the characteristics of patients with a pre-operative diagnosis of DCIS eligible for BCS who are most likely to harbour an invasive component, with the aim of identifying those who might benefit from SLNB.
