Radial Artery Cannulation: A Comprehensive Review of Recent Anatomic and Physiologic Investigations.email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Consistent anatomic accessibility, ease of cannulation, and a low rate of complications have made the radial artery the preferred site for arterial cannulation. Radial artery catheterization is a relatively safe procedure with an incidence of permanent ischemic complications of 0.09%. Although its anatomy in the forearm and the hand is variable, adequate collateral flow in the event of radial artery thrombosis is present in most patients. Harvesting of the radial artery as a conduit for coronary artery bypass grafting, advances in plastic and reconstructive surgery of the hand, and its use as an entry site for cardiac ...

To the Editor: We read with interest the article of Prakash and associates entitled “A new diagnostic algorithm for assessment of patients with single ventricle before Fontan operation.” We found it appealing to spare these children invasive and costly procedures in the era of rampant health care costs. In their retrospective study the authors developed an algorithm that identifies high-risk patients for the completion of partial cavopulmonary connection. This algorithm is based on clinical and noninvasive imaging criteria and limits cardiac catheterization only to high-risk patients. (Source: The Journal of Thoracic ...

We would like to thank Soueidi and colleagues for their thoughtful comments on our work. We agree that the decision to proceed with a Fontan operation is a delicate one with major implications for the child's future course. It is standard clinical practice to perform cardiac catheterization for preoperative evaluation. However, this practice is based on data from the 1970s and does not take into consideration advances in noninvasive imaging and changes in the characteristics of subjects presenting for a Fontan operation. Although cardiac catheterization is the reference standard for hemodynamic assessment, it is invasive a...

Conclusions The findings of the present study show that transradial coronary angiography and angioplasty are safe, feasible, and effective with similar results to those of the transfemoral approach. However, procedural duration and radiation exposure are higher using the transradial access. In contrast to the transfemoral route, the rate of major vascular complications was negligible using the transradial approach. Optimization of radiation protection devices for the operator is achieved by minimizing the effective dose (E) on the basis of the recommendations of Publications 60 and 103 of the International Commission on Radiological Protection (ICRP). Radiation exposure dosimetry was performed with thermoluminescence dosimeters using one Alderson phantom in the patient position and a second one in the typical position of the operator. Various types of protective clothing as well as fixed leaded shieldings (table mounted shielding and overhead suspended shields) were considered calculating E. Here, we demonstrate a real-time reconstruction pipeline for adaptive TSENSE with low image latencies and high frame rates on affordable commodity personal computer hardware. Conclusion: Road distance and travel time measurements are the most accurate estimates, but cannot be directly implemented in spatial analytical modeling. Euclidean distance tends to underestimate road distance and travel time; Manhattan distance tends to overestimate both. The optimized Minkowski distance partially overcomes their shortcomings; it provides a single model of travel over the network. The method is flexible, suitable for analytical modeling, and more accurate than the traditional metrics; its use ultimately increases the reliability of spatial analytical models. Opinion statement  Combined coronary artery valvular heart disease is a major cause of morbidity and mortality in the adult patient population. The standard treatment for such disease has been open heart surgery in which coronary artery bypass grafting (CABG) is performed concurrently with valve surgery using a median sternotomy and cardiopulmonary bypass. With the increasing complexity of patients referred to surgery, some patients may prove to be poor surgical candidates for combined valve and CABG surgery. In certain selected patients who fall into this category, valve surgery and percutaneous coronary int...

[Radial access and management of patients with long-term oral anticoagulation for cardiac catheterization.]email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Patients with long-term oral anticoagulation are exposed to high bleeding risk and should be managed with care for invasive procedure. Angioplasty is assumed to increase bleeding and access site complications and it is often recommended to temporarily interrupt anticoagulation therapy. Moreover, due to the lack of recommendations, antithrombotic regimen adopted after stenting represents a substantial challenge for the physician who must balance the risk of ischemic and bleeding events. Nevertheless, approaches as uninterrupted therapeutic oral anticoagulation, transradial approach and a best knowledge of ischemic risk,...

Revascularization for Unprotected Left Main Disease: Evolution of the Evidence Basis to Redefine Treatment Standardsemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Determining the most appropriate revascularization strategy for patients presenting with unprotected left main coronary (UPLM) disease has been a topic of great recent interest. During this current decade, there have been multiple clinical trials and registries addressing this subject. Previously, UPLM disease has almost exclusively resulted in utilizing a surgical revascularization treatment strategy. However, recent improvements in percutaneous coronary intervention (PCI) techniques in parallel with the benefits of drug-eluting stents (DES) to reduce clinical restenosis have enabled further investigation in catheterizati...

Transfemoral percutaneous removal of a knotted Swan-Ganz catheteremail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
We describe the successful nonsurgical removal of a Swan-Ganz catheter, after the formation of a large double knot, during a right heart catheterization performed without fluoroscopic guidance in the intensive care unit. This technique could serve as a last resort in cases that surgical removal seems inevitable due to size-related inapplicability of other nonsurgical methods. © 2009 Wiley-Liss, Inc. Complicated ruptured sinus of Valsalva: cardiac computed tomographic angiography (64 slice) predicts surgical appearance and obviates need for invasive cardiac catheterization [Case report - Aortic and aneurysmal]email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
We present a case report of a ruptured sinus of Valsalva aneurysm (SVA) that presented as aortic insufficiency following bacterial endocarditits in a cardiac transplant patient. Cardiac computed tomographic angiography (CCTA) including volume rendered images predicted the appearance of the fistula entrance and defined spatial relationships facilitating the surgical approach. CCTA ability to define the coronary anatomy obviated the need for invasive coronary angiography. The use of this imaging modality especially with three-dimensional spatial visualization, and multiphase cine angiography can add significant value to the ...

Cardiac catheterization remains the standard means of diagnosis. Our purpose in this study is to emphasize the importance of assessing the electrocardiogram when making the diagnosis, in addition to taking note of transthoracic echocardiographic findings. We also analyzed the sensitivity of each parameter under investigati...

Primary left ventricular rehabilitation is effective in maintaining two-ventricle physiology in the borderline left heartemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Conclusion: In patients with borderline left hearts, primary left ventricular rehabilitation with endocardial fibroelastosis resection and mitral and aortic valvuloplasty results in improved left ventricular systolic and diastolic performance and decreased right ventricular pressures. This approach may provide an alternative to single-ventricle management in this difficult patient group. (Source: The Journal of Thoracic and Cardiovascular Surgery)

Conclusions The majority of patients with clinically indicated coronary angiography during follow-up had detectable coronary calcification at baseline. Although there is a significant relationship between the extent of calcification and mean degree of stenosis in individual coronary vessels, 16% of the coronary arteries with significant stenoses had no calcification at baseline.
