CONCLUSIONS: A single IV infusion of propofol 2.4 mg/kg produces a statistically significant, but not clinically meaningful, reduction in disability from CDH 30 days after infusion and does not reduce pain intensity or analgesic use. This study does not support this regimen of IV propofol for clinical management of CDH.

Conclusions. Introduction of a peripheral subcutaneous field stimulation directly to the painful knee area is a novel and simple procedure that was extremely effective for the relief of pain and may provide a breakthrough in the treatment of chronic intractable knee pain following total knee arthroplasty. The periarticular approach has several advantages, including only small incisions over the lateral and medial knee, proximal thigh and abdomen resulting in minimal strain on the lead array with flexion and extension contributing to overall stability of this system. The Lowdown on STD’s: Chlamydia
Chlamydia is the third most common sexually transmitted infection (STI) in the United States. It is the most common bacteria causing an STI. According to a 2007 Center for Disease Control report, South Carolina ranks 3rd out of the 50 states in chlamydial infections. Those at most risk of infection are single, minority women between ages 15-21 with new or multiple sexual partners. However, Chlamydia affects women and men of all backgrounds.
Like all sexually transmitted infections, Chlamydia enters a woman’s body during intercourse. Chl...

Conclusions:
This study addressed contribution of GABAB receptors expressed on primary afferent nociceptive fibers to the modulation of pain. We observed that neither the development of acute and chronic pain nor the analgesic effects of a systematically-delivered GABAB agonist was significantly changed upon a specific deletion of GABAB receptors from peripheral nociceptive neurons in vivo. This lets us conclude that GABAB receptors in the peripheral nervous system play a less important role than those in the central nervous system in the regulation of pain. (Source: Molecular Pain)

Several newspapers have reported that pregnant women and other vulnerable people are refusing to have the swine flu vaccine. The Times reported that a poll of GPs found that only 46% of people who were offered the vaccine have accepted it, and one doctor estimated that only 5% of pregnant women have had it.
The news reports are based on a “snapshot” survey of 107 GPs. The survey asked how likely they were to hit the government’s target of vaccinating at least half of their patients who are under 65 and in high-risk groups this winter. GPs were also asked to estimate how many of the people who were offered the vacci...

Semin intervent Radiol 2009; 26: 345-351DOI: 10.1055/s-0029-1242198ABSTRACTChronic mesenteric ischemia is a rare condition, generally characterized by postprandial abdominal pain. Although chronic mesenteric ischemia accounts for only a small percentage of all mesenteric ischemic events, it can have significant clinical consequences. There are multiple etiologies; however, the most common cause is atherosclerosis. The diagnosis of chronic mesenteric ischemia requires a high clinical index of suspicion. Complex brains have developed specialized mechanisms for the grouping of principal cells into temporal coalitions of local or distant networks: the inhibitory interneuron 'clocking' networks. They consist of GABAergic (where GABA is gamma-aminobutyric acid) interneurons of a rich diversity. In cortical circuits, these neurons control spike timing of the principal cells, sculpt neuronal rhythms, select cell assemblies and implement brain states. On the basis of these considerations, the deficits in cognition, emotion and perception in psychiatric disorders such as anxiety, depression or schizophrenia are considered to m...

Conclusion For the first time, urine analyses may be used to monitor methadone over- or under-use in a clinical setting, regardless of the state of patient hydration or the manipulation of a sample by addition of another substance, such as bleach, soap, or even methadone, which could render an appropriate sample inappropriate or an inappropriate sample appropriate. A similar approach may prove useful for other drug treatments, allowing for more accurate monitoring of commonly abused prescription medications.
