Conclusions: Compared with larynx cancers salvage rates are poorer for hypopharyngeal cancers and the role of primary surgery seems to be significantly higher and more decisive in overall survival (OS). Hypopharyngeal cancers therefore warrant a closer follow-up after an organ preservation protocol, to detect recurrence at the earliest stage and plan for an effective salvage surgery. Objective: To evaluate the survival and morbidity of patients treated by surgery for carcinoma of the larynx and hypopharynx according to site. Patients and methods: All patients who had undergone laryngectomy at the Division of Surgical Oncol...

Recurrence after neoadjuvant chemoradiation and surgery for esophageal cancer: Does the pattern of recurrence differ for patients with complete response and those with partial or no response?

Conclusions: Most esophageal cancer recurrences after neoadjuvant therapy and surgery are distant, and survival time after recurrence is short regardless of pathologic response. Fewer patients achieving complete response had recurrences, and distant recurrences in these patients manifest later than in patients showing partial response and those showing no response. Only pathologic response is significantly associated with disease recurrence, suggesting that tumor biology and chemosensitivity are critical in long-term patient outcome. Residual disease is the first predictive factor for survival in women with ovarian cancer. Neoadjuvant chemotherapy still has not proved superiority in terms of overall survival compared with complete initial resection. Hence, initial surgery remains the cornerstone of management for patients with advanced ovarian cancer. Various parameters have been proposed to evaluate the ability of complete resection. Clinical evaluation of peritoneal carcinomatosis and ascite is not relevant enough while general status (ASA score) is correlated with the risk of postoperative complications. Preoperative dosage of CA-125 higher than...

Accurate assessment of primary tumor size is an essential part of management in breast cancer patients. Tumor size is part of the TNM system used to stage breast cancer and provide prognostic information. Adaptation of the principles of breast conservation surgery requires evaluation of tumor size to judge appropriateness and extent of surgery. Precise measurement of tumor size is also important to monitor response to neoadjuvant chemotherapy for locally advanced tumors. (Source: Cancercompass News: Breast Cancer)

Minimally invasive resection has emerged as a surgical technique for gastric cancer, and there has been continued investigation to determine the appropriate extent of lymphadenectomy in gastric cancer patients. There has also been significant progress in evaluating the role of chemotherapeutic regimens used in the neoadjuvant and adjuvant settings for patients with resectable disease. We also summarize a selection of RCT trials focused on the perioperative care of the gastric cancer patient. (Source: Surgical Oncology Clinics of North America)

During the past eight years, there have been significant advances in the treatment and outcome of patients with metastatic colorectal cancer. This is likely the result of improved imaging, staging, chemotherapy, and surgery. After conducting a standardized MEDLINE literature search, this article reviews the prospective randomized controlled trials in advanced colorectal cancer published between May 2001 and October 2008. There are a total of 96 studies, all chemotherapy-related trials, divided into five categories based on trial focus: (1) types of chemotherapy; (2) administration of adjuvant or neoadjuvant chemotherapy fo...

The treatment of esophageal cancer with curative intent remains highly controversial, with advocates of surgery alone, chemoradiotherapy alone, surgery with adjuvant therapy (including neoadjuvant and postoperative), and trimodality therapy each contributing prospective randomized controlled trials (PRCTs) to the body of scientific publications between 2000 and 2008. Any improvements in survival have been small in absolute percentage terms, and as such PRCTs published over the last decade have met the same primary obstacle encountered by the studies from the two prior decades, namely lack of power to detect small differenc...

Resection of hepatocellular carcinoma (HCC) offers the only hope for cure. However, in patients undergoing resection, recurrences, in particular, intrahepatic recurrence are common. The effectiveness of transarterial chemoembolization (TACE) as a neoadjuvant therapy for unresectable HCC was exploited by numerous liver units and employed preoperatively in the setting of resectable HCC with an aim to prevent recurrence and prolong survival. A systematic literature search of databases (Medline and PubMed) to identify published studies of TACE administered preoperatively as a neoadjuvant treatment for resectable HCC was undert...

Patients with invasive upper tract transitional cell carcinoma (UTTCC) pose a difficult management problem with prognosis dependent on the degree of invasion, the relationship, if any, to previous superficial or invasive bladder cancer, and the presence of metastatic nodes. Similar to invasive bladder cancer, preoperative staging can often be inaccurate and distinguishing superficial invasion from deep invasion may not always be possible. Unlike invasive bladder cancer, strategies to improve survival rates using neoadjuvant systemic chemotherapy before nephroureterectomy have not been subjected to clinical trials. However,...

Conventional development of multivariate gene expression models (GEM) predicting therapeutic response of cancer patients is based on analysis of patients treated with specific regimens, which limits generalization to different or novel drug combinations. We overcome this limitation by developing GEMs based on in vitro drug sensitivities and microarray analyses of the NCI-60 cancer cell line panel. These GEMs were evaluated in blind fashion as predictors of tumor response and/or patient survival in seven independent cohorts of patients with breast (n = 275), bladder (n = 59), and ovarian (n= 143) cancer treated with multiag...

Data from the Austrian Breast and Colorectal Cancer Study Group (ABCSG)-24 study presented at the joint 15th ECCO and 34th ESMO congress in Berlin, show that adding capecitabine (Xeloda) to anthracycline- and taxane-containing regimens prior to surgery completely eradicated the tumor in 24% of women with HER2-positive or HER2-negative early breast cancer. This is an impressive finding since the proportion of women achieving total tumor eradication with standard chemotherapy for HER2-positive or HER2-negative early breast cancer is less than 20% (range = 6%–18%). Thrombotic microangiopathy following docetaxel and trastuzumab chemotherapy: a case report

We report a 47-year-old patient with localised
breast cancer who developed thrombotic microangiopathy (HUS/TTP) acutely following concurrent neoadjuvant therapy with trastuzumab
and docetaxel. We demonstrate from our case that mild confusion occurring on a background of mild anaemia and thrombocytopenia
may sometimes be the only clue to the diagnosis. Clinicians should be aware of the possibility of thrombotic microangiopathy
associated with trastuzumab and docetaxel therapy, as early intervention can improve clinical outcome.

CONCLUSIONS: Detection of >/=1 CTC/7.5 ml before neoadjuvant chemotherapy can accurately predict OS. Our findings may change the clinical management of nonmetastatic breast cancer and indicate that the metastatic efficiency of CTC could be higher than previously reported.

This study is a retrospective evaluation of the efficacy of neoadjuvant chemotherapy (NC) with a vinorelbine (V) and epirubicin (E) intravenous combination regimen and is aimed at identification of predictive markers for the long-term outcome in noninflammatory locally advanced breast cancer (NLABC).
Methods:
One-hundred-and-nineteen patients with NLABC were identified from September 2001 to May 2006. Analysis was performed in March 2008, with a median follow-up of 63.4 months (range, 9-76 months). All patients were diagnosed with invasive breast cancer using 14G core needle biopsy and treated with three cycles of VE befor...

Definitive chemoradiotherapy (dCRT) has been proposed as an alternative therapy for selected patients with oesophageal cancer. The aim of this study was to determine the outcomes of dCRT, surgery alone, and neoadjuvant chemotherapy followed by surgery (CS) in patients with oesophageal cancer.Consecutive patients diagnosed with oesophageal cancer and managed by a multidisciplinary team were staged by computed tomography and endoluminal ultrasonography. Those deemed unsuitable for surgery on the grounds of performance status, bulky local disease or personal choice received dCRT. The primary outcome measure was overall surviv...

CONCLUSION: Compared with surgery alone, neoadjuvant chemoradiotherapy can improve the long-term survival and reduce local-regional cancer recurrence. Concurrent administration of neoadjuvant chemoradiotherapy was superior to sequential chemoradiotherapy.

Conclusion The high expression of bag-1, bcl-2 protein and the low expression of bax protein exist in nonsmall cell lung cancer. The expression level of bag-1 protein is closely related to the differentiation degree of non-small cell lung cancer. A highly positive correlation exists between bag-1 and bcl-2 expression, and a highly negative correlation is observed between bcl-2 and bax expression. The study doesn’t provide the evidence that there is a close correlation between the expression levels of bag-1, bcl-2, bax and the multi-drug resistance in non-small cell lung cancer. Conclusion
Treatment with 4 to 6 weeks of celecoxib had no effect on intermediate biomarkers of prostate carcinogenesis, despite the achievement of measurable tissue levels. We caution against using celecoxib 400 mg twice daily as a preventive agent for prostate cancer in additional studies. (Source: Journal of Clinical Oncology)

Introduction: Neoadjuvant chemotherapy (NC) is used in treating locally advanced operable breast cancer. After surgery, further adjuvant treatment is offered based on the estrogen receptor(ER), progesterone receptor(PR) and HER2 status of the core biopsy taken preoperatively. However a change in ER/PR or HER2 status following NC could have profound effect on adjuvant treatment. The aim of our study was to determine the percentage of patients whose ER/PR, HER2 receptor expression change with NC and if these changes lead to change in their adjuvant treatment. Conclusions Surgical resection still remains the most important therapeutic and prognostic factor for patients with locally recurrent
rectal cancer. Multimodal treatments can be safely performed by an experienced team in referral tertiary centers and can result
in a safer outcome, better local disease control, and even long-term survival in carefully selected patients.
