Calcific Tendinitis: A Pictorial Reviewemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Abstract: Calcific tendinitis is caused by the pathologic deposition of calcium hydroxyapatite crystals in tendons and is a common cause of joint pain. The disease typically affects the shoulder and hip, with characteristic imaging findings; however, any joint can be involved. Occasionally, calcific tendinitis can mimic aggressive disorders, such as infection and neoplasm, especially on magnetic resonance imaging. Radiologists should be familiar with the imaging findings to distinguish calcific tendinitis from more aggressive processes. Image-guided percutaneous needle aspiration and steroid injection of calcific tendiniti...

The single major cause of emotional suffering and stress in our lives comes from the accumulated habitual emotional reactions to life events that we acquire through unconscious learning. We become victims of recurrent negative thoughts and patterns of emotional reactivity that operate automatically in the mind, and that operate outside the sphere of conscious choice. We become prisoners of our habitual thinking and suffer accordingly. Therefore, it stands to reason that if we want to reduce our level of emotional stress and suffering, we must learn new strategies to counteract and neutralize our conditioned habitual reacti...

Abstract  Recurrent low back pain (recurrent LBP) is a common condition, however, it is unclear if uniform definitions are used in studies investigating the prevalence and management of this condition. The aim of this systematic review was to identify how recurrent LBP is defined in the literature. A literature search was performed on MEDLINE, EMBASE, CINAHL, AMED, and PEDro. Studies were considered eligible if they investigated a cohort of subjects with recurrent LBP or if they were measuring the prevalence of recurrent LBP. Two independent reviewers assessed inclusion of studies and extracted definitions of...

This study also suggests we are able to deliver convincing acupuncture placebos/controls and that a ‘placebo’ needle (SN) appears as clinically effective as MES. This implies that an enhanced placebo response is not associated with acupuncture needling. This suggests we should consider that the most important factor influencing treatment outcome for pain may be related to the therapist and not the procedure they employ. This hypothesis will require further investigation across a number of different interventions but may be important for pain management. (Source: European Journal of Integrative Medicine)

An exploratory double-blind randomised placebo controlled trial in rheumatoid arthritis to evaluate whether the effects of homeopathy are attributable to the consultation process, or the homeopathic remedyemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Conclusion: The primary outcomes are negative. The secondary outcomes show that the homeopathic consultation mediates moderate clinically relevant benefit in relatively stable RA and that the addition of individualised remedy to consultation provides further clinical benefit for patient's subjective experience of their RA. These findings appear robust and warrant further exploration in a cost-effectiveness study (Source: European Journal of Integrative Medicine)

Palmoplantar pustular psoriasis (PPP) is an uncommon form of chronic psoriasis. Characterized by sterile, intraepidermal pustules located on the palms and soles, it is highly resistant to treatment. Our patient presented with palmar inflammation and throbbing joint pain in his hands, as well as erythematous, pustular, and micaceous scaling skin on his right foot, legs, elbows, and hands. Approximately 4 percent of his body surface area was involved and he was diagnosed with PPP after skin biopsy. After conventional therapy failed, the patient underwent treatment with adalimumab and the majority of his symptoms resolved...

Evaluation of the breathworks mindfulness-based pain management programme: effects on well-being and multiple measures of mindfulnessemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Two studies of a mindfulness training programme are presented. Study 1 reports on a pilot investigation of the impact on well-being of the Breathworks mindfulness-based pain management programme. Significant positive change was found on self-report measures of depression, outlook, catastrophizing and pain self-efficacy in the Intervention Group, but not the Comparison Group. Particularly large effects were found for pain acceptance. These results support the short-term efficacy of the Breathworks programme and reinforce the importance of acceptance for positive outcome with chronic pain patients. Study 2 investigated alter...

Incorporating Extrinsic Goals Into Decision and Cost-Effectiveness Analysesemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
It has not been widely recognized that medical patients as individuals may have goals that are not easily expressed in terms of quality-adjusted life years (QALYs). The QALY model deals with ongoing goals such as reducing pain or maintaining mobility, but goals such as completing an important project or seeing a child graduate from college occur at unique points in time and do not lend themselves to easy expression in terms of QALYs. Such extrinsic goals have been posited as explanations for preferences inconsistent with the QALY model, such as unwillingness to trade away time or accept gambles. In this article, the author...

The practice of post-operative pain therapy continues to be a problem. We conducted a survey among nurses and physicians about their views of an established post-operative pain management program. A questionnaire was sent to all nurses and physicians of nine surgical wards (general, trauma, cardio-thoracic and oromaxillofacial surgery and gynecology). Questions were developed from qualitative interviews with staff. Patient data were derived from a post-operative pain registry. Seventy-eight physicians and nurses answered; the overall response rate was 23%. Post-operative pain therapy had high personal priority on an 11-poi...

Conclusion: There was a 78% reduction in analgesic requirements in women given epidural morphine after vaginal delivery compared with placebo for both primiparous and multiparous patients. Women who receive epidural labor analgesia for vaginal deliveries and stay in the hospital for 24 h after delivery may benefit from postpartum administration of epidural morphine.
