The Roles of Dopamine Transport Inhibition and Dopamine Release Facilitation in Wake Enhancement and Rebound Hypersomnolence Induced by Dopaminergic Agentsemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Conclusions: All DA releasing agents tested, and some DAT-inhibiting agents, produced RHS at equal wake-promoting doses. Thus amphetamine-like DA release appears sufficient for inducing RHS, but additional properties (pharmacologic and/or pharmacokinetic) evidently underlie RHS of other DAT inhibitors. Enhancing wake while mitigating RHS can be achieved by combining DAT-inhibiting and DA-releasing agents. A recent case report published in the American Journal of Emergency Medicine described a 48-year-old woman with no significant medical history who developed a myocardial infarction and subsequent ventricular tachycardia/fibrillation. This patient also happened to be taking phentermine, and the authors therefore conclude “that this agent, which is a chemical analogue of the potent amphetamine class of noradrenergic stimulants, may have the ability to promote potentially fatal cardiac outcomes.” We believe that the authors have left the reader with an unbalanced and exaggerated impression that phentermine is a dangerous ...

Worldwide, obesity has assumed epidemic proportions. Between 2003 and 2004, 32.2% of adults in the United States were classified as obese (body mass index ≥30 kg/m2). Many obese individuals have difficulty trying to lose weight using the conventional methods of diet and exercise, and, under mounting medical and societal pressures, many are compelled to seek an “easy fix” through pharmaceutical agents. The Internet has become a popular venue for the sale and purchase of such supplements, raising fears that lack of regulation may expose consumers to life-threatening risks. Obesity is an increasingly serious socioeconomic and clinical problem. Between (1/4)-(1/3) of population in the developed countries can be classified as obese. Four major etiological factors for development of obesity are genetic determinants, environmental factors, food intake and exercise. Obesity increases the risk of the development of various pathologic conditions including: insulin-resistant diabetes mellitus, cardiovascular disease, non-alcoholic fatty liver disease, endocrine problems, and certain forms of cancer. Thus, obesity is a negative determinant for longevity. In this review we provide broad overview of...

A letter to the editor "Cusp tear in bicuspid valve possibly caused by phentermine" published in volume 106 (2006) stated "Phentermine is known to cause valvular disease with prolonged use." There is no evidence that phentermine causes valvulopathy. Phentermine is the most widely used anti-obesity drug. In view of the serious cardiovascular risks associated with obesity, it is important to clarify that phentermine is a safe and effective anti-obesity medication. The obesity epidemic currently plaguing the United States has spurred the development of a vast number of drugs to assist in the battle against obesity and its associated complications. The need to loose weight often causes patients to loose sight of or even ignore the serious side effects of some of the most widely used weight-loss medications. Here we present the case of ventricular tachycardia/fibrillation arrest in an otherwise healthy 48-year-old woman who was taking no medications other than phentermine, a common appetite suppressant that functions as a central nervous system stimulant through activation of the n...
