Review article: emulsifiers in the food supply and implications for gastrointestinal disease
ConclusionsThere is limited evidence to directly link emulsifiers and thickeners to human disease, but multiple potential pathogenic mechanisms. Knowledge of actual dietary intake and high ‐quality interventional studies is needed to enable the risks associated with their intake to be understood. Sustained virologic response rates in patients with chronic hepatitis C genotype 6 treated with ledipasvir+sofosbuvir or sofosbuvir+velpatasvir
ConclusionWhether treatment ‐naïve, treatment‐experienced, or cirrhotic patients with HCV GT 6 residing in the US had excellent outcomes when treated with SOF+VEL or LDV+SOF. Since LDV+SOF is more readily available globally, our results may provide clinicians with a treatment option when cost and availability limit the tr eatment choice. Review article: the pharmacological causes of colon ischaemia
ConclusionsPatients presenting with abdominal pain, diarrhoea, or bloody stool need to be evaluated for the possibility of this condition and treated accordingly. Timely diagnosis is necessary to improve patient outcomes. This review aims to increase awareness among clinicians regarding the presentation of pharmacologically induced colonic ischaemia. Combining hepatitis B core ‐related and surface antigens at end of nucleos(t)ide analogue treatment to predict off‐therapy relapse risk
ConclusionsSerum HBcrAg and HBsAg levels were independent predictors of off ‐NA relapse and can be factored into a risk score to guide treatment cessation in patients with CHB. (Source: Alimentary Pharmacology and Therapeutics)

Response to treatment is more important than disease severity at diagnosis for prediction of early relapse in new ‐onset paediatric Crohn’s disease
ConclusionsEarly relapses were associated with a higher risk for disease complications at followup. Relapse prediction based on week 12 disease activity or inflammation is superior to prediction using data from diagnosis. (Source: Alimentary Pharmacology and Therapeutics)

Systematic review and meta ‐analysis: portal vein recanalisation and transjugular intrahepatic portosystemic shunt for portal vein thrombosis
ConclusionsTransjugular intrahepatic portosystemic shunt for portal vein thrombosis recanalisation was highly feasible, effective, and safe. Cavernous transformation was the main determinant of technical failure. Additional catheter ‐directed thrombolysis was associated with higher risk of severe complications. Ascitic cholesterol is superior to serum ‐ascites albumin gradient in the detection of non‐portal hypertensive ascites and the diagnosis of mixed ascites
ConclusionAscitic cholesterol is an excellent measure for detecting NPH ascites and for identifying peritoneal lesions in mixed ascites. Thus, this simple and cost ‐effective measure should be determined in patients with new‐onset ascites (www.chictr.org.cn; ChiCTR ‐DCD‐15006907). (Source: Alimentary Pharmacology and Therapeutics)

Incidence and predictors of new persistent opioid use following inflammatory bowel disease flares treated with oral corticosteroids
ConclusionsNew persistent opioid use is common in IBD patients who experience a flare, especially among those with mental health disorders, COPD, and Crohn's disease or indeterminate colitis. These findings can be helpful in risk ‐stratifying patients when choosing an acute pain therapy and providing counselling before choosing to prescribe opioids to opioid‐naïve patients experiencing an IBD flare. Randomised clinical trial: emricasan versus placebo significantly decreases ALT and caspase activation in subjects with non ‐alcoholic fatty liver disease
ConclusionsEmricasan decreased ALT and biomarkers in subjects with NAFLD and raised aminotransferases after 28  days. These results support the further development of emricasan in patients with NAFLD. Trial registration: ClinicalTrials.gov, Identifier: NCT02077374. (Source: Alimentary Pharmacology and Therapeutics)

Loss of paraspinal muscle mass is a gender ‐specific consequence of cirrhosis that predicts complications and death
ConclusionsThis study links muscle wasting in patients with cirrhosis predominantly to males. However, the presence of a low PSMI mass is a gender ‐independent predictor of developing cirrhosis‐associated complications and death. Scores combining the MELD with muscle parameters should be re‐validated by utilizing the PSMI. (Source: Alimentary Pharmacology and Therapeutics)

Systematic review and meta ‐analysis: defecography should be a first‐line diagnostic modality in patients with refractory constipation
ConclusionsPathological structural abnormalities, as well as functional abnormalities, are common in patients with chronic constipation. Since structural abnormalities cannot be evaluated using nonimaging test modalities (balloon expulsion and anorectal manometry), defecography should be considered the first ‐line diagnostic test if resources allow. Loss of tolerance to glycoprotein 2 isoforms 1 and 4 is associated with Crohn ’s disease of the pouch
ConclusionsThe novel anti ‐GP21 and GP24 antibodies are associated with Crohn's disease of the pouch in ulcerative colitis patients after IPAA. Serological anti ‐GP2 antibodies could aid in diagnosis of Crohn's disease of the pouch. (Source: Alimentary Pharmacology and Therapeutics)

Systematic review: efficacy and safety of switching patients between reference and biosimilar infliximab
ConclusionsWhile available data have not identified significant risks associated with a single switch between reference and biosimilar infliximab, the studies available currently report on only single switches and were mostly observational studies lacking control arms. Additional data are needed to explore potential switching risks in various populations and scenarios. (Source: Alimentary Pharmacology and Therapeutics)
