Structures of truncated versions of the influenza A virus M2 proton channel have been determined recently by x-ray crystallography in the open conformation of the channel, and by NMR in the closed state. The structures differ in the position of the bound inhibitors. We appreciated the excellent article by Stevens et al on the comparison of drug dosing recommendations based on measured glomerular filtration rate– and kidney function–estimating equations. Although the study reflected a diverse population, it failed to provide detailed information about subgroups, including the elderly. Several studies have compared drug dosing recommendations based on the Cockcroft-Gault (CG) and the Modification of Diet in Renal Disease (MDRD) Study equations, finding higher values for glomerular filtration rate estimated using the MDRD Study equation and subsequently higher dosing recommendations....

We examined changes in prescribing practices resulting from this recommendation and found that prescribing of adamantanes declined nationwide, with these drugs accounting for approximately 40% of the antivirals prescribed for influenza from 2000 to 2005 and only 2% in 2006. This finding provides evidence of a rapid change in clinical practice associated with the dissemination of treatment guidelines. Evaluating the effectiveness with which public health recommendations are translated into practice is important given the ongoing emergence of resistance to antiviral drugs and a novel H1N1 influenza virus.

A chemically modified carbon-paste electrode has been described for the sensitive and selective determination of amantadine. Beta-cyclodextrin was used as modifier. The electrode shows a sub-Nernstian response of 51.0 +/- 1.0 mV decade(-1) for amantadine in the concentration range of 6.3 x 10(-10)-7.1 x 10(-7) M at 25 degrees C. To investigate the frequency of amatadine resistance among influenza A viruses isolated in Korea during the 2003-2009 seasons, 369 (16.8%) 2,199 A/H1N1 viruses and 780 (14.8%) of 5,263 A/H3N2 viruses were randomly selected. The M2 and HA1 genes of each isolate were amplified by reverse transcription-polymerase chain reaction and followed by nucleotide sequencing. This report demonstrates that a triple combination of antivirals was highly synergistic against influenza A. Importantly, the synergy of the triple combination was 2- to 13-fold greater than the synergy of any double combination, depending on the influenza subtype. These data support the investigation of a novel combination of oseltamivir, amantadine, and ribavirin as an effective treatment for both seasonal and pandemic influenza, allowing efficient use of the existing drug supplies.

Adamantanes have been used for the prophylaxis and treatment of Influenza A virus (IAV) infections worldwide. However, they have limited use because of increasing number of resistant viruses during recent years. In investigating the frequency of amantadine-resistant IAVs (H3N2) circulating in Iran in 2005-2008, we found that M2 sequences of recently circulating viruses that were amantadine-resistant contained a Ser31Asn mutation. There are four influenza antiviral drugs approved for use in the United States (oseltamivir, zanamivir, amantadine and rimantadine). The swine influenza A (H1N1) viruses that have been detected in humans in the United States and Mexico are resistant to amantadine and rimantadine so these drugs will not work against these swine influenza viruses.
