[Free temporoparietal flap in hand coverage. Report of three cases.]

Hands defect coverage needs thin and pliable flaps. Few free flaps such as free temporoparietal flap are adequate. It provides moderate donor scar and unique range of motion for tendinous coverage. We expose three cases of hand reconstruction: two dorsal coverage with tendinous exposition and reconstruction and one thumb coverage. The postoperative results were satisfactory concerning hands. One patient developed alopecia and dysesthesis on scalp. We think that this flap is a good alternative for serratus free fascial flap.

DISCUSSION: The stenosing tenosynovitis of the first dorsal extensor compartment of the wrist is a relatively frequent pathology in the young woman. We use a longitudinal surgical approach to avoid the radial nerve lesions. Le Viet procedure using the palmar flap of the pulley fixed to the dermis works as a barrier and maintains the tendons sliding on the radial styloid groove.

CONCLUSION: CVG can be treated surgically with resection of the thickened excess skin in coronal and sagittal axis. Scalp lift must be effective all over the different areas of the scalp. The scalp flaps must have a reliable vascularisation. Combined incisions of the galea help to treat the residual folds. The excision pattern must be reproductible, as this disease is progressive.

Skin graft is the most common and simple procedure to cover superficial defect. Skin of variable thickness and size is completely detached from its origin (donor site) to cover a defect (recipient site). This simple procedure is the result of a long and eventful technical and theoretical evolvement. The aim of this article is to retrace the history of skin grafting, from its discovery until today.

CONCLUSION: Tarsomarginal grafts in eyelid reconstruction provide multiple advantages. With this simple and versatile technique, wide and complex eyelid repairs can be managed in a one-step surgery. This procedure is very reliable; it gives good functional and aesthetical results. Hübner's technique is a major tool in eyelid reconstructive surgery.

Hibernomas are uncommon tumors composed of brown fat. Their consistency is multiple but usually harder than a regular lipoma. It is a mobile, slow-growing mass. Clinical symptoms are exceptional (pain or massive weight loss). The case of a 26 years old patient who suffered of a cervical hibernoma is reported to discuss the clinical and paraclinical aspect of the diagnosis.

CONCLUSION: Those who promote the vertical incision have the greatest drive to encourage other plastic surgeons to abandon the traditional mutilating boat anchor scar. Nevertheless, they should always remember the deforming effect caused by exaggerated vertical incisions which may be avoided by transforming it into an inverted T with short horizontal branches, a small price to pay to obtain a superior aesthetic result.

We report the case of a chest-wall correction of a young male patient by a custom made silicone implant, detailing the original process of manufacturing, and justifying the choice not to use a conventional technique of reconstruction. The author comments the studies about midfacial ageing of Lambros as well as those of Rohrich and Pessa. The ageing becomes obvious through the autonomization of all the anatomical subunits of the midface and particularly the appearance of the limits of these areas. It seems that the loss of volume is not global but area-dependant. CONCLUSIONS: The muscle-sparing latissimus dorsi flap, pedicled on descending branch, is versatile and reproducible. It results in minimal functional deficit of the donor site, absence of seroma, large freedom of orientation of the skin paddle, low rate of flap complications, and a cosmetically acceptable scar. There are a lot of indications in breast reconstruction.

Since 2 years, the author uses an aesthetic mammary augmentation technique with a combination in the same surgical procedure of: placement of a breast implant in a retrofascial plane; lipofilling under the skin of the cleavage to cover the upper part of the implant. Forty-seven women have been operated with this procedure since 2006, with immediate satisfactory results in all the cases. We consider this procedure as an interesting alternative to retromuscular placement in thin patients with small breast.

Many studies showed we have a visceral adipose tissue and a subcutaneous adipose tissue. Roles and risks of these fats are different. The purpose of this work is to demonstrate that there are at least two types of subcutaneous fat; one is "metabolic" and diet sensitive and another genetic which is diet resistant. CONCLUSION: The "Chippendale" technic is a quick, cheap and easy learned procedure, efficient for preventing chronic seroma after the latissimus dorsi flap. The postoperative recovery is eased and the patients comforted.

In conclusion, fat grafting appears to be a simple and easily reusable technique which makes it possible to obtain the best morphological and cosmetic results in irradiated patients, whereas avoiding complex and potentially hazardous procedures.

[Leech-borne infection on a TRAM flap: A case report.]

We report a new case of delayed leech-borne infection in mammary reconstruction by a Transverse Rectus Abdominis Myocutaneous flap (TRAM) flap, which caused the flap loss. The use of prophylactic antibiotics is a way to prevent A.hydrophila infection (third generation cephalosporin, ciprofloxacin). This antibioprophylaxy must be followed until wound closure of the venous congested tissue.

The author describes a technical simplification of the lenghtening temporalis myoplasty based on the temporal muscle anatomical structure which is featherlike and after having observed the muscle lenghtening in more than 100 lengthening temporalis myoplasty. The major modification consists in avoiding superficial undermining of the anterior part of the temporal muscle. The surgery is easier, quicker and the postoperative care are simplified. The coronoid process is osteotomised throught the nasolabial approach avoiding the zygoma's osteotomy.

CONCLUSION: For defect loss of no more than 30mm in emergencies, the authors propose to use vein grafting. In other situations, the surgeon must take into account the patient's profile and the hemi-pulp concerned, dominant or non-dominant, before opting for a nerve or a vein graft.

We report our experience of 20 patients operated in 2006 and 2007, with one year minimal follow-up. All the cases are relative or absolute saddle noses, idiopathic, post-traumatic or iatrogenic. Moreover, two patients also had a concomitant chin augmentation with DC-F. No case of displacement or resorption was noted. We modified certain technical points in order to make this technique even more powerful and predictable.

Necrotic angiodermatis is a quite frequent ischemic ulceration found in various skin microvessels diseases. It usually occurs in elderly over 60 years particularly on legs. Lesions are rapidly spreading and very painful, without any response to common pain medicines. We want, through our 21 cases series, to confirm the analgesic effect of early excision and skin grafting.

The orbital septum, a well-known structure since a long time ago, is a controversy object anyway. Its physiological, anatomical and pathological importance is undeniable. However, it will be difficult not to consider it as a part of the whole structure which organizes the general system of the soft tissues of the face, in which the orbital frame is one of the pivots.

The MACS-lift technique, from Tonnard and Verpaele, is a simple procedure for mediofacial aging. They use three purse-string sutures. The anchor points are within the deep temporal fascia. Sagging facial soft tissues are suspended with this loop. This procedure needs only local anesthesia. The postoperative following is simple. The results are natural and interesting. We analyse this technique and its efficiency in the medial part of the face.

We report 10 cases of free tissue transfers with a one-stage technique lengthening the vascular pedicle of the free flap with interpositional vein grafts. Taking into consideration the good results and the low rate of morbidity, the authors emphasize the use of this technique rather than a two-stage procedure.
