Role of Computed Tomography in the Surgical Management of Patients with Bowel Obstruction Secondary to Recurrent Ovarian Carcinoma
Email this article to a colleague.

Discuss or comment on this article.
Conclusions CT scan seems to be helpful in patients with a solitary site as the cause of bowel obstruction. All the patients in our study
with this finding had a successful palliation. On the other hand, successful palliation is still feasible in the presence
of peritoneal carcinomatosis on CT scan; therefore, this finding alone should not be the reason to avoid surgery in well-selected
patients.

Feasibility of a Novel Approach to Inguinal Lymphadenectomy: Minimally Invasive Groin Dissection for Melanoma
Email this article to a colleague.

Discuss or comment on this article.
Conclusions Minimally invasive inguinal lymphadenectomy is feasible for patients with melanoma as demonstrated by nodal yield and visual
inspection. This technique may reduce complication rates and wound dehiscence, and the risk of exposed vessels is minimized
by eliminating the inguinal incision. This obviates the need for routine sartorius muscle transposition. A prospective, randomized
trial comparing the open versus the videoscopic approach is currently in progress.

Conclusions Breast cancer patients with SLN micrometastases in whom ALND was omitted had a very low locoregional failure rate. This study

provided to treat node-positive disease is administered. However, longer follow-up and results of additional prospective studies

Conclusions The Chamberlain mini anterior thoracotomy provides direct access to the pulmonary hilum, facilitating dissection and vascular
control for large and central tumors. Reduced perioperative pain and down-lung syndrome compared to lateral approaches can
be achieved. Muscle function is preserved, and intrapericardial/hilar access is expeditious. This approach enhances hilar
access and avoids the vascular control and hilar exposure challenges inherent with lateral thoracotomy.

Hypoxic Single-Pass Isolated Hepatic Perfusion of Hypotonic Cisplatin: Safety Study in the Pig

Conclusions A hypoxic single-pass isolated liver perfusion with hypotonic cisplatin is feasible and safe. Effluent from the liver is highly
cytotoxic on cancer cells. A clinical study with 50 mg/L of hypotonic cisplatin is warranted in patients with unresectable
liver metastases from colon cancer.

Conclusion Placement of self-expanding stents is highly effective for instant dysphagia relief, enabling adequate oral nutrition during
neo-adjuvant therapy, but is limited by a high re-intervention rate.

Predictive Factors of Response to Decongestive Therapy in Patients with Breast-Cancer-Related Lymphedema

Conclusions This study shows that compliance to bandages during CDT is one of the most important predictors of response. Moreover, data
support the idea that more severe lymphedemas have a worse response to treatment, and it should be recommended in early stages.
The association between the season of treatment and response was also very strong, so weather conditions are an additional
factor that must be taken into account in further studies.

Systematic Review of Randomized and Nonrandomized Trials of the Clinical Response and Outcomes of Neoadjuvant Systemic Chemotherapy for Resectable Colorectal Liver Metastases

Conclusion Current evidence suggests that objective response to neoadjuvant chemotherapy may be achieved with improvement in DFS in patients
with resectable CLM. A prospective randomized trial of neoadjuvant therapy versus adjuvant therapy after liver resection is
required to determine the optimal perisurgical treatment regimen.

Conclusions The finding of a high proportion of R0 resection among all resections performed confirms the activity of neoadjuvant radiochemotherapy
and should not be neglected. Based on these data, patients with unresectable pancreatic cancer without disease progression
after chemoradiotherapy should be considered for radical surgery.

Conclusion Pathologically negative nodes (as compared with pathological positive and not assessed), along with pathological and biological
features, independently correlated with increased survival following comprehensive treatment. This suggests the need for careful
node sampling when performing surgical cytoreduction for DMPM patients.

Impact of Selection Bias on the Utilization of Adjuvant Therapy for Pancreas Adenocarcinoma

Discussion One-third of patients in this contemporary dataset of patients did not go on to receive adjuvant therapy. The likelihood of
receiving adjuvant treatment is negatively impacted by the course of postoperative recovery. Moreover, the fact that adjuvant
therapy was undertaken less often for older patients and patients with favorable pathological features highlights the selection
bias impacting the decision to pursue postoperative therapy for this disease. This selective utilization of postoperative
therapy for patients with adverse oncological characteristics is likely to bias any retrospective analys...

Conclusions The recurrence rate of the index group (50%) in the current series, the overwhelming majority of whom did not receive adjuvant
mitotane, is indistinguishable from that reported for those who received adjuvant mitotane (49%) in the recent interinstitutional
report, emphasizing the importance of completeness of initial surgery in the management of patients with ACC.

Conclusions Tumor size and surgical margins affected outcome only on initial presentation. However, wide surgery was feasible in a minority
of cases. Most patients died of local–regional disease even when metastases occurred. Indeed, long-term prognosis was such
that disease-free survival at 10 years was only 26%.
