Patients with severe symptomatic aortic stenosis, who from November 2008 to March 2009 were treated with Edwards-Sapien transcatheter aortic valve implantation (TAVI) within the POL-TAVI First Polish Registry, were included in the analysis. Nineteen patients aged 78±4,8 years with high operation risk and Logistic EuroSCORE 25 ± 7.6% were reported (74% were females). In 15 (79%) patients the valve was implanted transapically (TA), in the other four (21%) – via the femoral arterial access (TF). Conclusions: The diagnostic and therapeutic strategy of urgent angiography of coronary arteries and grafts in the event of perioperative ischaemia after coronary artery surgery (in the first 24 hours after operation) allows the precise diagnosis and establishment of optimal management. In more than 40% of pts percutaneous intervention is possible with a high success rate (above 85%). 80% of patients with acute perioperative ischaemia can avoid reoperation and those referred for repeat cardiac surgery had established indications and the range of reintervention. Conclusions: Levels of anxiety and depression are lower a few days after on-CABG compared with values obtained before the surgery. Post-operative anxiety and depression positively correlate with baseline values. Determinants of pre-operative depression are: operative risk, previous myocardial infarction and low ejection fraction. Post-operative anxiety and depression are positively correlated with subjects’ age, angina symptoms and extent of the surgery. Conclusions: The therapy of pleural empyema requires a differential approach with regard to: general state of patient, dimensions and empyema location, disease duration, coexisting lung pathology and surgical experience. Superior results are achieved with early stages of pleural empyema using antibiotic therapy, pleural drainage along or with the administration of fibrinolytics or videothoracoscopy. The authors present a case of a 59-year-old man admitted to the hospital with the diagnosis of acute pulmonary embolism. After an additional diagnostic process including multislice computed tomography of pulmonary arteries, chest and abdomen the patient was sent to surgery where, besides the emboli, a tumour of the pulmonary trunk and pulmonary arteries was removed. On the basis of immunohistochemistry the diagnosis of leiomyosarcoma, high grade, was established. A lack of such information results in the fact that a patient has not received full information as to a planned operation, and his/her agreement to the surgery is invalid. A doctor performing an operation without a patient’s agreement may be sued by the patient for damages on the basis of acts written in the civil code. Conclusions: Ultrastructural investigations indicate that the probable mechanism of cardiomyocyte injury in TOF and HLHS groups is closely connected with apoptosis activated in the mitochondrial pathway. Morphological changes of intercalated disks also suggest the possibility of injury of CAM proteins, responsible for normal organogenesis and forming the anchoring apparatus of myofilaments. Air leak is one of the most common complications occurring after pulmonary resection. Risk factors as well as methods of prevention and surgical treatment are discussed. Based on the author’s experience and overview of the literature the following conclusions were drawn: preoperative identification of high-risk patients, careful dissection of pulmonary fissures and immediate surgical suturing of parenchymal and bronchial air leak together with the use of sealants are the most important factors leading to significant reduction of postoperative pulmonary air leak. Delayed contrast-enhanced magnetic resonance imaging (DE-MRI) with use of gadolinium-based contrast enables assessment of postinfarction scar size in the left ventricle wall. It is known as transmural extent of hyperenhancement (TEI) and is the percentage ratio of scar thickness to total end diastolic wall thickness in a given segment. Conclusions: A man older than 67, with right sided large cell carcinoma that has stage higher than IIa, is the worst candidate for pneumonectomy concerning complications and long-term survival. Conclusions: The use of complex grading systems is unrealistic in everyday work by Polish ENT surgeons and thoracosurgeons. We suggest implementing the Cotton system and Medical Research Council dyspnoea scale as the optimal way to classify and assess the outcomes of LTS treatment. The former system is well known and widely advocated in Poland, the latter requires a modicum of time from the physician. Both scales, as a combination of subjective and objective parameters, show a strong correlation and therefore complement each other. Conclusions: The fundamental treatment of tracheal stenosis is the surgical approach. The endoscopic management (dilatation and laser) need repeated procedures and in some cases tracheotomy had to be maintained. Better recovery, short hospitalization and excellent results were obtained with the cricotracheal segmental resection. Cricotracheal resection is a reliable and versatile technique for the reconstruction of the subglottic airway, almost regardless of the underlying cause. Thoracic aortic aneurysm after heart transplantation is a rare but serious complication. The authors describe 3 cases (0.6%) of thoracic aortic aneurysm during 20 years of follow-up on 485 heart transplant patients at their institution. Dissected and non-dissected thoracic aortic aneurysms, diagnosed 23 to 38 months post transplantation, were treated with surgical repair or repeat transplantation.
