Successful treatment of locally advanced anaplastic thyroid carcinoma by chemotherapy and hyperfractionated radiotherapyemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Abstract: The authors report a case of anaplastic thyroid carcinoma which was treated with chemotherapy and radiotherapy and proved to achieve pathological CR (complete response) after a radical operation. A 53-year-old female presented with a mass of about 4cm in diameter arising from the left lobe of her thyroid and surrounding her cervical esophagus. Pre-operative aspiration cytology and open biopsy revealed the tumor was a giant cell anaplastic carcinoma. There was no sign of distant metastasis. Two cycles of chemotherapy were very effective in treating the tumor but regrowth appeared to occur during hyperfractionated ...

We report a case of low-grade cribriform cystadenocarcinoma. The patient was a 32-year-old woman who presented with a mass in the right parotid region. Computed tomography and magnetic resonance imaging revealed a polycystic mass in the right parotid gland. The preoperative fine needle aspiration biopsy showed no malignancy, however the frozen section during surgery showed carcinoma. The patient subsequently underwent total parotidectomy with preservation of the facial nerve. Microscopically, the tumor had a typical feature of intraductal proliferation. There was a golden brown pigment and PAS-positive/diastase-resistant c...

Adenosquamous carcinoma of the larynx associated with necrotizing sialometaplasia—A diagnostic challengeemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
We report a rare case of necrotizing sialometaplasia in the larynx, probably initiated by an underlying malignant process. We suggest an aggressive diagnostic approach, when necrotizing sialometaplasia involves the larynx and no recent exposure to radiation, surgery or trauma has been recorded. Necrotizing sialometaplasia of the larynx should be regarded as secondary to malignancy until proven otherwise. (Source: Auris, Nasus, Larynx)

We present the case of a patient who suffered from indolent cervical nodular tumour. Biopsy and histopathological workup showed parts of a poorly differentiated SRCA with p53 overexpression and mutations. Immunostaining gave no further hints for the origin of the malignancy. Thorough staging revealed an extended tumour of the oropharynx as primary origin. The definitive surgical therapy consisted of a transoral tumour resection with CO2-laser and bilateral neck dissection. Final classification was pN2c cM0 G3 R0 L1 V0. Adjuvant fractioned radiotherapy with 66Gy was applied because of bilateral lymph node metastases and ext...

We report a case of an 80-year-old man who was referred to our clinic with a chief complaint of dysphagia and muscle weakness in all four limbs. The patient was diagnosed with polymyositis based on pathological findings, muscle weakness, electromyogram findings, and an elevated creatine phosphokinase level. The patient was also positive for HLA-DR3. Intravenous predonine administration was initiated, but dysphagia was not improved. We considered a cricopharyngeal myotomy, but this could not be performed because of heart failure. Endoscopic balloon dilation was performed and dysphagia improved on the same day. Therefore, we...

Abstract: Ocular flutter is a relatively rare eye movement disorder characterized by intermittent bursts of conjugate horizontal saccades without an intersaccadic interval. Its anatomical and neurophysiological basis remains controversial. Here we present a case of ocular flutter in which the flutter declined as the clinical stage of the associated ailment declined, until it eventually disappeared. Radiological examination revealed a tiny lesion in the right upper pontine tegmentum that included areas of the reticular formation, superior cerebellar peduncle and locus ceruleus. Because the lesion occupied the upper part of ...

Conclusions: Postural restrictions do not increase the efficacy of the canal-repositioning maneuver despite the fact that patients report a subjective improvement after post-procedural instructions. In the review of the literature, all studies except one conclude that postural restrictions are unnecessary. However, a number of methodological issues such as inadequate sample size are not addressed and more conclusive evidence is required. Based on current evidence, the use of postural restrictions after the canal-repositioning maneuver is unjustified. (Source: Auris, Nasus, Larynx)

This report describes a patient with thyroid cancer who demonstrated a complete response to gefitinib. The patient was a 56-year-old woman. She had been treated for lung cancer. The lung cancer had initially disappeared following chemoradiotherapy. However, a few months later, a distant metastasis was detected on the opposite lung field. Thyroid cancer was then incidentally observed when these lesions were examined. Fine needle aspiration cytology of the thyroid tumor with ultrasonography (US) showed papillary carcinoma. Finally, all of the lesions were treated with gefitinib. Gefitinib demonstrated a remarkable effect on ...

Conclusion: In endoscopic endonasal DCR, the closure of bare bone with a posteriorly based nasal mucosal flap that creates an anastomosis between the lacrimal sac mucosa and the nasal mucosa decreases the formation of granulation tissue. But, there is no significant difference of success rate between two groups. (Source: Auris, Nasus, Larynx)

Abstract: Objective: To assess the advantage of binaural and bimodal hearing for subjects with cochlear implant (CI) using auditory event-related potentials as well as speech perception tests.Subjects and methods: The subjects comprised four binaural CI users (CI/CI group) and eleven bimodal CI users, who wore a hearing aid (HA) contralaterally (CI/HA group). All subjects had used binaural or bimodal fitting for over 6 months. Their speech perception was examined in a quiet environment using monosyllabic words. Late cortical waves were measured while subjects were engaged in an oddball task of 1kHz frequent and 2kHz rare t...

Conclusion: : Significant improvement in THI and VAS scores was found as early as 1 month of treatment and later compared with those on initial examination, suggesting that TRT with a TCI may be an effective treatment for tinnitus. It resulted that many patients chose the speech noise or the white noise. And also it was indicated that noise generators set at just below mixing point with tinnitus are more effective. In this study, however, speech noise was often selected probably because of the reduced output at high frequencies and the level of comfort. As white noise produces greater sound volume, patients tended to switc...

Prevalence of acute otitis media among children with pyrexia in a Nigerian hospitalemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Conclusion: AOM with late presentation in the suppurative stage of the disease is a common cause of pyrexia in children with a male preponderance and two third of the children were from the low socio-economic status. There is the need for otoscopic examinations of all pyretic children as the resultant hearing loss is related to difficulties in language acquisition in children below 2 years of age with effects on literacy and school achievement. Conclusion: We clearly showed the biofilm formation of clinical NTHi isolates from AOM children. In addition, the biofilm formed by NTHi would play an important role in persistent or intractable clinical course of AOM as a result of lowered treatment efficacy of antibiotics. Abstract: Objective: To elucidate the role of facial recess bony plate in the thermal transmission route from external auditory canal to lateral semicircular canal during caloric stimulation test, we performed the test on patients before and after removal of the plate, i.e. posterior tympanotomy. In the present study, we adopted facial nerve decompression (FND) as posterior tympanotomy without surgery-induced inner ear damages and cochlear implantation (CI) as posterior tympanotomy with surgery-induced inner ear damages. Methods: Between 1999 and 2003, we performed FND on 19 patients with unilateral facial nerve paresis due...

Paraneoplastic syndromes in patients with nasopharyngeal canceremail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Abstract: Paraneoplastic syndromes (PNS) represent the clinical manifestation of the remote and indirect effects produced by tumor metabolites or other products. Paraneoplastic effects are not directly mediated by tumor invasion of normal tissue, or by the disruption of normal function of the involved organ, or by distant metastases. More than 260 cases of nasopharyngeal carcinoma (NPC) associated with PNS have been reported in the literature. These syndromes can be divided into six main groups: cutaneous or dermatologic, endocrine, hematologic, osteoarticular or rheumatologic, neurologic, and ocular. The most common derma...
