Gastrinoma is one of the most symptomatic pancreatic endocrine tumors and is associated with recurrent gastric or duodenal ulcer and an induction of chronic gastric acid hypersecretion. This tumor is commonly involved within the gastrinoma triangle. Herein we report a peripancreatic lymph node gastrinoma in a 56-year-old man who was suffered from a recurrent duodenal or jejunal ulcer perforation. We report a 62-year-old man who presented with general weakness and poor oral intake for 6 months, and showed multiple masses at the duodenum, jejunum, ileum and the ascending colon. The patient underwent right hemicolectomy with small bowel segmental resection. Microscopically, the tumor masses were characterized by incohesive sheet of neoplastic cells with vesicular nuclei, large nucleoli, and abundant eosinophilic cytoplasm. Immunohistochemical studies showed strong reactivity of the tumor cells for pan-cytokeratin, anti-endomysial antibodies (EMA) and vimentin. Activated charcoal is commonly used in the treatment of patients experiencing an oral overdose of drugs and is considered to be a safe, inert agent. However, various complications associated with the use of activated charcoal have been reported, with acute respiratory failure being a fatal one. In the presenting case, a 29-year-old female arrived at an emergency room having taken an overdose of drugs. When a nasogastric tube was inserted for gastric lavage and charcoal administration, she vomited and respiration ceased. The present paper reports a rare case of granulocytic sarcoma (GS) with plasmacytoid features occurring in the maxillary sinus of a 63-year-old man. The diagnosis of GS has always been difficult, and diagnosis in the present case was made more difficult by the tumor's manifestation as longstanding maxillary sinusitis, the late development of leukemia, unusual location of the tumor, and the plasmacytoid features of the tumor cells. Iatrogenic fatal bleeding of ruptured gastric varix mimicking submucosal mass: An autopsy caseEmail this article to a colleague. Save this article to My Clippings. Discuss or comment on this article.
A 66-year-old woman died during a routine gastroscopic examination. The physician tried to biopsy a submucosal mass-like lesion with tortuous and exaggerated submucosal blood vessels. Massive bleeding occurred due to iatrogenic injury. Isolated gastric varices are less prevalent than esophageal varices, but it has a higher motality rate because of the difficulty of controlling bleeding. We report a case of metastatic MTC presented as palpable neck and breast nodules in a 39-year-old woman. Fine needle aspiration of breast nodules showed a cellular smear with relatively homogenous, poorly cohesive tumor cells in a relatively clean background. The tumor cells were round to oval, with hyperchromatic nuclei, abundant cytoplasm and plasmacytoid in appearance. A few amorphous materials, possibly amyloid, were also noted. We describe the immunohistochemical findings of the skin lesion of Kikuchi's disease (KD) in two patients. They presented with fever, lymphadenopathy, and eruptions on the skin of palm and face.
