One of the benefits of being a freelance writer and consultant is being able to live anywhere. I live near the beach, which is where I head whenever the surf is up. The Carolina coast is wonderful, but it is home to some jellyfish that can leave a painful sting. We encountered one of these today. How did we know? Not that many things will sting like a hundred wasps at once out in the surf. We didn't get any tentacles stuck to us, which would have been another way to tell it was a jellyfish. CONCLUSION: Of the 17 drug combinations tested, 5 combinations were observed to be visually incompatible during simulated Y-site injection. The combinations found to be visually incompatible included acyclovir with cyclosporine, diphenhydramine, gentamicin, granisetron, or metoclopramide.

CONCLUSIONS: When applied topically to rabbit corneas, 5% diphenhydramine solution has a significant anesthetizing effect compared with salt solution (control eyes). Topical diphenhydramine may be a safe alternative in patients requiring topical anesthesia who have multiple allergies to topical anesthetics. Additional studies are needed to determine a dose-response curve and to further evaluate corneal toxicity prior to use in humans.

CONCLUSIONS: Celecoxib can produce an anaphylactic reaction in patients who have previously tolerated sulfonamide antibiotics and who have previously tolerated celecoxib. This case also reviews the potentially biphasic presentation of anaphylaxis. Clinicians need to be aware of this biphasic anaphylactic response to ensure optimal duration of evaluation.

A 23-year-old pregnant woman at 38 weeks of gestation presented with a 2-day history of a red eruption on her abdomen and upper thighs. The patient had applied baby oil to her abdomen a few days earlier, but she denied using any other topical products. Her only medications were prenatal vitamins and iron, which she had been taking since the beginning of her pregnancy. Conclusion: Trastuzumab can be safely infused over 30 minutes for 4 and 6 mg/kg maintenance doses and this is the current practice at our institution. This change has resulted in improved efficiency and patient convenience, and decreased cost associated with the longer duration infusions.
